Sometimes—but not in the way the headline may suggest. Apple Watch’s Vitals app can flag overnight changes that sometimes occur when you are becoming ill, but it cannot reliably predict illness, identify an infection, measure a fever, or diagnose COVID-19, flu, or a cold.

Think of Vitals as a personal trend dashboard. It can tell you that several measurements are unusual for you. It cannot tell you why they changed or whether you are contagious.

What the Apple Watch Vitals app actually does

Vitals uses overnight Apple Watch measurements to establish typical personal ranges. It can track:

  • Heart rate
  • Respiratory rate
  • Wrist temperature
  • Blood oxygen, where supported and available
  • Sleep duration

Rather than comparing everyone with one universal “healthy” number, the app compares your results with your historical baseline. When multiple measurements move outside their usual ranges, Vitals may send a notification and offer possible explanations, including illness, medication, alcohol, elevation changes, or other factors. An “out of range” result means only that a measurement is unusual for you. It does not mean you definitely have a fever, COVID-19, influenza, pneumonia, or another infection.

Can Vitals warn you before you feel sick?

It can sometimes identify a change before you consciously notice symptoms. Infection and other physiological stresses may affect resting heart rate, heart-rate variability, breathing, temperature trends, sleep, and recovery.

However, the most accurate description is deviation detection, not illness prediction. Vitals identifies that your current overnight pattern differs from your normal pattern. It does not forecast with certainty that an illness is about to begin.

A higher overnight heart rate, for example, may be related to infection—but also to dehydration, alcohol, stress, poor sleep, medication, hard training, or recent travel. A change in respiratory rate may reflect congestion or illness, but it can also be associated with anxiety, asthma, altitude, disrupted sleep, or imperfect measurement.

What research shows—and what it does not

Research supports the general idea that wearable data can contain signals associated with infection. It does not establish that the consumer Vitals app is a clinically validated illness detector.

Apple Watch research

The Warrior Watch study found changes in heart-rate-variability patterns among healthcare workers who later received a COVID-19 diagnosis. This used an Apple Watch and a research analysis, not the Vitals app’s consumer notification system.

Another study of nearly 5,300 smartwatch users found physiological or activity changes in 26 of 32 COVID-19 cases, with some changes appearing before symptoms. That was a retrospective analysis using research methods—not proof that Vitals will identify illness with the same performance. See the Nature Biomedical Engineering study.

Apple says its Vitals outlier algorithm was developed using real-world information from the Apple Heart and Movement Study, as explained in its WWDC24 presentation. Apple does not publish consumer illness-detection figures such as sensitivity, specificity, or positive predictive value for Vitals.

The false-alarm problem

Wearable studies show why an early warning is not the same as a diagnosis. The 2024 DETECT-AHEAD trial found that wearable and symptom alerts could prompt respiratory-virus testing, but symptoms generated more prompts than physiological changes.

The large 2025 COVID-RED trial also illustrated the sensitivity-versus-specificity trade-off: an experimental wearable-and-symptom algorithm could provide earlier alerts, but in some detection windows it had very low specificity and substantially overestimated infections.

In plain English, a system designed to catch more possible illnesses may also produce many false alarms. Research on wearable infection detection remains promising, but reliable real-world viral diagnosis has not been established; see this review of wearable viral-infection monitoring.

How to interpret a Vitals notification

Use an alert as a reason to check your health—not as a diagnosis.

  1. Check for symptoms. Look for sore throat, cough, congestion, chills, measured fever, fatigue, unusual weakness, vomiting, or shortness of breath.
  2. Confirm temperature separately. Wrist temperature is a trend, not the same as an oral, ear, or core temperature reading. Use a proper thermometer if fever is suspected.
  3. Consider testing. If symptoms or exposure make it appropriate, follow current instructions for an authorized COVID-19 or flu test. A single early negative result may not rule out infection.
  4. Review recent confounders. Consider alcohol, unusually hard exercise, poor sleep, stress, travel, altitude, medication, dehydration, or vaccination.
  5. Look for a pattern. Several metrics changing across multiple nights are more useful context than one isolated outlier.
  6. Rest when appropriate. Avoid intense training if you feel unwell or several measurements remain abnormal.
  7. Contact a clinician when needed. Persistent, worsening, or concerning symptoms deserve medical advice, especially if you have a high-risk condition.

Seek urgent care immediately for severe breathing difficulty, chest pain, confusion, fainting, blue or gray lips, or rapidly worsening symptoms. Do not wait for Vitals to confirm a problem.

What each Vitals metric can—and cannot—tell you

Heart rate

A sustained increase in overnight or resting heart rate can accompany illness, but it is nonspecific. Dehydration, alcohol, stress, poor sleep, fever, medication, and strenuous exercise can produce the same pattern.

Respiratory rate

Respiratory rate may rise with infection or congestion, but it can also change with asthma, anxiety, altitude, sleep disruption, or sensor limitations. It is not a substitute for a clinical respiratory assessment. Research also shows that wearable respiratory measurements can vary in agreement with clinical observations; see this 2025 performance study.

Wrist temperature

Wrist temperature is intended for trend tracking. It can be affected by room temperature, watch fit, skin contact, sleep position, and inconsistent wear. Apple says wrist-temperature tracking is available on Apple Watch Series 8 or later and all Apple Watch Ultra models. It is not a fever measurement.

Blood oxygen

Blood-oxygen availability depends on the model, region, and purchase history. Apple says that on certain Apple Watch models purchased in the United States on or after January 18, 2024, blood-oxygen analysis is unavailable in Vitals, with relevant data handled differently and viewable in the Health app.

Even when available, a consumer wearable oxygen reading should not be used alone to rule in or rule out respiratory illness. Concerning symptoms require medical assessment and, when appropriate, a validated medical pulse oximeter or clinical measurement.

Sleep duration

Short or disrupted sleep can occur during illness, but poor sleep can also cause next-day changes in heart rate and how you feel. Sleep duration is useful context, not an illness marker by itself.

How to get more useful Vitals data

Apple’s documented setup requirements include:

  • An Apple Watch running the latest available compatible watchOS
  • An iPhone running the latest compatible iOS
  • Sleep tracking configured
  • Sleep Focus enabled while wearing the watch overnight

Wear the watch consistently and keep it sufficiently charged. A new watch, a watch worn only occasionally, or a loose-fitting watch may not have enough reliable data to establish a meaningful baseline.

On Apple Watch, open Vitals and review the overnight metric cards. On iPhone, open Health, tap Search, choose Vitals, and select a daily, weekly, monthly, or six-month view. Tap Show More Vitals Data to inspect individual measurements and outlier history.

Why Vitals can be wrong

The app is most useful when its limitations are kept in view:

  • Insufficient baseline: New users may have limited personal history.
  • Inconsistent wear: Removing the watch overnight, charging it during sleep, or wearing it loosely creates gaps.
  • Measurement conditions: Tattoos, cold skin, sweat, movement, and poor contact can affect optical readings.
  • Lifestyle changes: Alcohol, hard workouts, stress, poor sleep, travel, and altitude can move several metrics together.
  • Medication or vaccination: Both may temporarily affect temperature, heart rate, sleep, or other readings without indicating infection.
  • Chronic conditions: Asthma, arrhythmia, thyroid disease, sleep apnea, pregnancy, and other conditions can alter a personal baseline.
  • Normal readings during illness: You can be sick without producing changes in the specific measurements Vitals uses.
  • Abnormal readings without illness: A notification can occur when no infection is present.

A personal baseline is an advantage because it accounts for individual differences. It can also mislead if it was established during an unusual period or if your watch use is irregular.

Vitals is not the same as a medical Apple Watch feature

Apple Watch includes features with separate regulatory histories for specific purposes, such as irregular-rhythm notifications or sleep-apnea notifications. That does not make Vitals a general-purpose illness detector. Apple presents Vitals as health and wellness information, not a diagnostic system, and advises consulting a healthcare provider before making health decisions.

Do not buy an Apple Watch primarily as a fever detector or infection test. It may be a useful addition to an existing Apple-health setup for sleep, fitness, and personal trends, but a thermometer and an authorized diagnostic test answer different questions. No consumer wearable should replace medical care.

Bottom line

Apple Watch Vitals may flag that your body is behaving differently before or around the time you feel sick. It cannot tell you whether the cause is a virus, poor sleep, alcohol, stress, exercise, medication, altitude, or another factor.

Use it as an early-warning dashboard for your own trends. Confirm suspected fever with a thermometer, consider appropriate testing, avoid overtraining when unwell, and seek medical advice for persistent or serious symptoms. A normal Vitals result does not rule out infection, and an alert does not prove that you have one.
