Apple Watch does not directly measure your oxygen consumption. It estimates VO₂ max—shown in the Health app as Cardio Fitness—by comparing your heart-rate response with outdoor walking, running, or hiking performance. GPS and motion data describe the workload; your age, sex, height, weight, and relevant medication information help personalize the model. Apple does not publish the complete production formula.
The result is useful for following aerobic-fitness trends, but it is not equivalent to a laboratory gas-exchange test.
What Apple Watch calls VO₂ max
Cardio Fitness is Apple’s consumer-facing name for its VO₂ max estimate. HealthKit stores the underlying quantity as vo2Max (Apple Developer documentation). The value is expressed in milliliters of oxygen per kilogram of body mass per minute (mL/kg/min).

A laboratory VO₂ max test progressively increases exercise intensity while a mask and metabolic cart measure inhaled and exhaled gases. Apple Watch performs no gas-exchange measurement. It uses a predictive model based on submaximal exercise data and extrapolates an estimate of maximal oxygen use.

The data Apple Watch uses
Heart-rate response
The optical heart-rate sensor records how hard your cardiovascular system is working. The important relationship is not simply your pulse or your speed alone, but how much movement you produce for a given heart-rate response.
GPS and motion
GPS supplies outdoor distance, route and pace information, while motion sensors help interpret the activity. Conceptually, covering more ground at the same heart rate suggests greater aerobic capacity than moving more slowly at that heart rate. That is a useful explanation of the model, not Apple’s published equation.
Personal and medication data
Apple says the estimate incorporates age, sex, height, weight and medications that may affect heart rate (Apple Support). Incorrect profile data can create a persistent bias. Weight is especially important because the displayed unit is normalized per kilogram; a change in body mass can change the relative score even when absolute oxygen-processing ability has not changed.
If you take a drug that lowers or changes your achievable heart rate, recording that information in Health can allow the system to interpret the exercise response more appropriately (HealthKit documentation).

When Apple creates a sample
Apple says Series 3 and later can generate Cardio Fitness estimates during qualifying workouts. The current consumer guidance lists:
- Outdoor Walk
- Outdoor Run
- Hiking
These workouts must be recorded with the appropriate Workout app activity. Apple’s support page says indoor workouts do not count for this feature. A third-party app may write workout data to Health, but Apple’s public documentation does not promise that every third-party workout will produce a VO₂ max sample.
The technical eligibility conditions are stricter than merely starting an outdoor workout. Apple’s HealthKit documentation describes the need for:
- Relatively flat ground (approximately less than 5% incline or decline).
- Adequate GPS data.
- Adequate heart-rate signal quality.
- Sufficient exertion, with heart rate approximately at least 130% of resting heart rate.
That 130% figure is a comparison with your resting baseline—not 130% of maximum heart rate. A very easy stroll may therefore be recorded normally but provide too little information for an estimate.

The first value is not normally produced from the first workout. Apple says to wear the watch for at least 24 hours and complete several qualifying outdoor workouts, with passive wearing measurements contributing to setup (support guidance; developer guidance).
Why there is no simple Apple Watch formula
Apple publicly describes the inputs and the general approach—heart-rate response plus exercise performance—but not the complete current weighting, calibration, filtering or extrapolation equation. Therefore, claims that Apple Watch literally calculates “speed divided by heart rate” or uses a single publicly known formula are oversimplifications.
Terrain, stops, temperature, sensor quality and individual physiology can all affect the relationship between pace and pulse. Apple’s model is designed to reject or discount sessions that do not provide a suitable signal rather than treat every workout as equally informative.

- Correct Health details. Check your date of birth, sex, height and current weight in the Health profile.
- Record the workout natively. Use Workout > Outdoor Walk, Outdoor Run or Hiking on the watch.
- Choose a suitable route. Prefer an open, relatively flat path with a steady surface. Tall buildings, dense trees, tunnels, steep hills and repeated sharp turns can degrade GPS or violate the flat-terrain assumption.
- Allow positioning time. Let the watch acquire a reliable GPS position before setting off.
- Keep the effort continuous. A sustained, purposeful pace is more informative than frequent stops or a casual stroll.
- Wear the watch snugly. It should maintain consistent skin contact without being painfully tight. Cold skin, poor circulation, tattoos, sweat, loose fit and wrist flexion can reduce optical heart-rate quality.
- Enter relevant medication information. If medication affects heart rate, use the available Health setting where applicable.
- Repeat comparable sessions. Compare similar routes and conditions over time rather than treating one sample as definitive.
- Keep software current, but be cautious with breaks in the series. A watchOS or algorithm change, revised profile data or newly entered medication information can alter estimates; do not assume every change is physiological.
Apple says frequent outdoor workouts can improve accuracy and that Health may offer an option to update older Cardio Fitness estimates when it determines that newer processing could be more accurate (Apple Support).

Why a VO₂ max reading may be missing
| Likely cause | What to check |
|---|---|
| Ineligible activity | Use native Outdoor Walk, Outdoor Run or Hiking; indoor treadmill, gym-machine and strength sessions are not listed as qualifying inputs. |
| Too little effort | Raise intensity enough above your resting heart rate for a sustained section, without exercising unsafely. |
| Poor GPS | Try an open route and wait for positioning; tunnels, urban canyons and heavy tree cover can corrupt distance or pace. |
| Heart-rate signal problem | Improve fit and skin contact; consider cold conditions, sweat, tattoos, wrist movement or an irregular rhythm. |
| Steep or variable terrain | Use flatter ground. A hike can be an eligible category, but a steep mountain route may not satisfy the model’s conditions. |
| Insufficient wear history | Wear the watch for at least 24 hours and complete multiple qualifying sessions. |
| Incomplete profile | Review age, sex, height, weight and medication information in Health. |
Viewing the result
On the iPhone, open Health → Browse → Heart → Cardio Fitness. Labels can move between iOS releases; if the path differs, search Health for “Cardio Fitness.”

Apple has published development and validation work for its cardio-fitness metric (Apple’s technical paper). Those reported averages describe the tested devices, algorithm and study populations; they are not a guarantee for every wearer.
Independent evidence can show larger individual errors. A recent validation study reported a mean absolute error of about 6.92 mL/kg/min and a mean absolute percentage error of about 13.31% in its sample (peer-reviewed study). Device model, software, population, workout conditions and the laboratory reference test all matter, so those figures should not be treated as a universal specification.
In practice, Cardio Fitness is more defensible as a repeated field-trend measure than as a laboratory-grade value. A stable but biased estimate is possible if, for example, the watch consistently misreads your pulse or your profile weight is wrong.

Why one number can jump or fall
Before interpreting a dramatic change, check whether the route, weather, temperature, stops, GPS quality, watch fit, heart-rate signal, profile data, medication settings or software changed. Look for the same direction across several comparable workouts. A single outlier is often a data-quality event rather than a sudden change in aerobic physiology.
Also remember that VO₂ max is relative to body mass. If weight rises, the mL/kg/min value can fall even when your absolute oxygen consumption is similar; if weight falls, the relative value can rise.
Apple Watch versus a laboratory test

If you need a defensible physiological measurement for clinical evaluation, high-level performance testing or investigation of symptoms, ask a qualified clinician about a supervised cardiopulmonary exercise test. A low Cardio Fitness notification is not itself a diagnosis of heart or lung disease (Apple’s low-cardio-fitness documentation).

A note about Apple’s stated range
Apple’s current consumer Support page describes a supported range of 14–65 mL/kg/min, while its HealthKit developer page says 14–60 mL/kg/min. Apple does not explain the discrepancy publicly, so the applicable range may depend on the documentation or implementation being referenced. Apple says Cardio Fitness classification is available to users aged 20 and older.
