Samsung’s redesigned BioActive Sensor has a more unusual optical array than a typical smartwatch sensor: it uses green, red, infrared, blue, yellow, violet and ultraviolet LEDs, alongside a rearranged set of photodiodes. That is a genuine hardware change—but it does not mean the watch can see through your body or diagnose your health.

One naming note: Samsung announced the Galaxy Watch7 and Galaxy Watch Ultra as separate watches on July 10, 2024. “Galaxy Watch 7 Ultra” is common shorthand, not the official product name. This article focuses on the original 2024 Galaxy Watch Ultra and its BioActive Sensor.

The short answer

The BioActive Sensor is Samsung’s name for a package of optical, electrical and bioelectrical-impedance sensors—not one all-purpose medical instrument. Its redesigned optical hardware gives Samsung more kinds of light signals to work with, while software combines sensor data to produce readings and estimates such as heart rate, body composition and the AGEs Index.

The most distinctive addition is the Advanced Glycation End Products (AGEs) Index, a wellness indicator Samsung associates with diet, lifestyle, metabolic health and biological aging. It is not a direct reading of your biological age or a diagnosis. Other headline features, including blood-pressure estimates and sleep-apnea risk assessment, also have important limits and depend on software, phone compatibility and availability in your region.

First, what changed in the BioActive Sensor?

Samsung says it redesigned the optical part of the sensor for the Galaxy Watch7 generation. It more than doubled photodiode performance, reduced the photodiode count from eight to four without reducing sensing capacity, and expanded the LED mix. The array uses green, red and infrared light, with additional blue, yellow, violet and ultraviolet LEDs. Samsung says the components were rearranged to improve optical performance and make room for additional measurements.

Different light wavelengths provide different optical signals for the watch’s algorithms to interpret. But Samsung’s public explanation does not establish that each color corresponds to a particular diagnosis or that the watch directly detects a particular substance in the body. The careful description is that the array gathers signals which, together with software, can support health and wellness features.

Samsung also says the redesign improves intensive-workout heart-rate accuracy by 30% compared with the Galaxy Watch6. That is Samsung’s claim, not an independently established improvement for every wearer or activity. The published claim does not, by itself, tell shoppers how results vary by exercise type, fit, user or testing conditions.

It is a sensor package, not a single sensor

Samsung describes three main parts of the BioActive Sensor:

  • Optical biosignal sensing: LEDs illuminate the skin and photodiodes capture returning light. This is used for signals such as heart rate and blood oxygen, from which software may derive further estimates.
  • Electrical heart-signal sensing: electrical contacts collect signals used by ECG-related functions. ECG availability and use depend on Samsung Health Monitor, a compatible phone and regional authorization.
  • Bioelectrical impedance analysis (BIA): the watch uses an electrical signal to generate body-composition estimates. Those estimates are not the same as a clinical body-composition test.

The watch also has separate motion and environmental sensors, including an accelerometer, gyroscope, barometer, geomagnetic sensor, light sensor and temperature sensor. Software can use movement and other signals to interpret activity or sleep. The sensor package gathers inputs; it does not make every resulting metric a direct measurement.

Fit matters. The watch needs consistent contact with the skin, and position, movement, moisture and individual physiology can affect readings. Tattoos, hair, workout type and how tightly the watch sits may also matter. A more capable optical array cannot eliminate motion artifact or compensate for poor contact in every situation.

Not every feature is available everywhere or on every phone. Samsung Health Monitor and local regulatory rules matter as much as the watch hardware. Check the current Samsung feature page and requirements for your country and exact watch model before buying for a particular health function.

What the AGEs Index does—and does not—tell you

Advanced glycation end products, or AGEs, are compounds formed when sugars react with proteins or fats. Samsung presents its AGEs Index as a lifestyle- and diet-influenced indicator related to metabolic health and biological aging. That framing can make the feature sound more definitive than it is.

The index is not a clinical diagnosis of aging, diabetes, cardiovascular disease or overall health. Samsung describes it as a fitness and wellness feature, not a tool to diagnose or treat a medical condition. Treat it, at most, as a potential long-term wellness trend to discuss in context—not as a verdict about your health. Do not change medication, diet or treatment solely because of a watch score.

Samsung announced the feature in 2024 as an upcoming Galaxy Watch capability. Availability can depend on model, region and software release, so do not assume that every original Ultra owner can use it simply because the hardware is present.

Medical features come with conditions

Blood pressure requires a cuff

The Ultra does not inflate around your wrist to take blood pressure. Samsung’s feature estimates blood pressure after calibration with a traditional upper-arm cuff; Samsung says to recalibrate every four weeks. The function is available only in select markets and is not intended to diagnose hypertension, heart attack or another medical condition. Use a properly validated cuff and follow Samsung’s instructions; do not treat the watch as a cuff replacement or use its readings instead of professional care.

Sleep-apnea assessment is limited screening

In the United States, Samsung’s sleep-apnea feature was cleared by the FDA for a limited use: it looks for signs of moderate-to-severe obstructive sleep apnea in adults aged 22 or older who have not previously been diagnosed. Samsung’s instructions say it cannot detect central sleep apnea and is not a replacement for a clinical sleep study. It can also miss cases, so a negative result does not rule out sleep apnea.

The U.S. setup requires a compatible Galaxy Watch and Galaxy phone, Samsung Health Monitor installed and updated on both, and regional support. The user must enable Sleep apnea in Samsung Health Monitor and wear the watch snugly for two nights within a 10-day period, with about four hours of sleep each night. The feature uses PPG-based blood-oxygen sensing and actigraphy to look for relative oxygen decreases and movement-related breathing-disruption patterns. Samsung says analysis may take up to 90 minutes after a night’s data is collected.

Samsung’s validation study described in the instructions included 470 subjects and reported 82.7% sensitivity and 87.7% specificity. Those are results from that study, not a guarantee for an individual user or every sleep condition. A concerning result warrants medical evaluation; it is not itself a diagnosis and should not be used to change treatment or medication.

ECG and other features are not universal

ECG, irregular-rhythm notifications, blood pressure and sleep-apnea assessment rely on Samsung Health Monitor and can have phone, market and regulatory restrictions. The presence of the sensor hardware does not guarantee that a particular feature is enabled where you live or works with a non-Samsung phone. Check Samsung’s current local requirements rather than assuming launch-era compatibility applies to later watches or software.

Does the sensor make the original Ultra worth buying?

The BioActive redesign is more than a marketing rename: Samsung describes concrete changes to photodiode performance, count and LED colors. But the everyday value depends on which features you will actually use. If you want Samsung’s mix of exercise tracking, sleep metrics, ECG, BIA and wellness tools, the platform may be useful—provided you understand which outputs are estimates and which features are region- or phone-dependent.

The sensor alone is a weaker reason to choose the Ultra over a regular Galaxy Watch. Samsung introduced the Watch7 alongside the Ultra, and both share the BioActive Sensor architecture and several health capabilities. The Ultra’s case is the separate argument: it is a large, rugged 47 mm watch with LTE, dual-frequency GPS, a 590 mAh battery and 10 ATM water resistance. At launch it weighed 60.5 g, used a 1.5-inch 480 × 480 Super AMOLED display, Exynos W1000 five-core 3 nm processor, 2 GB RAM and 32 GB storage, and ran Wear OS 5 with One UI 6 Watch. Those are launch-generation specifications, not a description of every later Ultra model.

That size and weight can be a drawback for smaller wrists and for wearing overnight, when sleep tracking needs consistent wear. Battery life is measured in days rather than weeks. The original Ultra launched with Android 11-or-newer compatibility and a phone with more than 1.5 GB of memory; later models may have different requirements. It is not a suitable choice for iPhone users, and some Samsung Health Monitor features are a poorer fit if you do not use a compatible Galaxy phone.

Which Galaxy Watch makes sense?

  • Consider the 2024 Galaxy Watch Ultra if you want a rugged, large-format Samsung watch for outdoor use, value its LTE and dual-frequency GPS positioning, and will use Samsung’s broader health and fitness platform. Buy it for the overall watch, not for a medical promise or the AGEs Index alone.
  • Consider Galaxy Watch7 if you want many of the same named sensor capabilities in a smaller, lighter, less rugged watch and do not need the Ultra’s 47 mm case, larger battery or 10 ATM rating.
  • Compare the newer Galaxy Watch Ultra2 if buying current-generation hardware matters more than getting the original model at a lower price. Samsung describes the newer model as having improved fit and an 800 mAh battery. The BioActive Sensor name and broad sensing categories remain similar, so upgrading solely for the sensor branding may not make sense for an original Ultra owner.
  • Choose a validated blood-pressure cuff if your priority is dependable blood-pressure measurement. The watch’s feature still requires cuff calibration and is not a cuff substitute.

Samsung’s U.S. store showed the original Galaxy Watch Ultra from $649.99 before trade-in when observed on August 18, 2026; prices and promotions can change. Compare the current price of the original, Ultra2 and Watch7 rather than treating that snapshot as a permanent price.

Verdict

Samsung’s BioActive Sensor is technically interesting because it combines a revised multicolor optical array with electrical heart-signal and impedance sensing. It broadens the kinds of wellness features Samsung can offer, but it does not turn a wristwatch into a medical lab. The AGEs Index is a wellness indicator, blood pressure still needs cuff calibration, and sleep-apnea screening has a narrow intended use. Choose the Galaxy Watch Ultra for its rugged hardware and Samsung ecosystem if those fit your needs—not because the sensor sounds like it can diagnose what it cannot.
