How Apple Watch Uses Its Accelerometer to Spot Possible Sleep Apnea

Apple Watch does not watch your airway close or measure apnea directly. During sleep, its triaxial accelerometer records large movements and tiny wrist motions associated with breathing. Algorithms turn those time-series signals into a nightly Breathing Disturbances result, then examine the pattern over about 30 days. A persistent pattern can produce a Sleep Apnea Notification for possible moderate-to-severe sleep apnea.

That notification is a screening prompt, not a diagnosis, an apnea-hypopnea index (AHI), a treatment monitor, or a substitute for a home sleep-apnea test or polysomnography.

The sensor is the accelerometer—not the blood-oxygen monitor

The sleep-apnea feature’s stated input is the Apple Watch accelerometer. It measures motion along three axes (often represented as X, Y and Z), including subtle movements transmitted to the wrist as the chest and body move during breathing. Apple’s described validation compares accelerometer signals with conventional respiratory measurements such as abdominal belts and airflow sensors; the watch is classifying related motion patterns, not measuring airflow at the nose or mouth.

The feature therefore does not require blood-oxygen readings. That distinction matters for Apple Watches sold in the United States whose pulse-oximeter function is disabled because of the Masimo patent dispute. Oxygen saturation remains clinically relevant to sleep apnea, but it is not the stated detection input for this Apple feature. From wrist motion to “Breathing Disturbances”

1. Sleep tracking records a motion signal

When you wear the watch overnight with sleep tracking active, it records an accelerometer time series. Breathing can create small, repeating body movements that reach the wrist, while turning over, getting up and other activity create much larger signals.

2. Machine learning separates likely respiratory patterns from noise

Apple says machine-learning algorithms were developed using thousands of nights of clinical-grade sleep-apnea tests. At a high level, the system looks for changes over time that resemble interruptions in normal respiratory rhythm and distinguishes them from ordinary movement. Apple has not publicly specified the model architecture, thresholds, sensitivity or specificity in the materials cited here, so an “elevated” result should not be treated as a quantified event count.

3. Each night receives a simple classification

The user-facing result is whether Breathing Disturbances were elevated or not elevated. This derived metric is not the clinical AHI, which is scored from a sleep study using physiological signals and defined event rules.

How the 30-day notification works

A single elevated night is not the trigger. Apple Support says the notification algorithm evaluates a pattern over a 30-day period. If the pattern consistently suggests possible moderate-to-severe sleep apnea, the watch and iPhone can issue a Sleep Apnea Notification. The wording describes a possible condition and intended screening target; it does not assign your personal severity.

Apple’s launch description says the Health app can show historical views over one month, six months and one year. A notification includes the period in which potential sleep apnea may have occurred, educational information and a PDF containing three months of Breathing Disturbances data to discuss with a healthcare professional. Which Apple Watch models support it?

At the watchOS 11 launch on September 16, 2024, Apple listed the feature for Apple Watch Series 9, Series 10 and Apple Watch Ultra 2. Apple said the feature became available in more than 150 countries and regions after regulatory authorizations, but availability depends on country, language and authorization.

Later Apple product announcements continue to list sleep-apnea notifications among Apple Watch health capabilities. Do not assume that every newer model or every country supports it: check Apple’s current regional feature list and the model’s requirements before buying. How to turn on and review Sleep Apnea Notifications

Set up the feature on iPhone

  1. Open the Health app.
  2. Tap your picture or initials.
  3. Tap Health Checklist.
  4. Under Sleep Apnea Notifications, tap Set Up.
  5. Follow the on-screen instructions.
  6. Wear the compatible Apple Watch to bed with sleep tracking enabled.

Review your data

  1. Open Health.
  2. Tap Search.
  3. Tap Respiratory.
  4. Tap Breathing Disturbances.

Apple can change labels or menu locations between iOS and watchOS releases, so the exact wording may differ on your devices. A loose fit, removing the watch overnight, a dead battery or disabled sleep tracking can reduce usable sleep data.

Is it as accurate as a sleep study?

No. Apple says the feature was validated in what it describes as the largest study conducted for FDA clearance of a sleep-apnea device, and says every participant identified by the algorithm had at least mild sleep apnea. Those are claims from Apple’s announcement, not independent estimates of sensitivity, specificity or predictive value. The feature received clearance for its intended notification use; that does not make the entire Apple Watch a diagnostic sleep-apnea device.

A home sleep-apnea test or laboratory polysomnography can establish a diagnosis and, where appropriate, produce an AHI. Apple’s Breathing Disturbances result cannot be converted into an AHI category or used as a replacement for either test. Apple Support also says the feature is not intended for people already diagnosed with sleep apnea or for anyone under 18.

What can make disturbances look elevated?

Apple notes that alcohol, medications, sleep position and other factors can influence Breathing Disturbances. Ordinary night-to-night variation, movement artifacts, unusual sleeping conditions and insufficient usable data can also affect what the algorithm sees. The watch is inferring a pattern from motion, so an elevated result does not identify the cause.

What to do after a notification

  • Save or open the notification’s PDF and take it to a healthcare professional.
  • Describe symptoms such as loud snoring, witnessed breathing pauses, gasping, morning headaches, daytime sleepiness or difficult-to-control hypertension.
  • Let the clinician decide whether a home sleep-apnea test or overnight polysomnography is appropriate for your history and symptoms.
  • Do not start, stop or change CPAP or another treatment solely because of the watch.

A notification is a reason to seek evaluation, not proof that you have sleep apnea. Apple’s support guidance states that the watch is not intended to replace a doctor for diagnosis, treatment or monitoring.

What if you have symptoms but no notification?

No notification does not rule out sleep apnea. The algorithm can miss events because of limited sleep coverage, fit, movement, unusual conditions or model limitations, and the feature is designed as a screening aid rather than a rule-out test. If you or a bed partner notice persistent snoring, pauses, gasping, excessive daytime sleepiness, morning headaches or related risk factors, discuss them with a clinician even when the Health app shows no elevated pattern.

What the feature cannot tell you

  • It cannot see an airway obstruction or directly measure airflow.
  • It does not measure oxygen drops for this feature.
  • It does not provide an AHI or confirm disease severity.
  • It cannot prove that breathing is normal when no notification appears.
  • It is not intended to monitor diagnosed sleep apnea or determine whether CPAP is working.

Bottom line

Apple Watch uses wrist-motion data from its accelerometer to classify breathing-disturbance patterns, then looks for a persistent signal across roughly 30 days. That makes it a potentially useful, low-friction prompt to seek medical evaluation—especially when symptoms are present—but not a sleep study, AHI calculator or treatment monitor.
