FDA qualifies Apple Watch AFib History feature for clinical-trial research—but the scope is narrow

On May 1, 2024, the FDA qualified Apple’s AFib History feature under its voluntary Medical Device Development Tools (MDDT) program. The qualification permits a specific use: estimating atrial-fibrillation (AFib) burden as a secondary effectiveness endpoint in clinical studies of cardiac-ablation devices. It did not approve every Apple Watch as a general medical-research instrument, make the watch a continuous monitor, or authorize it to diagnose AFib or guide treatment on its own.

The feature was already FDA-cleared as an over-the-counter, software-only medical application (clearance K213971). The 2024 action recognized how that existing tool may be used in a defined device-development context, not the safety or effectiveness of any ablation treatment.

What the FDA actually qualified

FDA terminology matters here. Clearance allows the AFib History software to be marketed for its indicated medical use. An MDDT qualification means the FDA has determined that a measurement tool can be used in a specified medical-device-development setting. It is not approval of a treatment, a clinical outcome, or the Apple Watch product line as a whole.

Under the qualification, sponsors of cardiac-ablation-device studies may use AFib History as a secondary effectiveness endpoint. Investigators can compare estimated burden between study groups or before and after ablation, while still relying on the protocol’s primary safety and effectiveness endpoints. The qualification does not establish what reduction in AFib burden is clinically meaningful and does not, by itself, prove that an ablation device works. FDA qualification summary

How AFib History estimates burden

AFib burden is the estimated percentage of time a wearer appears to be in AFib during periods when the watch is worn. The feature uses the watch’s photoplethysmography (PPG) sensor to analyze pulse-to-pulse intervals. It produces a retrospective estimate for the preceding seven days after sufficient readings are collected; it is not a live, continuous ECG.

  • Burden: an estimate of the proportion of monitored time showing AFib patterns.
  • Detection: an indication that an episode may be occurring; AFib History is not an alarm that alerts you during an episode.
  • ECG: a waveform recording from the separate ECG app on compatible models.
  • Continuous monitoring: AFib History does not provide this; estimates cover watch-wearing periods and can miss episodes.

What evidence supported the qualification?

The FDA reviewed a prospective study in which 280 subjects contributed to the primary endpoint analysis. Participants had known paroxysmal, persistent, or permanent AFib, and weekly watch estimates were compared with a Cardea SOLO Wireless ECG Patch reference.

Apple’s U.S. instructions for use describe 413 people enrolled in the broader validation study, with 280 included in the primary analysis. In that indicated population, Apple reports sensitivity of 92.6% and specificity of 98.8%. Those figures describe agreement for this feature in the studied group; they are not the probability that any Apple Watch user will receive a correct diagnosis. Why researchers may find it useful

  • Passive data collection during ordinary watch wear can reduce visits and participant workload.
  • Weekly estimates provide a standardized cadence for tracking burden over time.
  • Sponsors can compare burden before and after an ablation or between randomized study arms.
  • Commercial hardware may support decentralized or hybrid trial designs without an implanted monitor.

Those advantages do not remove adherence and missing-data problems. Protocols still need to define meaningful thresholds, account for wear time and unavailable weeks, and specify how the AFib History endpoint relates to the primary outcome.

Who should use the feature?

Apple positions AFib History for people who are 22 or older and already have a physician diagnosis of AFib. It is not presented as a population-screening tool for people without a diagnosis. Consistent estimates generally require wearing the watch at least 12 hours a day on five days each week. The feature is intended to support discussion with a clinician, not to replace clinical assessment or authorize medication changes.

Supported hardware and software

The FDA qualification document identifies Apple Watch Series 4 or later, all Apple Watch SE models, and Apple Watch Ultra models running watchOS 9 or later with an iPhone 8 or later. Apple’s lineup and regional software support change, so verify current compatibility before buying an older or refurbished watch. How a diagnosed patient turns on AFib History

  1. Open the Health app on the iPhone.
  2. Tap Search or Browse, then choose Heart.
  3. Tap AFib History, select Set Up, and tap Get Started.
  4. Enter your date of birth and confirm that a doctor diagnosed AFib.
  5. Follow the prompts and tap Done.

Heart Rate and Wrist Detection must be enabled. Low Power Mode can disable background heart measurements and prevent an estimate. If readings are insufficient, no weekly result is generated. Apple Support setup and requirements

What appears in the Health app

The app shows a weekly percentage and longer-term trends. After enough data accumulates, AFib History Highlights can show patterns by day and time, and lifestyle comparisons can include exercise, sleep, weight, alcohol consumption, and mindful minutes. You can export the history as a PDF to share. Apple displays the lowest result as “2% or less”, not 0%, so that label should not be interpreted as proof that no AFib occurred.

Important limitations for patients

  • AFib History is not continuous and may miss episodes when the watch is off, loose, charging, or generating too few readings.
  • It is not intended to detect heart attack, stroke, atrial flutter, or atrial tachycardia reliably.
  • A low estimate does not rule out clinically important events.
  • Do not change anticoagulants or other AFib treatment based on the percentage; consult your clinician.
  • Chest pain, severe shortness of breath, fainting, or other emergency symptoms require urgent medical care, regardless of the watch display.

These limitations also matter in trials: sensor contact, battery status, wear adherence, device generation, software version, geography, and missing weeks should be controlled in the protocol.

Which Apple Watch is a sensible fit?

For AFib History alone, the newest or most expensive watch is not inherently necessary. The FDA-qualified families include Series 4 or later, SE, and Ultra models when the required software and iPhone are supported. A current Apple Watch SE is the lower-cost route into that family; a standard Series model suits buyers who also want newer general-purpose features. Ultra and Hermès models add durability, battery or luxury design, not a broader AFib research qualification. Check Apple’s current compatibility and pricing before purchase.

An Apple Watch is a poor choice for someone without an AFib diagnosis seeking definitive screening, anyone who needs continuous ECG-quality monitoring, or a trial whose protocol specifies another monitor. Clinician-directed ambulatory monitoring remains more appropriate for diagnosis, medication decisions, symptoms, or formal continuous recording.

Research tool, not a new diagnostic approval

The FDA action is best understood as approval of a measurement pathway for a narrow clinical-trial purpose. AFib History can supply standardized, passive estimates of burden as a secondary endpoint in cardiac-ablation-device studies. It does not turn the Apple Watch into a universal AFib screener, replace ECG or physician evaluation, or validate any particular treatment.
