watchOS 11.4 added an option called “Break Through Silent Mode” for a Sleep Wake Up alarm. Turn it on in the Alarms app to have the alarm’s sound play while you wear Apple Watch, even with Silent Mode enabled; it does not make every notification or alarm audible.
This is a software setting, not a reason to buy an accessory. Apple’s guide does not specify whether the option is on by default, so check the setting if you want sound from your wake-up alarm.
What changed in watchOS 11.4?
Apple’s update summary highlights three changes: an option to let the Sleep Wake Up alarm break through Silent Mode, support for controlling Matter-compatible robot vacuum cleaners in the Home app and adding them to scenes and automations, and a fix for an issue that could make face selection unresponsive when switching faces.
The alarm setting is specific to the Sleep Wake Up alarm associated with a sleep schedule. It is not a general change to all Apple Watch alarms, notifications, or alerts.

How to let a Sleep Wake Up alarm make sound
Apple calls the control Break Through Silent Mode. Its Apple Watch User Guide says turning it on makes sound always play while wearing Apple Watch. Find the setting in the Alarms app:
- Open the Alarms app on Apple Watch.
- Tap the wake-up alarm.
- Turn on Break Through Silent Mode.
Wake-up alarms are part of a sleep schedule. With Silent Mode on and the override off, an alarm can tap your wrist without sound; with the override on, sound plays while you wear the watch. Apple’s guide does not say whether the override is enabled by default.

watchOS 11.4 added support for controlling Matter-compatible robot vacuum cleaners in the Home app and including them in scenes and automations. This applies to supported Matter-compatible devices, not every robot vacuum.
Watch-face selection fix
Apple also listed a fix for an issue that could make face selection unresponsive when switching between faces.

How do I activate an alarm through Silent Mode?
Open the Alarms app on Apple Watch, tap the wake-up alarm, and turn on Break Through Silent Mode. Apple says sound always plays while wearing the watch when this setting is on.
