Apple released watchOS 5.1.2 on December 6, 2018, adding an ECG app for Apple Watch Series 4 in the United States and specified U.S. territories. The free update also brought irregular-rhythm notifications to Apple Watch Series 1 and later. The ECG app recorded and classified a single-lead-style rhythm; it was not a general heart-disease detector or a substitute for medical care.
What watchOS 5.1.2 added
The update was the third release in the watchOS 5 series, arriving about a month after watchOS 5.1.1. Its headline change was ECG functionality on Series 4, whose electrical heart sensor uses an electrode on the watch’s back crystal and another in the Digital Crown. Apple also introduced background irregular-rhythm notifications for supported older watches.
Other changes included new Infograph complications for Apple apps such as Mail, Messages, Home, Maps, News, Find My Friends, Phone and Remote, plus a Control Center switch for Walkie-Talkie availability. The release also included fixes and other improvements. MacRumors’ release coverage reported these secondary changes.
Which watches and regions were supported at launch?

At launch, Apple made the features available in the United States, Puerto Rico, Guam and the U.S. Virgin Islands. Availability depended on regional regulatory authorization, not just the watch hardware; changing a device’s region was not an officially supported way to enable ECG. The original ECG launch requirements were an Apple Watch Series 4, an iPhone 5s or later running iOS 12.1.1 or later, watchOS 5.1.2, and a supported U.S. location. Those are requirements for the December 2018 release, not a statement of current 2026 compatibility rules.
How the ECG recording worked
The ECG app made an approximately 30-second recording. For the circuit, the wearer’s finger touched the Digital Crown while the back of the watch rested against the wrist.

- Wear the Series 4 snugly, with the watch on the wrist selected during setup.
- Open the ECG app and rest the arm on a table or in the lap.
- Touch the Digital Crown with a finger from the opposite hand without pressing it.
- Keep still and maintain contact until the recording finishes, about 30 seconds.
- Review the classification, add any applicable symptoms, and save the result in the Health app.
- If useful, export or share the PDF recording with a clinician through Health.
Apple’s current ECG app support documentation explains the recording, results and sharing workflow. Its guidance describes newer software as well, so it should not be read as a complete list of the 2018 launch requirements.
If the result is inconclusive
A loose band, poor wrist contact, wet skin or watch, movement, electrical interference, or a heart rate outside the app’s classification range can affect a recording. A pacemaker or implantable cardioverter-defibrillator and other physiological factors may also make a result inconclusive. Sit still, ensure the watch is dry and snug, and try again away from nearby plugged-in electronics. An inconclusive result is not itself a diagnosis.

What the ECG classifications meant
- Sinus rhythm: The recording did not show signs of atrial fibrillation (AFib) during that recording. It does not establish overall heart health or show that rhythm is normal at all times.
- Atrial fibrillation: The recording showed a rhythm pattern consistent with AFib. Discuss an unexpected result with a healthcare professional, particularly if you have not previously been diagnosed.
- Inconclusive: The app could not classify that recording. This can happen for signal-quality or physiological reasons and does not rule a condition in or out.
The app was intended to classify a limited rhythm recording, not detect every arrhythmia or diagnose heart disease. Apple said it could not identify a heart attack, stroke, blood clot, high blood pressure, congestive heart failure, high cholesterol or every other heart condition. It was not intended for people under 22 at launch.
ECG and irregular-rhythm notifications were different tools
The ECG app let a user initiate a short electrical recording. Irregular-rhythm notifications instead checked periodically in the background with the optical heart sensor. Apple said it could notify a user when an irregular rhythm suggestive of AFib appeared across five checks over at least 65 minutes. That logic describes intermittent screening, not continuous clinical monitoring.

What Apple’s accuracy figures did—and did not—show
In a clinical validation study of approximately 600 participants, Apple reported 98.3% sensitivity for classifying AFib and 99.6% specificity for classifying sinus rhythm. These figures applied to classifiable recordings under study conditions, not every real-world attempt and not detection of all cardiac conditions. Some readings are inconclusive or unusable.
The FDA documentation describes the app’s intended use and limitations; its regulatory status should not be mistaken for a finding that the watch diagnoses or treats heart disease. See the FDA documentation for the ECG app alongside Apple’s description of its validation study.

How to install the 2018 update
For the December 2018 release, contemporary instructions used the paired iPhone’s Apple Watch app. The update path and prerequisites below are historical:
- Open the Apple Watch app on the paired iPhone.
- Tap General, then Software Update.
- Download and install watchOS 5.1.2 if it is offered.
- Keep the watch on its charger, within range of the iPhone, and at least 50% charged during installation.
Contemporary installation coverage gives these steps. Because watchOS 5.1.2 is an older release, these instructions do not establish what software an Apple Watch can install today.
When not to rely on a watch reading
A watch ECG is not equivalent to a clinical 12-lead ECG or a clinician-directed ambulatory monitor. Seek professional advice about an AFib classification or ongoing symptoms rather than treating an app result as a diagnosis. Chest pain, pressure, tightness or suspected heart-attack symptoms require emergency medical attention; do not wait for a watch reading. Apple’s safety and limitations guidance also cautions that a sinus-rhythm result applies only to the recording.
