Short answer: Neupulse is a watch-like, app-controlled device that delivers rhythmic electrical pulses to the median nerve at the wrist. In a 2023 UK double-blind, sham-controlled trial of 121 people with Tourette syndrome or chronic tic disorder, four weeks of home treatment produced a greater reduction in tic measures than sham stimulation. That is encouraging evidence, not proof of a cure or a replacement for behavioral therapy and medication. Availability, regulatory status, price and delivery remain market-specific.
What Tourette syndrome involves
Tourette syndrome is a neurodevelopmental tic disorder involving repeated involuntary movements and sounds. Motor tics can include blinking, jerking or head movements; vocal tics include sounds, words or throat clearing. Many people feel a premonitory urge or mounting tension before a tic, followed by temporary relief after it occurs.
Tics naturally wax and wane. Stress, fatigue, excitement and focused attention can change their frequency or intensity from hour to hour. Tourette syndrome requires both motor and vocal tics over time; chronic motor or vocal tic disorder involves one category. Mild symptoms may not require treatment, and the right decision depends on impairment, distress, co-occurring ADHD, OCD, anxiety or depression, age and personal preference.
What Neupulse is—and is not
Neupulse is a wrist-mounted, watch-like wearable developed by University of Nottingham researchers and their spin-out company. It is controlled through an app and is designed to provide mild, rhythmic peripheral electrical stimulation to the median nerve at the wrist. The commercial company now trades as Neupulse (University of Nottingham explanation).

The concept is different from a pain-relief TENS unit, muscle-building EMS device, deep-brain stimulation, transcranial electrical stimulation or a Pavlok-style aversive bracelet. It does not identify an individual tic and punish the wearer. The pulses are intended to modulate nerve and brain activity continuously during a session, rather than condition someone to fear a movement (University description).
What the clinical trial tested
The key evidence is a peer-reviewed 2023 Journal of Neuropsychology study (17(3), 540–563) of rhythmic 10-Hz median-nerve stimulation (trial paper). The UK-wide study enrolled 121 people with Tourette syndrome or chronic tic disorder. It was double-blind and sham-controlled: participants received either active stimulation or a convincing sham condition at home.

University reporting said active-device users had significant reductions in tic frequency and severity, and that 59% achieved at least a 25% reduction in tic severity compared with the sham group (University trial announcement). The percentage is a responder threshold, not a promise that every user’s tics fell by exactly 25%. The paper is the appropriate source for the precise denominator, analysis population, endpoint definition and statistical comparison. A score reduction also does not mean that tics disappeared, that motor and vocal tics improved equally, or that benefits continued after stimulation stopped.

How wrist stimulation might influence tics
The research hypothesis is that rhythmic stimulation of the median nerve can entrain brain oscillations involved in movement control and suppression. In this model, signals enter through a peripheral nerve and influence sensorimotor networks; the device is not electrically stimulating the brain directly. Foundational work on this approach is described in Current Biology (2020 study), with a University research summary at Nottingham’s commercialisation announcement.
“Entraining” is a proposed neurophysiological mechanism, not evidence that the device permanently rewires the brain. A tic reduction during or after a session does not establish that the underlying disorder has been eliminated, and individual response may vary.
How strong is the evidence?
This is an unusually encouraging early result for a non-invasive device because it included a sham control and blinding. It is still a limited evidence base.

- The principal controlled treatment period lasted only four weeks, so it cannot establish six-month or multi-year durability, habituation or what happens when use stops.
- A prototype used in a trial may not be identical to final commercial hardware, electrodes or software.
- Tics fluctuate naturally, making short before-and-after comparisons difficult even with a sham group.
- Expectancy and placebo effects remain relevant for a wearable that users can feel.
- Participants in a UK study may not represent young children, people with severe comorbidities or populations outside the UK.
- The study does not show that Neupulse is more effective, safer, cheaper or better for quality of life than established care.
Replication, longer follow-up and evidence on real-world use are needed before the device can be considered a settled standard treatment. The peer-reviewed paper—not early news coverage referring to a preprint—is the strongest clinical citation (Journal of Neuropsychology).
Does Neupulse replace therapy or medication?
No. Nothing in the cited evidence supports replacing Comprehensive Behavioral Intervention for Tics, habit-reversal-based therapy, prescribed medication or treatment for ADHD, OCD, anxiety or depression. Neupulse may eventually be an additional option, or an alternative for selected people who prefer a non-drug, home-administered approach, but suitability should be assessed with a qualified clinician.

Who might consider it?
It may be relevant to someone with bothersome motor or vocal tics who wants a non-invasive, non-drug intervention, can follow a regular schedule and has access to an approved product and appropriate support. It is not a guaranteed fit for every tic type, severity level or diagnosis. People whose greatest impairment comes from a co-occurring condition may need treatment aimed at that condition rather than tic reduction alone.

Safety questions to settle before use
“Non-invasive” does not mean risk-free. Final instructions for use should govern eligibility. Ask a clinician and check the manufacturer’s warnings if you have an implanted electronic device such as a pacemaker, epilepsy, neuropathy, significant heart disease, are pregnant, or are considering use in a child. Also ask about use with broken or irritated skin and whether the product is authorized for the user’s age.
Possible practical problems with peripheral stimulation include electrode irritation, tingling, discomfort, pain, numbness or temporary sensory changes; the trial and final labeling should be consulted for the recorded adverse events and withdrawal data. Do not use it while sleeping, bathing, driving or exercising unless the instructions explicitly permit those activities. Stop if stimulation is painful or produces unusual symptoms and contact the manufacturer or a clinician.

Research publication and product authorization are separate questions. A University of Nottingham update said the company was working toward CE and UKCA marking, with approval expected in 2026; that statement is not confirmation that approval was granted in every market (University/NICE update).

- Avoid substituting an unregulated electrical-stimulation product bought online; generic TENS, EMS and “nerve stimulation” wearables have not been established by this evidence as interchangeable Tourette treatments.
