Short answer: an Apple Watch can display glucose readings today, but it does not independently measure blood glucose. Dexcom’s authorized G7 system can send readings and alerts directly to a compatible Apple Watch; the sensor doing the measuring is still worn on the body and penetrates the skin. The “breakthrough” reports instead combine that existing integration with an investigational breath device and unconfirmed reports of Apple optical-sensing research.
What the January 2026 “breakthrough” story actually describes
The headline refers to three different developments, not one confirmed Apple component.
- Available now: Dexcom G7’s Direct to Watch feature sends data from an external continuous glucose monitor (CGM) to Apple Watch.
- Investigational: PreEvnt’s isaac is a pendant-style breath-analysis device intended to provide glucose-related alerts. PreEvnt announced clinical trials at Indiana University Indianapolis on November 18, 2025.
- Reported Apple research: January 2026 coverage described alleged work by Apple’s Exploratory Design Group on silicon photonics and optical absorption spectroscopy. The reported proof-of-concept was approximately iPhone-sized and worn on the upper arm, with miniaturization presented as a long-term engineering goal.
Those facts do not establish that Apple has a working glucose sensor for an Apple Watch, a launch schedule, a supported model, accuracy data, or a regulatory filing. The Apple work remains reported and unconfirmed, while isaac is a clinical-trial project rather than an approved consumer CGM.
The original January 9, 2026 report discusses all three threads, but they should not be presented as a single Apple product pipeline.

Can an Apple Watch measure glucose today?
No—not by itself. Apple Watch can serve as a screen for an authorized CGM. In Direct to Watch mode, the Dexcom G7 transmitter communicates with the watch over Bluetooth, so the watch can show the latest glucose value, trend direction, graph and alerts even when the iPhone is not nearby.
The distinction matters:

The FDA says no smartwatch or smart ring has been authorized, cleared or approved to measure or estimate blood glucose independently without piercing the skin. The agency warns that inaccurate values could lead to incorrect insulin or glucose-lowering medication decisions and dangerous hypoglycemia.
What Direct to Watch does—and what it does not do
Direct to Watch is an approved CGM-display capability, not a sensor hidden in the watch. A compatible smartphone is still needed to pair and start a new G7 sensor, even though the watch can subsequently receive readings directly from the transmitter.
Setup route for Dexcom G7
- Open the Dexcom G7 app on the iPhone.
- Open Connections.
- Select Direct to Watch.
- Follow the on-screen pairing instructions.
- If the watch app is not installed, open the iPhone’s Watch app, choose My Watch, find Dexcom G7 under available apps and install it.
When the direct connection is active, the watch can show the current reading, trend arrow, trend graph and glucose alerts. Dexcom’s setup instructions and smartwatch FAQ describe the supported behavior.

The orange phone icon is a limitation, not a new measurement mode
An orange phone icon indicates that Direct to Watch is temporarily unavailable and the watch is receiving copied data through the iPhone instead. Keep the phone nearby until the icon disappears. A connection problem can therefore affect the watch display even though the CGM sensor itself remains on the body.
The FDA’s G7 documentation describes a wear period of up to 10 days plus a 12-hour grace period and a reportable range of 40–400 mg/dL; values outside that range appear as “LOW” or “HIGH.” These are properties of the Dexcom system, not of Apple Watch hardware.
What is PreEvnt isaac?
PreEvnt describes isaac as a noninvasive glucose-alert device that analyzes exhaled breath, particularly acetone and other compounds that may change with glucose-related metabolism. The company announced that the system entered clinical trials at Indiana University Indianapolis under Dr. Mangilal Agarwal in November 2025.
That announcement means the device is being studied. It does not establish FDA clearance or approval, clinical accuracy equivalent to a CGM, commercial availability, or suitability for insulin dosing. The word alert is also important: an alert system may flag a condition or trend without producing the continuous, treatment-grade glucose curve users expect from a CGM. Its intended use, performance metrics and regulatory status should be confirmed in formal labeling and FDA records before relying on it.

What Apple is reportedly investigating
The January 2026 report attributes Apple’s work to silicon photonics and optical absorption spectroscopy: lasers and optical components would analyze changes through the skin without an inserted filament. The described prototype was far larger than a watch and reportedly worn on the bicep.
Reducing that apparatus to a wristwatch would require solving size, power, heat, waterproofing, optical alignment and signal-quality problems simultaneously. Nothing in the cited reporting confirms a public prototype, final sensor design, product announcement, accuracy results, regulatory submission or release date. “Apple is reportedly exploring” is therefore accurate; “Apple Watch will soon measure blood sugar” is not.

- Weak signal: Glucose concentrations are low compared with many other substances in tissue, making the target signal difficult to isolate.
- Biological variation: Skin tone, hydration, temperature, tissue thickness and placement can alter optical measurements.
- Motion: A moving wrist changes contact and optical geometry, adding noise during exercise and ordinary activity.
- Timing: Interstitial-fluid glucose is not identical to a direct blood sample and can lag during rapid rises or falls.
- Changing conditions: Meals, illness, dehydration, exercise and medication changes challenge calibration across users and over time.
- Safety threshold: A watch must avoid false highs and lows that could prompt harmful treatment decisions.
- Wearable constraints: The sensing system must fit within a small, cool, low-power, water-resistant product while producing medically reliable data.
A laboratory correlation or a clinical-trial enrollment is only one step. Regulatory authorization requires evidence that the finished device performs safely for its specifically labeled use.

What has actually been authorized?
FDA records list Dexcom G7 systems, including versions supporting the G7 Watch App and Direct to Watch. A later Devices@FDA record dated June 8, 2026 covers the G7 and G7 15 Day submission and describes the Apple Watch application and direct mode (K261402). The earlier G7 decision summary is K234133.
Those clearances authorize the CGM ecosystem and its display functions. They do not authorize an Apple Watch to measure glucose independently. PreEvnt’s trial announcement likewise does not demonstrate FDA authorization for isaac.

If you need glucose data now
The established route is a compatible Dexcom G7 system with Apple Watch integration. It provides real-time readings and alerts and can be useful when carrying an iPhone is inconvenient. Confirm model, operating-system, app, regional, prescription and insurance requirements before starting. Remember that the G7 remains an external sensor worn through the skin; it is not a needle-free watch.
If a product claims “no needles”
Do not use a smartwatch or ring that claims to measure glucose independently for insulin or other treatment decisions unless its exact intended use is backed by appropriate regulatory authorization. “FDA registered,” “FDA listed” and “FDA compliant” are not interchangeable with FDA clearance or approval.

If readings stop updating
Check the watch connection state, the orange phone icon, the phone’s proximity and the Dexcom app. A stale value can look reassuring while no longer being current. The FDA has documented a Dexcom iOS-app recall involving a bug that could leave stale glucose data displayed; concurrently connected displays could continue receiving transmitter data, so users should follow the manufacturer’s current troubleshooting and safety instructions rather than rely on an old screen value.
Bottom line
Today: Apple Watch can display glucose from an external, authorized CGM such as Dexcom G7, including directly from the transmitter when Direct to Watch is working. Not verified: a commercially available Apple Watch that measures glucose on its own without a sensor inserted through the skin. Future: Apple’s reported optical research and PreEvnt’s breath-based isaac show that needle-free approaches remain active areas of investigation—but neither provides a verified Apple Watch launch or a replacement for an authorized CGM.
