Apple Watch SOS: How to Set Up and Use Emergency Features

Learn how Apple Watch Emergency SOS works, how to configure Medical ID and emergency contacts, and what Fall Detection, Crash Detection and Ultra 3 satellite SOS require.

Apple Watch SOS is a group of safety tools, not a guarantee that help can always be reached. You can call emergency services manually, share Medical ID and location with configured contacts, and—on compatible models—use automatic Fall Detection or Crash Detection. Ordinary calling still needs a usable connection through the watch, a nearby iPhone, or Wi-Fi Calling. Apple’s watch-based Emergency SOS via satellite is a separate option limited to Apple Watch Ultra 3 in supported regions.

Before relying on any of these features, set up Medical ID and emergency contacts, enable Emergency SOS location sharing, and check the settings for your particular watch. Menus below reflect Apple’s watchOS 26 documentation; labels and placement may change in later software.

What Apple Watch SOS can do—and what it cannot

Apple Watch has several distinct emergency features:

  • Emergency SOS: You manually call the local emergency number, such as 911 in the United States.
  • Fall Detection: A compatible watch can alert you after detecting a hard fall and attempt to call for help if you remain immobile and do not respond.
  • Crash Detection: Supported watches can alert you after detecting a severe car crash and attempt an emergency call if you do not respond.
  • Medical ID and emergency contacts: Medical information can be displayed for responders, and configured contacts may receive a message and location updates after an emergency call.
  • Emergency SOS via satellite: Apple Watch Ultra 3 can send emergency text messages by satellite in supported locations when ordinary connections are unavailable.

None of these features guarantees a connection or a response. Standard emergency calls depend on a cellular network, Wi-Fi Calling, or a nearby iPhone that can connect. Satellite SOS has separate model, region, software, and line-of-sight requirements. Carrier-provided satellite services are also separate from Apple’s Emergency SOS via satellite.

Check your model and connection first

A GPS Apple Watch usually relies on its paired iPhone being nearby for connectivity or Wi-Fi Calling. A GPS + Cellular model can call without the iPhone when cellular service is available and the watch is set up with a compatible carrier. Cellular capability does not mean a watch can connect everywhere, and a cellular model is not the same as a satellite communicator.

Feature Compatibility and practical requirement
International emergency calling Apple lists support beginning with Apple Watch Series 5 and later, Apple Watch SE and later, and all Ultra models. Calling, location sharing, and contact messaging can vary abroad. Apple Watch Emergency SOS guidance.
Fall Detection Apple Watch SE or later, Series 4 or later, and Ultra or later; available only to users age 18 or older. Apple Fall Detection guidance.
Crash Detection Apple Watch Series 8 or later, Apple Watch SE (2nd generation) or later, and Apple Watch Ultra or later, with supported software. Apple Crash Detection guidance.
Watch-based satellite Emergency SOS Apple Watch Ultra 3 only, with compatible watchOS, in a supported region and with a clear view of the sky. Older Ultra, Series, and SE models do not have this Apple watch satellite feature. Apple satellite features guide.

Some emergency systems may ask callers to respond using a keypad; Apple notes that the watch may not be able to complete those prompts automatically. Keep your iPhone available when practical, particularly when using a GPS model.

Set up Medical ID and emergency contacts

  1. On the paired iPhone, open Health and open the Medical ID section.
  2. Add information that could help responders, such as your name, age, allergies, medical conditions, medications, blood type if known, and organ-donor status.
  3. Add emergency contacts. Choose people likely to answer and who understand your health or travel circumstances.
  4. Enable sharing of Medical ID information during an emergency call if the option is available in your region.
  5. Review the details after changes to medications, diagnoses, phone numbers, or relationships.

Medical ID information is available on the watch. From the emergency screen, hold the side button and select the Medical ID control. Apple says sharing Medical ID with emergency services is available only in certain regions, including the United States and Canada, and depends on the applicable emergency-data settings. Enable Emergency SOS location sharing

On the iPhone, open Settings → Privacy & Security → Location Services → System Services, then make sure Emergency Calls & SOS is enabled. Location sharing with emergency services and emergency contacts varies by country and region. Apple’s regional guidance is at Emergency SOS and Crash Detection.

Turn on Fall Detection and know what happens

Choose when Fall Detection is active

On the iPhone, open Watch → My Watch → Emergency SOS → Fall Detection. Choose Always on or Only on during workouts. You can also change it on the watch at Settings → SOS → Fall Detection. Fall Detection may turn on automatically for users 55 and older if the age is correctly recorded in Medical ID and the Health profile, but check the setting rather than relying on that behavior.

Keep Wrist Detection enabled

Automatic emergency calling after a fall requires Wrist Detection. On the watch, open Settings → Passcode and make sure Wrist Detection is on.

How a fall alert works

  1. After detecting a hard fall, the watch taps your wrist, sounds an alarm, and displays an alert.
  2. You can dismiss the alert or drag the emergency-call control to place a call.
  3. If the watch detects movement, it waits for you to respond. If you remain immobile for about one minute, a countdown begins.
  4. If the countdown ends without cancellation, the watch attempts to call emergency services.

After the call, the watch can notify emergency contacts and share your location. During a call, it may play a recorded message saying it detected a hard fall and giving latitude and longitude. Apple says Fall Detection cannot identify every fall, and high-impact activity can sometimes trigger an alert. It is not a medical diagnosis, and calling depends on a satellite, cellular, or Wi-Fi Calling connection through the watch or a nearby iPhone. Details: Apple Fall Detection guidance.

Check Crash Detection

Crash Detection is designed for severe car crashes, not every collision. Apple says it is on by default on supported devices, but verify its status in the Watch app’s emergency settings. If the watch detects a severe crash, it displays an alert and sounds an alarm so you can respond. If there is no response, it can initiate an emergency call after about 30 seconds. Medical ID and emergency contacts can be involved if configured, though location sharing and contact behavior vary by region. Apple says Crash Detection can work if you are unresponsive; do not deliberately stage a collision to test it. See Apple’s Emergency SOS and Crash Detection guidance.

Make a manual Emergency SOS call

Use the Emergency Call slider

  1. Press and hold the Apple Watch side button—the button below the Digital Crown—until the emergency sliders appear.
  2. Drag Emergency Call to the right.

Use the countdown

Keep holding the side button instead of releasing it when the sliders appear. The watch sounds a warning and starts a countdown; when it ends, the watch calls emergency services. The warning sounds even in Silent Mode. Use the side button, not the Digital Crown. The call goes to the local emergency number, such as 911 in the United States. If a call was accidental

If the call has connected, do not hang up immediately. Wait for a responder to answer and explain that it was accidental and you do not need assistance. Apple gives the same advice for an accidental fall-detection call.

What emergency contacts receive

Emergency contacts are selected through Medical ID. After an emergency call, Apple Watch can send them a message with your current location; while SOS mode is active, they may receive location updates for a period when your location changes. This depends on regional availability and functioning location services.

Important: Apple says contacts may not be automatically notified if the Messages app has been deleted or is not selected as the watch’s messaging app. Keep Messages available and check that it is selected if you rely on automatic contact notifications. This does not replace calling emergency services.

Use Apple Watch SOS when traveling

Your watch may call the local emergency number while abroad, but do not assume that the U.S. 911 experience carries over. The watch may not share your location or send a message to your emergency contacts, and support depends on the country, model, and connection. Some international emergency calls may work without an active watch cellular plan, but that is not universal. Use Emergency SOS via satellite on Apple Watch Ultra 3

Requirements and availability

As of August 18, 2026, Apple’s watch-based satellite emergency messaging requires Apple Watch Ultra 3, compatible software, and a supported country or region. You must be unable to use cellular or Wi-Fi and have an outdoor position with a clear view of the sky and horizon. Indoors, under heavy foliage, near buildings or other obstructions, or in wet conditions, a connection may fail. Apple lists availability in the United States, Canada, Mexico, the United Kingdom, Ireland, France, Germany, Italy, Spain, Portugal, Switzerland, Austria, Belgium, Luxembourg, the Netherlands, Australia, New Zealand, Japan, and other European markets. Availability can change with regulation and market approvals; check Apple’s current country and software requirements. Mexico requires watchOS 26.0.1 or later; Andorra and Iceland require watchOS 26.4 or later.

Send an emergency message

  1. Try calling the local emergency number first.
  2. If the call does not connect, tap Emergency Text via Satellite.
  3. Tap Report Emergency and answer the onscreen questionnaire.
  4. Follow the watch’s positioning instructions to connect to a satellite.
  5. Choose whether to notify emergency contacts, then continue the session until responders confirm communication is complete.

The watch can send Medical ID, configured emergency-contact information, questionnaire answers, location including elevation, and remaining battery level. Apple provides Emergency SOS via satellite for two years after Ultra 3 activation. Messages and location sharing via satellite require an active carrier plan, so not every satellite feature is included in the emergency service period. If a satellite SOS session starts accidentally, do not simply disconnect. Exchange messages with the responder and explain that help is no longer needed.

Other useful safety tools

Apple Watch Ultra models include a built-in siren that can attract nearby attention; it is not Emergency SOS and does not contact dispatch. Check In, Compass Backtrack, Find My, location sharing, and Emergency Call waypoints can support a trip or help people find one another, but they are not emergency-dispatch services. If the watch is unavailable, use iPhone Emergency SOS if you have a compatible iPhone and a working connection.

Troubleshoot common SOS problems

Problem Likely reason What to check
SOS will not place a call No usable cellular or Wi-Fi connection; a GPS model may be away from its iPhone. Move near the paired iPhone, use available cellular service, or connect through Wi-Fi Calling if supported. A call cannot be guaranteed without a connection.
Fall did not trigger an automatic call Wrist Detection may be off, the wearer may have moved or canceled, or the fall may not have been detected. Enable Wrist Detection and review Fall Detection settings. The feature does not detect every fall.
Emergency contacts were not notified Messages may be deleted or not selected as the watch’s messaging app; regional or location settings may also affect sharing. Keep Messages installed and selected, and verify Emergency Calls & SOS location services.
Medical ID is incorrect Health information or contacts are outdated. Update Medical ID in the iPhone Health app.
Satellite option is missing The watch may not be Ultra 3, the location or software may be unsupported, or ordinary connectivity may still be available. Confirm the exact model, software, location, and connection status using Apple’s availability guidance.
Satellite connection fails Buildings, foliage, terrain, indoor use, wet conditions, or poor sky visibility can obstruct the connection. Move to an open outdoor area and follow the watch’s positioning prompts.
An accidental call connected The SOS slider or countdown initiated a call. Stay on the line and tell the responder it was accidental.

Which Apple Watch makes sense for emergency use?

Choose based on the connection you need, rather than assuming one model can solve every coverage problem.

  • GPS model: A sensible fit if you normally carry your iPhone and need its connection for calls when the watch is away from cellular service. It is not a fully independent emergency communicator.
  • GPS + Cellular: More useful if you run, travel, provide care, or otherwise leave your iPhone behind. Independent calling requires compatible carrier service and coverage, and may involve a recurring plan.
  • Apple Watch Ultra 3: The relevant choice if you specifically need Apple’s watch-based satellite emergency messaging and are willing to meet its region, software, and sky-visibility requirements. Older Ultra models do not provide this feature.
  • Dedicated satellite communicator: Consider separate expedition-oriented hardware if you need extended off-grid messaging, tracking, or a different battery and service model. It is not equivalent to Apple Watch SOS.

For an older adult who needs dedicated monitoring, a help button, or caregiver escalation, a personal emergency response service may be more appropriate than treating any smartwatch as a replacement.

Pre-trip emergency setup checklist

  • Charge the watch and check battery level before leaving.
  • Update watchOS and the paired iPhone’s software.
  • Review Medical ID and confirm emergency contacts’ phone numbers.
  • Enable Emergency Calls & SOS under Location Services.
  • Verify Fall Detection and Wrist Detection settings.
  • If using GPS + Cellular, confirm carrier activation, coverage, roaming, and plan eligibility with your carrier.
  • For Ultra 3, confirm satellite availability for your destination and software version; try the satellite demo and learn the positioning prompts before going off-grid.
  • Tell emergency contacts what an SOS message or location update may look like.
