HabitAware’s Keen2 may help some people interrupt automatic body-focused repetitive behaviors (BFRBs), especially hair pulling, face or scalp picking, and nail biting. It detects a movement pattern the wearer has trained it to recognize and responds with a vibration. That cue can build awareness—but it does not stop the hand, remove the urge, or provide treatment by itself. Think of Keen2 as a possible aid to a behavior plan, not a cure or a replacement for professional care.

What Keen2 is—and what “compulsive habits” means here

Keen2 is a wrist-worn wearable paired with a mobile app. It is designed to recognize trained hand or wrist movements associated with certain body-focused repetitive behaviors, then vibrate to alert the wearer. The intended uses include trichotillomania (recurrent hair pulling), skin picking, and nail biting. HabitAware describes the product and its intended uses here.

BFRBs are repetitive grooming behaviors that can cause physical damage, distress, or interference with daily life. Occasional nail biting or picking does not, by itself, mean someone has a disorder. Skin-picking disorder, for example, involves recurrent picking that causes lesions, unsuccessful efforts to stop, and significant distress or impairment; other possible causes also need consideration. See the TLC Foundation’s overview of skin-picking disorder.

“Compulsive habits” is broader than Keen2’s intended scope. The available evidence does not establish it as a treatment for OCD generally, gambling, substance use, eating disorders, tics, or every kind of fidgeting or repetitive movement.

How the bracelet is supposed to help

  1. Choose a target behavior and train the device. The wearer performs representative movements so the system can learn the gesture to look for. HabitAware’s support documentation says the device can be trained for up to four areas; app controls and limits can change, so check the current app and company setup guidance.
  2. Wear it in situations where the behavior is likely. If it recognizes a trained movement, Keen2 vibrates.
  3. Pause and choose a response. The vibration is a reminder, not a restraint or punishment. The wearer still needs to move their hand, use a competing response, or try another prepared strategy.
  4. Review what is happening and adjust. Tracking and retraining may help the wearer notice triggers or address missed and excessive alerts.

This is why Keen2 is best understood as an external awareness cue. Habit Reversal Training (HRT) uses awareness as one part of a broader approach that also includes competing-response practice and social support. A bracelet can potentially assist with noticing; it does not supply the whole treatment. TLC’s treatment overview explains these elements.

Which behaviors are the best fit?

HabitAware says Keen2 works best when a behavior involves the hand moving above the shoulders. That makes scalp or facial hair pulling, face or scalp picking, and some nail-biting movements plausible candidates. It is not a guarantee that the device will recognize every person’s version of those movements.

  • More plausible fit: repetitive hand-to-head or hand-to-face movements, such as pulling scalp hair or picking facial skin.
  • Less certain or potentially poor fit: picking fingers or feet, behaviors below the shoulders, movements with little wrist motion, or movements that vary substantially from episode to episode.
  • Ask HabitAware before relying on it: cheek or lip biting, hair eating, thumb sucking, complex multi-step behaviors, and other behaviors not clearly covered by the company’s guidance.

The company specifically cautions about finger picking and behaviors below the shoulders on its page explaining what Keen2 can and cannot do. Public information does not establish validated detection sensitivity or false-alert rates for the current Keen2 across different behaviors and users. A vibration may occur during a harmless similar movement, or the device may miss an episode.

Does it reduce the behavior—or just make it noticeable?

Those are different outcomes. Keen2 may detect a trained movement and alert the wearer; an alert may increase awareness; and awareness may help someone interrupt or reduce the behavior if they act on it. The last step depends on the person, the behavior, and having a response ready. An alert that is missed, ignored, or followed by a different BFRB may not lead to meaningful change.

HabitAware’s product pages include company-reported outcome figures, including claims about increased awareness and reduced unwanted behaviors. Those are not the same as independently controlled estimates of how well Keen2 works for the general population. Treat them as company claims, not a promise of an individual result. The Keen2 System page presents the company’s figures and bundle information.

What the research can—and cannot—tell you

A peer-reviewed pilot study evaluated an earlier prototype of HabitAware’s wrist-device technology and app in 15 adults with trichotillomania over four weeks. It examined usability, acceptability, feasibility, and preliminary efficacy. The small sample and short duration make this encouraging early evidence that the approach merits study, not proof that retail Keen2 reliably reduces hair pulling for everyone, works for other BFRBs, or has lasting effects. The study record is available through PubMed.

A registered open-label feasibility study is also evaluating Keen2 for adults with trichotillomania. A study listing is not a result: it does not establish effectiveness. See ClinicalTrials.gov for the study record and current status.

The evidence does not show that Keen2 is superior to therapist-delivered HRT, Comprehensive Behavioral Treatment (ComB), or other behavioral care. Nor does the pilot establish long-term adherence, relapse prevention, or current-device performance across body sites and movement styles. HabitAware says its development has received NIH and NSF support; that is not the same as NIH endorsement or approval of the retail bracelet. The company’s research page describes its research claims.

What to do when it vibrates

A cue is more useful when it prompts a specific, non-punitive action. Consider making a plan in advance:

  1. Pause without judging yourself. Notice whether you were pulling or picking, or whether it was a similar movement.
  2. Identify the context. Was it happening during boredom, stress, fatigue, concentration, mirror use, an itch, or contact with a rough patch?
  3. Use a competing response. Temporarily position or occupy your hands so pulling or picking is harder—for example, hold an object with both hands or use a response developed with a clinician.
  4. Try a suitable alternative or barrier. Depending on the behavior, a sensory object, bandage, glove, hat, or change to mirror access may help. Use barriers safely and without causing pain or skin damage.
  5. Note the result and carry on. A lapse is information, not proof of failure. Review patterns and adjust your plan.

These are examples of strategies used in BFRB care, not a substitute for individualized advice. The goal is not punishment or pain. TLC’s treatment guidance describes competing responses and stimulus-control approaches.

Keen2 alongside therapy

For BFRBs that cause injury, distress, or repeated unsuccessful attempts to stop, a clinician familiar with BFRBs can assess what is happening and help build a treatment plan. CBT-based approaches—particularly HRT and ComB—are established options described in expert BFRB guidance. Keen2 may be worth discussing as one aid for awareness, but it cannot provide trigger assessment, individualized competing-response training, or care for co-occurring problems by itself. TLC offers a directory of medical and therapeutic providers.

Depression, anxiety, OCD symptoms, trauma, ADHD, skin irritation, and medication effects can complicate repetitive behaviors and may need separate evaluation. For children and teens, involve a parent or guardian and a qualified clinician; consider comfort, privacy, fit, and whether alerts could draw unwanted attention at school.

Limitations to weigh before buying

  • Missed and unnecessary alerts: detection may be late, miss a behavior, or respond to a similar harmless movement. No independent current false-positive or false-negative rate is established in the available evidence.
  • Training and variation: the system needs representative training, while posture, target area, and movement may change.
  • Coverage boundaries: below-the-shoulder and low-wrist-motion behaviors may not suit the device.
  • Sensory and attention burden: some people may find vibration unpleasant, distracting, embarrassing, or easy to tune out.
  • No automatic replacement for the urge: interrupting one behavior does not address its trigger or prevent another behavior from taking its place.
  • Wearability and dependence: benefit depends on wearing and charging the bracelet; it cannot cue you when it is unavailable.
  • Cost and access: device price, optional services, availability, shipping, and return terms can change.

Do not rely on a wearable for urgent medical issues. Persistent open wounds, spreading redness, pus, fever, or severe pain warrant medical care. People who swallow pulled hair should discuss it with a clinician because hair ingestion can rarely lead to a gastrointestinal blockage; see TLC’s hair-pulling information.

How to judge whether a trial is helping

If you try Keen2, agree on what improvement would look like before focusing only on a zero-episode goal. Track, where practical, episode frequency or duration, whether alerts were acted on, time between urge and interruption, physical damage, distress, and ability to use a competing response. Also notice whether any benefit persists when the bracelet is off. If alerts are consistently missed or excessive, the behavior is outside its useful range, or wearing it makes you feel worse, contact the seller about your options and discuss alternatives with a clinician.

Who might consider it—and who should look elsewhere

Keen2 may be worth considering if your main behavior is a recognizable hand-to-head, hand-to-face, or nail-biting movement; you often notice only after it starts; vibration is acceptable; and you are willing to train the device and practice a response. It is most defensible as an adjunct to therapy or a structured self-management plan.

It may be a poor fit if the behavior happens mainly below the shoulders, uses little wrist movement, varies widely, or is not a BFRB; if vibration is distressing; or if you expect the bracelet to physically prevent the behavior. Seek professional care first if there are significant injuries, infection concerns, substantial distress or impairment, or a need for diagnosis or treatment of another condition.

Alternatives and complements

Clinician-guided HRT or ComB is a stronger starting point when the behavior is significantly affecting health or daily life. Lower-cost supports—such as manual phone tracking, a reminder timer, a suitable sensory object, a written competing-response plan, or noninjurious barriers—may help some people notice patterns or manage triggers, though they do not offer Keen2’s trained gesture-detection feature. Peer support may also help. HabitAware has described a NOCD partnership on its website; check current location, eligibility, insurance participation, and offer terms rather than assuming the service or any device offer is available to you.
