Apple Watch’s High Blood Pressure Detection Feature Is Now Available—Here’s What It Really Does

Apple’s Hypertension Notifications feature is now available in the United States with watchOS 26. It can alert eligible Apple Watch users when background heart-sensor data shows a pattern suggestive of hypertension—but it does not measure blood pressure, diagnose high blood pressure, or replace a cuff.

Apple Watch can now flag possible hypertension—but it does not measure your blood pressure

Apple’s Hypertension Notifications feature is now available in the United States with watchOS 26, following FDA marketing authorization. On supported Apple Watch models, the feature analyzes heart-sensor data in the background and can notify eligible users when its algorithm detects patterns suggestive of chronic high blood pressure.

That wording matters. The Apple Watch is not taking a blood-pressure reading, diagnosing hypertension, or replacing an upper-arm cuff. A notification is a prompt to take conventional blood-pressure measurements for seven days and discuss the results with a healthcare professional.

Apple says the feature is available on Apple Watch Series 9 and later and Apple Watch Ultra 2 and later, paired with an iPhone 11 or later. Regional availability can differ, so users should check Apple’s current support documentation rather than assume the feature works in every country or territory.

In practical terms, buying an Apple Watch does not automatically provide high-blood-pressure notifications. An older model, an Apple Watch SE, an unsupported iPhone, disabled Wrist Detection, or an unsupported region can prevent access.

If you are shopping specifically for this feature, look for Apple Watch Series 9 or later or an Apple Watch Ultra 2 or later. These models provide access to the notification feature; they do not provide cuff-style blood-pressure readings. Confirm the exact model, software requirements, and regional availability before purchasing because Apple’s current lineup and retailer inventory change over time.

How the feature works

Hypertension Notifications uses the Apple Watch’s optical heart sensor. That sensor collects photoplethysmography, or PPG, data—the same general type of optical signal used for several heart-related measurements on a smartwatch.

The watch-side software evaluates qualifying PPG signals and assigns risk scores. Software on the paired iPhone then aggregates those scores and looks for a pattern associated with hypertension. Apple describes this as passive, background analysis rather than a deliberate blood-pressure measurement.

The evaluation takes place over a 30-day period. The watch therefore does not produce an immediate result when you tap a button, and it does not display a systolic or diastolic number. If the accumulated pattern meets the notification criteria, the user receives an alert that they may have hypertension.

This is best understood as a screening or notification system:

  • It can: identify a pattern that may justify conventional blood-pressure testing.
  • It cannot: measure your blood pressure like a cuff.
  • It cannot: establish a hypertension diagnosis by itself.
  • It cannot: reliably rule out hypertension when no alert appears.

Apple reports that the algorithm was trained using studies totaling more than 100,000 participants and validated in a clinical study involving more than 2,000 participants. Those are Apple’s reported development and validation figures, not independent testing by this article. Algorithm validation also does not turn a notification into a clinical diagnosis.

The FDA’s 510(k) record describes the product as Class II cardiovascular machine-learning-based notification software. Its intended use is to notify adults who may have hypertension and prompt further assessment—not to act as a blood-pressure surveillance device.

It is not intended for use during pregnancy. It is also not intended to monitor whether an existing hypertension treatment is working. Someone already diagnosed with high blood pressure should follow the monitoring plan provided by their clinician rather than treating the watch feature as a replacement.

Apple and the FDA also warn that the system will not identify every case of hypertension. No notification can mean that the algorithm did not find the relevant pattern, or that insufficient data was collected; it does not prove that blood pressure is normal.

What to do if Apple Watch sends a hypertension notification

  1. Do not self-diagnose. The notification means that the watch detected a pattern suggestive of possible hypertension. It is not a diagnosis and is not a reason to change medication on your own.
  2. Use an appropriate upper-arm cuff. Apple directs users to take blood-pressure readings with a third-party blood-pressure monitor.
  3. Record readings for seven days. Apple’s guidance describes taking measurements in the morning and evening and logging the results.
  4. Discuss the readings with a healthcare professional. A clinician can assess the measurements alongside your medical history, medications, symptoms, and other risk factors.

The most useful purchase after an alert is an FDA-authorized upper-arm blood-pressure monitor with a cuff that fits your arm. The cuff—not the Apple Watch—is what supplies the blood-pressure measurement for the follow-up process. Before buying, check the monitor’s current authorization, independent accuracy validation, cuff-size range, and whether its app can reliably share readings with Apple Health if that matters to you.

The FDA specifically warns consumers not to rely on unauthorized wearable or other devices that claim to measure or estimate blood pressure. Inaccurate readings can delay care, cause unnecessary concern, or contribute to inappropriate medication changes. Its blood-pressure device safety communication is worth reading before choosing a monitor.

A possible hypertension notification is generally a follow-up issue, not a substitute for urgent medical assessment. If you have severe or sudden symptoms, seek urgent or emergency care instead of waiting for a seven-day log or relying on the watch.

What the FDA clearance does—and does not—mean

FDA marketing authorization is important because it covers this particular notification feature for its stated intended use. It does not mean that every smartwatch’s blood-pressure claim is reliable, and it does not mean that Apple Watch has become a cuffless blood-pressure monitor.

The cleared use is narrower: analyze multiple days of Apple Watch PPG data, identify patterns suggestive of hypertension, and notify an eligible adult so that conventional blood-pressure follow-up can take place. The clearance specifically retains the limitations around prior hypertension diagnosis, pregnancy, treatment monitoring, and traditional diagnosis.

So these statements are accurate:

  • “Apple Watch can notify eligible users when its algorithm detects patterns suggestive of hypertension.”
  • “An Apple Watch hypertension notification should be followed by cuff measurements and clinical review.”

These statements are not accurate:

  • “Apple Watch measures your blood pressure.”
  • “Apple Watch diagnoses high blood pressure.”
  • “A normal Apple Watch result proves you do not have hypertension.”

How to think about the feature before buying an Apple Watch

Hypertension Notifications may be useful if you want passive screening and meet the eligibility requirements. It is less useful as a buying reason if you already have diagnosed hypertension, are pregnant, need regular treatment monitoring, or expect a watch to replace a validated cuff.

Use this checklist before spending money:

  • Is your country or region supported?
  • Is the watch a Series 9-or-later model or Ultra 2-or-later model?
  • Do you have an iPhone 11 or later?
  • Are iOS and watchOS updated to the latest compatible versions?
  • Is Wrist Detection enabled?
  • Are you at least 22 and not previously diagnosed with hypertension?
  • Do you understand that the feature needs up to a 30-day evaluation period and does not show a blood-pressure number?
  • If alerted, do you have access to a properly fitting, authorized upper-arm cuff and a healthcare professional?

The watch can add a useful layer of passive screening, but it is only one part of a responsible blood-pressure assessment. The confirmatory measurement still comes from a suitable cuff, and the interpretation still belongs with a qualified healthcare professional.
