Short answer: an Apple Watch cannot measure blood sugar by itself. It can display glucose readings from a separate continuous glucose monitor (CGM). With Dexcom G7 or G7 15 Day Direct to Watch, readings can appear on a compatible Apple Watch even when your iPhone is not nearby—but the iPhone is still needed for initial setup and some connected features.
- Apple Watch alone: No.
- Dexcom G7 Direct to Watch: Yes, for local viewing after setup.
- Dexcom Stelo: The iPhone must remain connected and nearby for Apple Watch readings.
- FreeStyle Libre: Apple Watch support and connectivity vary, but Abbott’s cited guidance requires the watch to be within Bluetooth range of the iPhone.
The Apple Watch does not measure glucose itself
The accurate description is: an Apple Watch can display readings from a compatible CGM, but the watch itself does not measure glucose.
Apple Watch health sensors can support functions such as heart-rate tracking, ECG on supported models, activity measurement and sleep tracking. None of those sensors is a blood-glucose sensor. Glucose is measured by a separate CGM worn on the body, usually using a tiny filament positioned beneath the skin.

How glucose gets onto the watch
A typical Apple Watch-and-CGM setup works as a chain:
- The CGM sensor measures glucose in interstitial fluid beneath the skin.
- The sensor sends readings over Bluetooth.
- In Dexcom Direct to Watch mode, the sensor communicates directly with the Apple Watch.
- The watch displays the current glucose value, trend information and supported alerts.
That makes the watch a receiver, display and alert device—not the measuring instrument. Interstitial-fluid glucose can lag behind blood glucose, especially when glucose is changing quickly, so follow the CGM’s labeling and your clinician’s instructions.
Dexcom G7 is the current answer for leaving the iPhone behind
According to Dexcom’s current documentation, Dexcom G7 and Dexcom G7 15 Day can connect directly to a compatible Apple Watch through Bluetooth. Once Direct to Watch is configured, the wearer can view readings on the watch without carrying the iPhone.
This is the important distinction: “no iPhone nearby” does not mean “no iPhone required.” A compatible iPhone is still required to pair a new sensor and complete the initial configuration. Dexcom also says the phone may be needed for settings, sensor replacement, troubleshooting, data synchronization, remote sharing and other connected functions. See Dexcom’s iPhone pairing FAQ.
Is a cellular Apple Watch necessary?
No—not for the local Dexcom sensor-to-watch connection. Dexcom says Direct to Watch uses Bluetooth between the sensor and watch, so a cellular plan is not required for local readings. Cellular or Wi-Fi may still matter for unrelated online watch features, cloud services or remote-monitoring workflows. Dexcom’s provider guidance explains the cellular requirement.

Direct to Watch versus Standard Smartwatch mode
Installing the Dexcom watch app does not automatically mean the watch can operate away from the phone. Dexcom supports two different connection paths:
- Direct to Watch: the G7 sensor sends readings directly to the Apple Watch. The iPhone does not need to be nearby for normal local viewing.
- Standard Smartwatch mode: the watch receives data through the iPhone. The phone generally needs to remain within Bluetooth range—Dexcom describes that range as approximately 20 feet in its documentation.

Compatibility checklist for Dexcom Direct to Watch
Dexcom’s cited minimum requirements include:
- Apple Watch Series 6 or later.
- watchOS 10 or later.
- An iPhone running iOS 17 or later.
- The required Dexcom G7 app version; Dexcom’s launch documentation cited version 2.1 or later.
- A supported G7 product variant and country or region.
These are not a guarantee that every Series 6-or-newer configuration works everywhere. Support can depend on the exact watch model, operating systems, app version, country, product variant and regulatory availability. Check the current Dexcom compatibility checker before buying a watch or sensor.

What happens without Wi-Fi, cellular service or the iPhone?
For Direct to Watch’s local sensor-to-watch connection, Bluetooth is the key requirement. The watch may continue showing local readings without Wi-Fi, an active cellular plan or the iPhone nearby.
That does not make every feature work offline. Internet connectivity can be relevant to:
- Uploading readings to cloud services.
- Remote monitoring and Share/Follow.
- Cloud dashboards and account features.
- Some notifications or connected settings.
Direct viewing by the wearer and remote viewing by a caregiver are separate functions. Do not assume that leaving the iPhone behind preserves caregiver alerts. Anyone relying on remote monitoring should test the complete setup—including the follower’s alerts—before depending on it during travel, exercise or diabetes management.

Dexcom G7, Stelo and FreeStyle Libre compared

Dexcom Stelo
Stelo is not the same product as Dexcom G7 Direct to Watch. Stelo’s current support information says the Apple Watch communicates with the smartphone rather than directly with the sensor, so the watch must remain connected to and within range of the phone.
Stelo is an over-the-counter system for adults age 18 and older who are not using insulin. Its labeling says it is not intended for people with problematic hypoglycemia. It is therefore a possible option for lifestyle, nutrition, exercise or glucose-pattern tracking for eligible users, but it is not the answer to a strict “leave the iPhone behind” requirement and should not be presented as a universal diabetes-management solution. Abbott FreeStyle Libre
Libre capabilities differ by sensor generation, app, operating system and region. Abbott’s cited support page says the Libre Watch app can display data without internet only when the Apple Watch is within Bluetooth Low Energy range of the iPhone. That is different from Dexcom’s direct sensor-to-watch mode.
If leaving the iPhone behind is non-negotiable, do not choose a Libre setup based only on the existence of an Apple Watch app. Verify the exact Libre product and local support documentation first. The exact menus can change with app and operating-system updates, but the practical path is:

- Check compatibility. Confirm the watch, watchOS, iPhone, iOS, Dexcom app, country and G7 variant using Dexcom’s current compatibility checker.
- Start the sensor with the iPhone. The iPhone is required to pair a new G7 sensor.
- Complete watch setup. Install or enable the Dexcom watch app and configure Direct to Watch while the phone is available.
- Confirm the connection mode. Make sure the watch is in Direct to Watch mode rather than Standard Smartwatch mode.
- Verify live data. Check that the watch shows a current glucose value, trend information and the alerts you intend to use.
- Test before relying on it. Leave the iPhone behind briefly in a safe situation and confirm that readings and alerts behave as expected.
- Keep the iPhone available for exceptions. You may need it for settings, sensor replacement, troubleshooting, sharing or cloud-dependent functions.
Troubleshooting when readings disappear
If the watch shows a stale value, no current reading or no alert, do not assume the last displayed number is current. Check:
- Whether Direct to Watch is actually enabled.
- Whether the system has fallen back to Standard Smartwatch mode.
- Whether Bluetooth is enabled and the sensor session is still active.
- Whether the watch and sensor have sufficient battery or remaining session time.
- Whether the watchOS, iOS and Dexcom app versions remain supported.
- Whether the watch app shows an error or stale-data state rather than a current value.
- Whether an iPhone is needed to reconfigure the system or start a replacement sensor.
A dead watch battery means no watch display or alerts even if the sensor itself continues measuring. A watch restart, disabled Bluetooth, software incompatibility, expired sensor or physical connection problem can also interrupt readings.
Can the Apple Watch replace a glucose meter?
The Apple Watch alone cannot replace a glucose meter or CGM. A compatible CGM shown on the watch may provide continuous readings and alerts, but the watch display does not change the medical indication, accuracy limits or safety instructions of the underlying CGM.
If a reading does not match your symptoms or expectations, follow the CGM manufacturer’s instructions for confirmation, which may include using a blood-glucose meter, and contact a healthcare professional when appropriate. Dexcom specifically instructs users to use a blood-glucose meter for treatment decisions when G7 readings do not match symptoms or expectations. Do not make medication changes solely because a watch complication displays a number.
Similarly, Stelo’s output is not a substitute for clinician-directed treatment decisions. Convenience on the wrist should never be confused with independent medical measurement.
What you are actually buying
For the iPhone-free viewing use case, the purchase is a combined workflow:
- A compatible CGM sensor system, such as Dexcom G7 or G7 15 Day where available and appropriate.
- A compatible Apple Watch to receive and display the data.
- A compatible iPhone for pairing, initial setup and some ongoing functions.
- Replacement sensors and any applicable prescription, insurance or regional requirements.
- Possibly Wi-Fi or cellular service if you need online watch features or remote sharing—not for the local Dexcom Bluetooth connection itself.
Before buying, verify the exact product availability, medical eligibility, app support and ongoing sensor cost in your country. The Apple Watch is not a standalone glucose meter, and a cellular Apple Watch is not automatically required.
The bottom line
If your requirement is “show my glucose on my wrist while my iPhone stays at home,” Dexcom G7 or G7 15 Day with Direct to Watch is the clearest current fit among the systems covered here. If your requirement is “measure blood sugar with the Apple Watch itself,” the answer is no. The sensor does the measuring; the watch displays the result. An iPhone remains part of the system for setup and may still be essential for sharing, configuration and recovery.
