Apple Watch hypertension-alert feature receives FDA clearance: what Series 11 and older models can—and cannot—do

Apple’s Hypertension Notification Feature is FDA-cleared, but Series 11 is not a blood-pressure monitor. Here is what the feature does, which watches support it, and what to do after an alert.

Apple’s Hypertension Notification Feature received FDA 510(k) clearance on September 11, 2025. The clearance applies to Apple’s software-based notification technology—not to the Apple Watch Series 11 as a blood-pressure monitor. The feature can flag patterns associated with possible chronic hypertension, but it does not display blood-pressure readings or diagnose the condition.

What the FDA cleared

The FDA cleared Apple’s Hypertension Notification Feature through the 510(k) pathway. The FDA database identifies the cleared technology as Hypertension Machine Learning-Based Notification Software, submitted by Apple under 510(k) number K250507. The decision date was September 11, 2025.

That wording matters. This is FDA clearance, not FDA approval of an Apple Watch blood-pressure monitor. The cleared product is software that generates a possible-hypertension notification. It does not authorize the watch to diagnose hypertension or replace conventional blood-pressure measurement.

How Apple Watch hypertension notifications work

The feature operates passively in the background, using data from the Apple Watch’s optical heart sensor. Apple says the software analyzes how blood vessels respond to heartbeats and looks for patterns associated with chronic high blood pressure.

It evaluates data over a rolling 30-day period, rather than taking a single reading at a particular moment. The watch therefore does not show systolic or diastolic numbers, and the notification is not a real-time blood-pressure measurement or continuous blood-pressure monitor.

Apple says the algorithm was trained using data from multiple studies involving more than 100,000 participants. Its validation study included more than 2,000 participants; the U.S. instructions for use specify 2,229 subjects who had no previous hypertension diagnosis. Those figures describe Apple’s development and validation work, not a guarantee of diagnostic accuracy for every wearer.

Which Apple Watch models support the feature?

Despite its association with the Series 11 launch, the feature is not exclusive to that model. Apple’s current support documentation lists:

The feature is not available on Apple Watch SE. Availability can also vary by country or region because regulatory authorization and software availability are not identical worldwide. Check Apple’s current support page for the latest local requirements.

Who can use it?

Apple says users must meet additional conditions:

  • Be at least 22 years old
  • Not be pregnant
  • Not already have a hypertension diagnosis
  • Have Wrist Detection enabled
  • Use a supported Apple Watch and iPhone
  • Live in a region where the feature is available

A person already diagnosed with hypertension should follow their clinician’s monitoring and treatment plan rather than relying on this screening feature. The pregnancy restriction is another reason not to generalize the feature’s eligibility to every Apple Watch owner.

How to turn on hypertension notifications

Apple’s current iPhone Health app path is:

  1. Open the Health app.
  2. Tap the profile icon in the upper-right corner.
  3. Under Features, tap Health Checklist.
  4. Tap Hypertension Notifications.
  5. Confirm your age and whether you have ever been diagnosed with hypertension.
  6. Tap Continue, review the information, and tap Done.

Apple may change menu names or paths in later iOS releases, so treat this as the current setup route rather than a permanent interface guarantee. If the option is missing, check regional availability, device compatibility, software versions, pregnancy status in the Health app, and whether an existing diagnosis makes you ineligible.

What to do if the watch sends an alert

A notification means the software found patterns in recent heart-sensor data associated with possible hypertension. It is a prompt for confirmation—not a diagnosis and not, by itself, an emergency warning.

Apple directs users to set up a Blood Pressure Log and take readings with a third-party FDA-cleared blood-pressure monitor for seven days. Those readings should then be discussed with a healthcare professional. The watch cannot complete this confirmation step because it does not produce the required blood-pressure values.

Use the cuff and logging process your clinician recommends. The sources establish Apple’s requirement for an FDA-cleared monitor but do not establish a specific brand, model, cuff size, price, or app integration that should be recommended universally.

Important: Chest pain, pressure or tightness, severe shortness of breath, fainting, or other emergency symptoms require emergency medical care. Do not wait for an Apple Watch notification or use one as a substitute for emergency services.

What the feature cannot tell you

  • It cannot provide a systolic or diastolic blood-pressure reading.
  • It cannot diagnose, treat, or manage hypertension.
  • It cannot identify every person who has high blood pressure.
  • A notification does not prove that you have hypertension.
  • No notification does not prove that your blood pressure is normal.
  • It does not detect heart attacks.
  • It is not a replacement for a cuff or clinical evaluation.

Apple’s U.S. instructions for use also warn that new data cannot be collected when Apple Watch storage is full. Poor watch contact, incomplete sensor data, fit issues, health conditions, and ordinary real-world use can affect the amount and quality of usable data.

How accurate is it?

A published population-screening analysis reported that approximately 41.2% of people with undiagnosed hypertension received an alert, while approximately 92.3% of participants without hypertension did not receive one. These figures illustrate the feature’s role as a screening prompt: a substantial number of people with hypertension did not receive an alert, and an alert still requires confirmation.

They should not be treated as an individual diagnostic guarantee. Performance can differ across populations and circumstances, including sensor contact, wrist fit, health conditions, and usage patterns. The published analysis is available through PubMed Central; Apple’s technical validation material is available in its validation paper.

Do you need a Series 11 for hypertension alerts?

No. If you already own a compatible Series 9, Series 10, Ultra 2, or newer Apple Watch, buying a Series 11 is not necessary solely to access hypertension notifications. The capability is software-based and is listed for those older supported models as well.

Series 11 was announced on September 9, 2025, alongside the feature, which explains why headlines connect the two. But the regulatory decision concerns Apple’s notification software, not a new hardware capability that turns Series 11 into a cuffless blood-pressure meter.

For someone choosing between watches, the feature can be one health-related consideration, but it is a poor reason by itself to treat Series 11 as a medical purchase. Apple’s U.S. Store listed Series 11 from $399 for aluminum GPS models and from $699 for titanium models when the cited store page was reviewed; prices and configurations can change. Apple said it expected the feature to notify more than one million people with undiagnosed hypertension in its first year. That is an Apple projection, not an independently verified outcome.
