The Galaxy Watch5’s headline health upgrade is an infrared skin-temperature sensor. It records overnight temperature trends for Samsung Health—most notably to support menstrual-cycle predictions—but it is not a fever thermometer or a general-purpose core body-temperature monitor. The larger upgrade is cumulative: Samsung combines that new signal with its BioActive heart, ECG and body-composition sensors, stronger overnight battery performance and Wear OS software.
For a Galaxy Watch4 owner, the Watch5 is an incremental but meaningful health-platform upgrade if you will sleep in the watch and use Samsung Health. It is a poor reason to upgrade if you want clinical temperature readings, a medical diagnostic device or the newest software support available in 2026.
What is actually new on the Galaxy Watch5?
The new hardware is an infrared temperature sensor. Samsung lists it alongside the existing BioActive Sensor, accelerometer, barometer, gyroscope, geomagnetic sensor and light sensor. The BioActive assembly combines optical heart-rate sensing, electrical heart-signal sensing and bioelectrical impedance analysis (BIA) for body-composition estimates. In other words, Watch5 adds another physiological input to an established health architecture rather than replacing it.
Samsung announced the Watch5 and Watch5 Pro in August 2022. The standard model came in 40mm and 44mm cases, launched with Wear OS 3.5 and One UI Watch 4.5, and was rated 5ATM, IP68 and MIL-STD-810H. Launch software and current software are not necessarily identical after later updates.

Skin temperature is not body temperature

Samsung’s developer documentation explicitly distinguishes skin temperature from body temperature: skin-temperature data specifications. Do not use a Watch5 reading to decide whether you are sick or to replace medical advice.
How Samsung Health uses the temperature data
The most visible consumer use is skin-temperature-assisted menstrual-cycle tracking, introduced through Samsung’s partnership with Natural Cycles in selected markets, including the United States. Availability depends on country, device market, software and regulatory approval.
- Open Samsung Health and select the Home tab.
- Open the Cycle tracking tile.
- Tap More options (the three vertical dots), then Settings.
- Enable Predict period with skin temp.
- Wear the Watch5 while sleeping and return to Cycle tracking to view data and updated predictions.
Samsung recommends at least four consecutive hours of wear between 7 p.m. and 9 a.m. Consistent watch position, room conditions, clothing and posture help because these affect skin readings. Samsung’s cycle FAQ says basal temperature generally rises about 0.3–0.5°C around the period before and after ovulation; that is Samsung’s documented explanation, not a guarantee of individual performance. The system presents predicted periods, fertile windows and ovulation dates, not certainty or universally reliable contraception. Illness, medication, travel, entered data and disrupted routines can affect results.
You do not need a Watch5 for basic cycle predictions. Samsung Health can use manually entered period history; the Watch5 adds overnight temperature trends intended to improve those predictions. If you also use a Galaxy Ring, Samsung recommends choosing one device for temperature tracking because wrist and finger temperatures differ.

Why the BioActive Sensor still matters
The three-in-one BioActive Sensor gives Samsung Health a broader foundation:
- Optical heart rate: pulse and exercise-related heart-rate trends.
- Electrical heart signal: the basis for supported single-lead ECG recordings.
- BIA: estimates of body-composition metrics.
Samsung’s Health Sensor SDK also identifies data types such as PPG, SpO₂, accelerometer, electrodermal activity and sweat-loss data, although not every type is exposed on every Watch5 model, software version or region. Sensor integration does not make each metric clinically equivalent to dedicated medical equipment.
Sleep is the connective tissue
Temperature tracking depends on sleeping in the watch. Samsung Health separately records sleep duration and stages, while Advanced Sleep Coaching interprets patterns and offers guidance. Samsung says coaching requires seven days of sleep wear followed by a survey. Temperature is therefore an additional overnight signal—primarily surfaced through cycle tracking—not automatically a universal sleep-temperature dashboard in every software release.

This creates a practical trade-off: charging must not routinely interrupt sleep. The Watch5’s larger battery and faster charging help, but comfort, fit and missed nights still determine how continuous your data is.
Other health and fitness functions
Heart rate, SpO₂ and activity
Samsung Health tracks heart rate, blood oxygen, stress, steps, daily activity, GPS workouts, elevation and common exercise metrics. Compatible Wear OS apps and Google services extend the platform.
ECG
Samsung Health Monitor can record an ECG and share it to a connected phone where supported. Sit still, clean and dry the sensor, wear the watch snugly above the wrist bone and follow the on-screen instructions. Samsung says the information is wellness or screening information, not a diagnosis or treatment. Regional, age and medical-device restrictions apply: Samsung ECG requirements.
Blood-pressure estimates
Blood-pressure functionality requires an upper-arm cuff. In Samsung’s current US instructions, calibration uses three cuff readings within 30 minutes and must be repeated every 28 days. The watch estimates pressure; it is not a cuff replacement and is not intended to diagnose or treat hypertension: Samsung calibration instructions.

These specifications come from Samsung’s Watch5 launch information and support documentation. The Watch5 Pro is a separate product with titanium, a 590mAh battery, GPX and Route Workout support and Track Back navigation; those are not standard Watch5 features.
Compatibility and regional limits
The Watch5 cannot be set up with an iPhone and is not a standalone watch. At launch Samsung specified an Android 8.0-or-later phone with at least 1.5GB of RAM, the latest Samsung Health app and the Galaxy Wearable app. Current requirements can differ after updates.

Samsung’s current compatibility guidance says Samsung Health Monitor functions—including ECG, irregular-rhythm notifications and sleep-apnea features—are restricted to Samsung phones: compatibility details. Regulatory availability also differs by market.

No temperature data
- Confirm Predict period with skin temp is enabled.
- Wear the watch for the recommended four consecutive overnight hours.
- Update the watch software and Samsung Health.
- Check market, account and phone compatibility.
- Improve fit and keep overnight conditions consistent.
Cycle predictions look wrong
Review entered cycle dates and consider illness, medication, travel or disrupted sleep. Treat predictions as estimates, not contraception or a substitute for reproductive-health advice.

ECG fails or is noisy
Remain still, dry and clean the sensor, wear the watch snugly and check regional support. Persistent symptoms require medical attention, not repeated watch readings.
Blood pressure is unavailable
Check country support, Samsung-phone requirements and cuff calibration status. The Watch5 cannot independently replace an upper-arm cuff.
Should a Galaxy Watch4 owner upgrade?
Consider the Watch5 if you already use a Galaxy phone and Samsung Health, will wear the watch overnight, want skin-temperature-assisted cycle predictions, or value the larger battery, sapphire crystal and closer-fitting rear sensor.
Skip it as a temperature purchase if your goal is fever measurement, clinical monitoring, maximum sports battery life, iPhone compatibility or the longest remaining software-support horizon. The Watch5 is a 2022 product; in 2026, compare used-unit battery condition and price with newer models on Samsung’s current comparison page.

A validated upper-arm cuff remains essential for anyone using blood-pressure estimates. Fitbit and Garmin can be alternatives for different sleep or training priorities, but their ecosystems and feature requirements differ.
