They are most useful for tracking trends over time and for selected applications with evidence and appropriate medical oversight. A smartwatch can support wellness monitoring; its readings should not automatically be treated as a diagnosis or a basis for changing treatment.
What wearable sensors can contribute to care
A wearable combines one or more sensors with power, computing and a wireless connection. Watches and bands commonly track movement and pulse; some also provide sleep estimates, oxygen-saturation readings or ECG-related features. Emerging devices aim to measure chemicals in sweat, saliva, tears or fluid beneath the skin. What a device can measure, and how reliably, depends on its sensors, design and intended use.
The practical promise is continuity: data collected between appointments may help a person and clinician notice changes that a single office measurement would miss. That promise depends on whether the signal is useful for the particular question, whether someone can review it, and whether there is a clear plan for responding.
Home monitoring and remote follow-up
Wearables can support telemedicine and follow-up at home, reducing some trips to clinics and extending monitoring to people in remote or underserved areas. They may help clinicians review trends or decide when further assessment is warranted. A sensor does not provide the care pathway by itself: data quality, connectivity and a reliable escalation process matter.

Prevention, rehabilitation and chronic care
Activity and physiological trends can help people and care teams monitor rehabilitation, adherence and self-management. Wearable monitoring has also been studied for issues such as atrial fibrillation, abnormal pulse, falls, seizures, respiratory changes and infection-related changes. Evidence is not equally strong across these uses; the existence of a feature or alert does not establish that it can detect a disease accurately in everyday use.
What the evidence says about potential savings
A 2024 systematic review in Mayo Clinic Proceedings: Digital Health included 10 studies of the economic impact of wearable health technology. It found potential quality-adjusted life-year gains and cost-effectiveness or savings in some settings. The review also emphasized that results depend on the device and condition, the economic perspective, local costs and the willingness-to-pay threshold. The finding is not proof that wearables generally lower healthcare spending.

Costs can shift rather than disappear. A program may avoid some travel or support earlier follow-up, yet require devices, connectivity, maintenance, staff time to review readings, and systems for handling alerts. Whether the total is lower depends on how the program is designed and evaluated.
How broad—and how mature—is the evidence?
Wearable health studies cover a range of conditions, but reviews also identify limits in study size, duration and real-world testing. These figures describe particular reviews and should not be read as pooled estimates for every wearable or clinical use.

In general, heart-rate tracking and some atrial-fibrillation applications have comparatively stronger support than broad claims that a consumer wearable can detect many diseases. Performance still depends on the person, measurement conditions and intended use.
Wellness tracker or medical sensor?
Choose according to the decision you need to make, not the number of sensors listed on a product page. A fitness tracker can be useful for activity or pulse trends without being a diagnostic device. A medical sensor intended to guide care is a distinct category, with specific authorization, eligibility and use conditions.

What can make a wearable unreliable or hard to use in care?
Measurement and engineering limits
- Movement and signal quality: Motion can interfere with readings, while sensors may drift or require calibration over time.
- Biochemical selectivity: Emerging sweat and other fluid sensors may struggle to distinguish the target of interest reliably.
- Wearability: Battery capacity, charging frequency, comfort, skin compatibility and washability affect whether a device can be worn consistently.
- Data handling: Limited memory, secure transmission and compatibility with clinical systems can constrain continuous monitoring.
Care delivery and access
- Alert responsibility: Programs need to define who reviews alerts, how quickly they act and what happens when a reading suggests a problem.
- Interoperability: Fragmented data standards can make it difficult to move readings into existing clinical workflows.
- Staff time and payment: Clinician workload and reimbursement affect whether remote monitoring can be sustained.
- Unequal connectivity: A program that requires a smartphone or reliable internet may exclude people without consistent access to either.
- Privacy and security: Health-related data need appropriate protections during collection, storage and transmission.
What FDA guidance means for smartwatch health claims
In the United States, the FDA’s general-wellness guidance gives the example of a wrist-worn product measuring “hours slept, sleep quality, pulse rate, and blood pressure” for wellness, provided its claims do not imply disease diagnosis or clinical equivalence. A wellness feature is not the same as authorization to diagnose or treat a condition.
Be especially cautious about needle-free glucose claims
In a February 21, 2024 safety communication, the U.S. Food and Drug Administration stated: “The FDA has not authorized, cleared, or approved any smartwatch or smart ring that is intended to measure or estimate blood glucose values on its own.” Do not rely on a watch or ring claiming to measure glucose without penetrating the skin for glucose readings or treatment decisions.

This does not mean every wearable glucose product is the same as a watch or ring making that claim. In March 2024, the FDA cleared Dexcom Stelo as an over-the-counter continuous glucose monitor for adults 18 and older who do not use insulin. It uses a wearable sensor paired with a smartphone and is a regulated glucose-monitoring product. Its clearance and eligibility are specific to that product and use.

A practical checklist for choosing a wearable or care program
- Define the question. Decide whether you want a wellness trend, a prompt to seek assessment, or data to support a clinician’s medical decision.
- Check the intended use. For medical decisions, verify the product’s regulatory status and whether the intended use and user eligibility match your situation.
- Look for evidence in the relevant population. Ask whether the metric has been validated for the people and conditions in question, and whether evidence comes from real-world use rather than only short or laboratory studies.
- Assess everyday usability. Consider comfort, charging, battery life, skin compatibility, cleaning and the phone or connectivity requirements.
- Understand what happens to the data. Check privacy and security practices, whether information can reach the relevant care team, and who is responsible for reviewing it.
- Count the full cost. Include the device, any subscription or connectivity costs, maintenance and the clinical time needed to act on readings.
- Agree on an escalation plan. Know what readings or symptoms should prompt contact with a clinician, and do not let an app alert replace urgent care when symptoms require it.
