A new randomized trial adds weight to the case that smartwatch monitoring can uncover atrial fibrillation (AFib) that routine care may miss, particularly in older adults at higher stroke risk. But “detect” and “predict” are not interchangeable: the strongest evidence is for flagging possible rhythm problems, while forecasting heart failure or a heart attack remains unproven for most consumer watches. A smartwatch alert is a reason to seek appropriate follow-up, not a diagnosis—and chest pain, fainting or severe shortness of breath needs urgent care regardless of what the watch says.

What the new trial found

The EQUAL randomized controlled trial compared six months of smartwatch-based monitoring with standard care in people at elevated risk of stroke. The watch used passive pulse-irregularity checks alongside an on-demand, single-lead ECG. The monitoring group had more new-onset AFib detected, including episodes that were asymptomatic or intermittent. That matters because a brief ECG taken during an appointment can miss a rhythm problem that comes and goes. The published trial and the American College of Cardiology’s summary describe a selected, higher-risk population; the result should not be assumed to apply equally to every healthy smartwatch owner.

The trial supports better detection, not proof that screening with a watch prevents strokes or deaths. To show that, research must establish that additional alerts lead to confirmed diagnoses, appropriate treatment and improved patient outcomes. More detected cases are not automatically better outcomes.

What a smartwatch can detect—and what remains experimental

Feature or finding What it can mean Evidence and limits
Possible AFib from passive monitoring An optical sensor detects an irregular pulse pattern that may be consistent with AFib. This is the best-supported consumer screening use, but it is a signal for follow-up, not a confirmed diagnosis.
Single-lead ECG A short electrical tracing can be recorded on compatible devices and classified for possible AFib or sinus rhythm. Useful for capturing a rhythm at that moment; it is not the same as a clinical 12-lead ECG and may not identify other conditions.
High- or low-heart-rate alert The measured rate is unusually high or low under the feature’s settings. Heart rate changes with exercise, fever, stress, dehydration, sleep and medication. An alert alone does not explain why.
Hypertension notification A device may flag a pattern associated with possible high blood pressure. This is not a blood-pressure reading. Use a validated cuff and clinical assessment to measure and diagnose high blood pressure.
AI screening for structural heart disease Research systems have explored whether smartwatch ECGs can flag signs associated with weakened pumping, valve disease or thickened heart muscle. Promising but preliminary research—not an established general-purpose consumer diagnosis.
Heart-failure deterioration or future event prediction Research is examining whether changes in wearable signals could precede worsening symptoms or an event. Investigational for most consumer watches; do not treat an ordinary watch as a validated early-warning system.

The Apple Heart Study previously demonstrated the feasibility of large-scale smartwatch AFib screening and follow-up with telemedicine and mailed ECG patches. Newer evidence builds on that idea, but watch features differ by manufacturer, model, software, country and intended use.

Detection is not the same as prediction

“Predicting a heart problem” can describe several quite different things:

  1. Detecting a present or recent rhythm abnormality: an ECG records a rhythm, or an optical sensor notices an irregular pulse pattern.
  2. Finding intermittent disease through longer monitoring: wearing a watch regularly may capture an episode that a one-time office ECG misses.
  3. Estimating risk from trends: changes in resting heart rate, cardio fitness, sleep or other signals may be studied as clues about health. A trend is not a diagnosis.
  4. Forecasting a future clinical event: for example, warning that heart failure will worsen days in advance. This is a more demanding claim and remains investigational for most consumer products.

The clearest consumer evidence is in the first two categories, especially for possible AFib. An AFib notification usually means the watch has detected a pattern that could reflect an episode; it does not mean the watch has predicted a heart attack, stroke or other event.

How accurate are the alerts?

Accuracy figures depend on the device and feature being tested. Sensitivity is how often a test flags people who have the condition. Specificity is how often it correctly avoids flagging people who do not. Neither number, by itself, tells an individual how likely an alert is to be confirmed: that also depends on the person’s risk and the setting in which screening is used.

A 2026 living review and meta-analysis estimated pooled Apple Watch AFib sensitivity at 0.79 and specificity at 0.91 across the included studies. In those studies, the feature was more likely to correctly avoid a false alarm than to catch every case. These pooled estimates vary by study design and population; they are not universal numbers for every watch or user. Read the review and meta-analysis.

Apple reports different results for two distinct features. In a clinical trial of roughly 600 people with classifiable recordings, its ECG app had 98.3% sensitivity for AFib classification and 99.6% specificity for sinus-rhythm classification. Those figures apply to usable, on-demand ECG recordings—not continuous real-world detection of every episode. Apple’s passive irregular-rhythm notification validation reported 88.6% sensitivity under its study protocol. A watch can miss AFib if an episode occurs between checks, and a normal result cannot rule out AFib or other heart disease. Conversely, movement, poor skin contact or other sources of noise can make a reading inconclusive or produce a false alert. Performance data from one feature, manufacturer or study population should not be transferred to another.

What a watch cannot tell you

A smartwatch is not a general-purpose heart diagnostic tool. A single-lead tracing provides less information than a clinical 12-lead ECG. AFib algorithms are designed to look for particular rhythm patterns, not every arrhythmia or cause of symptoms. A normal watch result does not rule out coronary artery disease, a heart attack, stroke, blood clots, valve disease, cardiomyopathy or a problem that is not happening while the watch records.

Some readings may be unusable because of movement, a loose fit, poor contact, sweat, cold skin, tattoos or electrical noise. Device and feature availability can also depend on the watch model, phone, operating system, user eligibility and country. For example, Apple says its ECG app is available on Series 4 or later and listed Ultra models, subject to regional availability. Check the manufacturer’s current instructions for your specific device.

Hypertension notifications deserve special care: a notification about possible risk is not a blood-pressure measurement. For actual readings, use a validated upper-arm cuff and seek clinical guidance. In the United States, the FDA warns against unauthorized devices claiming to measure blood pressure.

What to do after an alert

  1. Act on symptoms, not the screen. Chest pain or pressure, fainting, severe shortness of breath, or suspected heart attack calls for emergency help, even if the watch shows a normal result. Apple explicitly warns that its irregular-rhythm feature cannot detect a heart attack.
  2. If you feel well, pause and follow the device instructions. Sit quietly and, if your watch supports it, take an ECG recording. Do not keep checking repeatedly in a way that delays contacting a clinician.
  3. Save useful information. Keep the recording or alert and note when it happened, what you were doing, any symptoms, and whether it recurred. This can help a clinician decide what testing is appropriate.
  4. Contact a clinician about an unexplained rhythm alert. Confirmation may involve a 12-lead ECG or an ambulatory monitor such as a Holter, patch or event monitor. Other tests depend on the suspected problem; an echocardiogram, for example, assesses heart structure and pumping function.
  5. Do not change treatment based only on a watch. Do not start, stop or adjust anticoagulants, beta-blockers, blood-pressure medication or other medicines without medical advice.

The FDA is studying how cardiovascular notifications affect follow-up and care. Its guidance reinforces the distinction between a notification and a diagnosis: FDA: post-market evaluation of smartwatch cardiovascular notifications.

Who may get the most value?

Wearable rhythm monitoring may be especially useful for people at elevated risk of AFib or stroke, people with intermittent palpitations or suspected paroxysmal AFib, and patients whose clinician has recommended monitoring. It can also be useful when a clinician has defined how alerts should be confirmed and acted on. The EQUAL trial’s higher-risk participants are not a stand-in for every young, low-risk, symptom-free consumer.

If you already have AFib, follow your clinician’s monitoring plan rather than assuming a consumer notification feature is suitable for you. Some products, including Fitbit’s irregular-rhythm notification feature, state that it is not intended for people with known AFib. Likewise, confirm regional and phone compatibility before relying on a feature: Samsung Health Monitor ECG and irregular-rhythm functions depend on market, device and software availability.

For suspected intermittent rhythm problems, clinical options include a Holter, patch or event monitor; for a broader diagnostic snapshot, a clinician may use a 12-lead ECG. A validated cuff is the appropriate tool for blood-pressure measurements. A watch’s advantage is convenient, repeated monitoring; clinical tools offer more targeted or definitive assessment.

What the evidence means for smartwatch buyers

If heart-rhythm screening is your reason for considering a watch, look for the specific capability you need: passive irregular-rhythm notifications, an on-demand ECG, the ability to share a tracing, and compatibility with your phone and country. Wearing the device consistently matters, and alerts are most useful when you know how to get them checked. Do not choose a device on the assumption that it can predict heart attacks, replace a prescribed monitor or measure blood pressure unless that precise feature and intended use are supported for your model and region.

Smartwatches are becoming more credible tools for finding hidden AFib, particularly in people at higher risk. Evidence for predicting broader heart deterioration is earlier and more conditional. The practical value lies in using a watch as one source of information—and getting symptoms or alerts assessed through the right medical pathway.
