Apple introduced Hypertension Notifications in 2025, but Apple Watch still does not take blood-pressure readings. The feature analyzes optical heart-sensor data for patterns associated with possible chronic hypertension; it does not show systolic or diastolic numbers. If it sends an alert, you still need an external blood-pressure cuff to collect readings and confirm what is happening.

What Apple Watch’s blood-pressure feature actually does

“Blood-pressure monitoring” is an imprecise shorthand for Apple’s feature. Hypertension Notifications is a screening feature, not a cuff-free blood-pressure monitor. Apple’s validation paper says it does not directly measure blood pressure or provide a blood-pressure value.

Apple announced the feature alongside Apple Watch Series 11 on September 9, 2025. It uses data from the watch’s optical heart sensor, also known as a photoplethysmography (PPG) sensor, to analyze how blood vessels respond to heartbeats. The analysis happens passively in the background. If the watch detects consistent patterns associated with possible chronic hypertension during a 30-day period, it can send a notification. It does not inflate a cuff, directly sense arterial pressure or show a continuous blood-pressure graph. (Apple’s launch announcement)

Which Apple Watches support Hypertension Notifications?

Apple lists these compatible models and requirements in its support guidance:

  • Wrist Detection enabled.
  • The user must meet Apple’s eligibility criteria, including being at least 22 years old.

Apple Watch SE does not support the feature. Series 11 or Ultra 3 is not required: earlier Series 9 and Ultra 2 models are included when software and regional requirements are met. Check the Health app for the feature on your own device.

Who is the feature intended for?

Apple says Hypertension Notifications is intended to help identify possible previously unrecognized hypertension, not to manage an existing diagnosis. Apple says it is not intended for people who are younger than 22, have already been diagnosed with hypertension, are pregnant or have an active pregnancy recorded in Health, or want to track treatment or confirm blood-pressure control. What to do if your Apple Watch sends an alert

An alert is a prompt to check your blood pressure, not a diagnosis. Apple instructs users to take cuff measurements twice a day for seven days, enter them in the Health app and discuss the record with a healthcare professional. You need an external cuff; an upper-arm monitor with an appropriately sized cuff is a practical choice for obtaining actual readings.

  1. Use a blood-pressure cuff and follow its instructions and any guidance from your healthcare professional. For a more reliable reading, rest beforehand, support your arm at heart level and avoid talking during the measurement.
  2. Take morning and evening readings for seven days, as directed by Apple’s Health app workflow. Follow the app’s instructions for recording the measurements.
  3. To enter a reading manually, open Health, tap Search, then Heart, then Blood Pressure. Under Blood Pressure Log, tap Add a Measurement, enter the systolic and diastolic values, and tap Done. See the Apple Watch User Guide.
  4. Share the seven-day log with a healthcare professional and ask what follow-up is appropriate.

For setup, Apple documents this route: open Health, tap your profile icon, open Health Checklist, select Hypertension Notifications, confirm your age and hypertension history, and follow the on-screen instructions. Health app screens can change; if the labels differ, search Health for “Hypertension Notifications.”

What the feature cannot tell you

  • No alert does not prove your blood pressure is normal. Apple says the feature will not detect every instance of hypertension.
  • An alert does not confirm hypertension. It identifies a possible pattern that needs cuff readings and appropriate medical follow-up.
  • It is not for treatment tracking. The feature is not designed to show whether medication or lifestyle changes are controlling known hypertension.
  • It is not an on-demand test. The initial evaluation uses a 30-day window, so it cannot provide an immediate reading when you want one.
  • It is not an emergency monitor. Do not wait for a watch alert if you have alarming symptoms such as chest pain, severe shortness of breath, weakness or confusion. Seek emergency care for a possible emergency; Apple says Apple Watch cannot detect a heart attack.

How strong is the evidence, and what does FDA status mean?

Apple says the algorithm was trained using data from multiple studies involving more than 100,000 participants and validated in a clinical study involving more than 2,000 participants. Those figures and the performance description are Apple’s account in its 2025 launch announcement; they do not make a watch notification equivalent to a cuff measurement.

The FDA document describes Apple’s Hypertension Notification Feature as an over-the-counter mobile medical application that analyzes Apple Watch PPG data over multiple days and notifies users who may have hypertension. It is not a clearance of Apple Watch as a blood-pressure monitor, and the feature is not intended to replace traditional diagnosis, treatment-effect monitoring or blood-pressure surveillance.

Does it work in your country?

Availability varies by country, region, language, regulatory status and software version. Apple lists the United States for Hypertension Notifications, but readers elsewhere should check the current watchOS feature-availability page rather than assume the feature is enabled where they live.

Should you buy an Apple Watch for blood pressure?

  • Consider a compatible Apple Watch if you want its broader smartwatch and health features and are eligible to use Hypertension Notifications. If you already own a Series 9 or Ultra 2 that meets the software and regional requirements, a new watch may not be necessary.
  • Do not buy one as a replacement for a cuff. If your main goal is to get blood-pressure numbers, choose a clinically validated upper-arm monitor with a suitable cuff size and follow medical guidance.
  • If you already have diagnosed hypertension, use a cuff and the monitoring plan recommended by your healthcare professional. Apple’s notification feature is not intended to track treatment or confirm control.

The feature can provide a passive prompt to seek proper measurements, but it cannot take those measurements. Apple’s launch materials describe training data from multiple studies involving more than 100,000 participants and validation in a study involving more than 2,000 participants; Apple also cautions that the feature will not detect every case. No alert is not a clean bill of health, and an alert is not a diagnosis.
