Best blood pressure watches: top models and how they work

Blood-pressure watches use different methods: an inflatable cuff, calibrated optical estimates, or hypertension notifications without BP readings. Compare the key options and learn why a validated upper-arm cuff remains important.

A watch that displays a blood-pressure number is not necessarily measuring your blood pressure. Some wearables use a miniature inflatable cuff, some estimate pressure from optical signals after calibration, and others look for patterns associated with possible hypertension without displaying readings.

For a direct wrist measurement, the Omron HeartGuide uses an inflatable cuff. For cuffless routine readings, the Hilo Band/Aktiia G0 is a U.S.-cleared option with a separate cuff and daily calibration; Samsung offers calibrated estimates for compatible Galaxy owners, while Apple Watch provides hypertension notifications rather than blood-pressure readings.

This guide is based on regulatory records, manufacturer instructions and published validation information—not personal hands-on testing. No smartwatch should replace a validated upper-arm monitor or clinical advice. The American Heart Association (AHA) says cuffless devices still need robust accuracy and outcome evidence before their readings can be fully interpreted.

Important: “FDA-cleared” is the accurate regulatory wording for a 510(k) device. Clearance does not mean a watch is suitable for every person, condition or medical decision. The FDA warns against unauthorized smartwatch and smart-ring blood-pressure features.

1. Omron HeartGuide: best watch for a direct blood-pressure reading

The Omron HeartGuide BP8000-M and BP8000-L are the closest match to what most people mean by a blood-pressure watch. The band contains an inflatable cuff, so the device measures pressure at the wrist rather than inferring it solely from an optical sensor. Omron lists the models in its product support materials.

That design is also why it is thicker and less conventional than a normal fitness or smartwatch. It is a spot-measurement device, not a watch that continuously inflates its band throughout the day.

What to know before buying

  • The currently listed models are BP8000-M and BP8000-L.
  • Measurements transfer automatically to the companion smart-device app.
  • The HeartGuide display does not provide a history of readings; historical data must be viewed in the app.
  • The watch should be within 16 feet (5 meters) of the smart device for transfer, according to the HeartGuide instruction manual.
  • The cuff mechanism makes it substantially bulkier than an ordinary smartwatch.

Choose HeartGuide if seeing a measured blood-pressure result on a wrist-worn device matters more than having a slim case or a full smartwatch experience. It is a poor choice if you want unobtrusive, all-day tracking or a large app ecosystem.

2. Hilo Band/Aktiia G0: best cuffless wearable with current U.S. clearance

The Hilo Band is the consumer name associated with Aktiia’s G0 Blood Pressure Monitoring System. The FDA cleared system 510(k) K250415 on July 2, 2025. It is a significant option because routine readings can be taken without inflating a cuff around the wrist.

It is not, however, a completely cuff-free system. The cleared product consists of the wrist-worn monitor and an oscillometric cuff. The band uses pulse-wave information for routine readings, but the system must be calibrated with that cuff.

The limits are part of the product

The FDA indication covers adults aged 22 to 59, with a wrist circumference of 14 to 21 cm, using the system at home. Calibration is required every 24 hours. Those conditions make Hilo more specialized than a general-purpose smartwatch.

The setup includes the Hilo Band, Hilo App and Hilo Cuff:

  1. Purchase or subscribe to the U.S. kit.
  2. Pair the Band and cuff in the Hilo app.
  3. Complete the cuff-based calibration.
  4. Wear the band as instructed and keep up with the daily calibration requirement.

Hilo is a more relevant choice than an unverified marketplace watch when you want cuffless readings, but do not mistake “cuffless during routine readings” for “independent of a cuff.” Its cleared use is for spot-checking within the specified population and conditions, not unrestricted diagnosis or treatment management.

3. Samsung Galaxy Watch4 and later: best BP estimator for compatible Android users

Samsung’s Blood Pressure feature estimates systolic and diastolic pressure using optical sensor data. Before using it, you calibrate the watch against a separate upper-arm cuff. Samsung’s U.S. rollout began on March 31, 2026, for Galaxy Watch4 and later released models, excluding Galaxy Fit devices, according to Samsung’s announcement.

The feature requires:

  • A Galaxy Watch4 or newer supported watch
  • Wear OS 4.0 or later
  • An Android 12 or later phone
  • A compatible Samsung Galaxy smartphone
  • A separate upper-arm blood-pressure cuff for calibration

How to calibrate a Galaxy Watch

  1. Open Samsung Health Monitor on the phone.
  2. Tap Blood pressure at the bottom of the screen.
  3. Tap Calibrate the watch.
  4. Take three cuff measurements within 30 minutes and enter the results as prompted.

Samsung requires recalibration every 28 days. To start it manually, use Samsung Health Monitor → Blood pressure → Recalibrate → Recalibrate the watch. Menu wording can vary slightly with software version and region. See Samsung’s U.S. instructions.

How to take a reading

  1. Open Samsung Health Monitor on the watch.
  2. Tap Blood pressure.
  3. Tap Measure.
  4. Read the positioning prompt and tap OK.
  5. When the measurement finishes, review it and tap Done.

Samsung lists a measurement range of 70–180 mmHg systolic and 40–120 mmHg diastolic, and a calibration range of 80–170 systolic and 50–110 diastolic (Samsung Health Monitor).

Samsung describes the feature as a wellness tool, not a diagnostic or treatment device. Sensor cleanliness, sweat, lotion, skin characteristics, movement, watch position and inconsistent fit can affect the optical signal. Wear it on the same wrist, with a snug—not painfully tight—fit, and use consistent cuff technique during recalibration.

4. Apple Watch Series 9 and later: best for hypertension notifications

Apple takes a different approach. Hypertension Notifications do not measure blood pressure and do not display systolic or diastolic values. Instead, the watch analyzes optical heart-sensor data over a 30-day evaluation period and looks for patterns associated with possible chronic hypertension, as described in Apple’s validation paper.

The feature is available on:

  • Apple Watch Series 9 or later
  • Apple Watch Ultra 2 or later
  • iPhone 11 or later with current software

It is not available on Apple Watch SE. Apple says it is intended for people aged 22 or older who have not already been diagnosed with hypertension, and it is not intended for use during pregnancy (Apple Support).

How to enable Hypertension Notifications

  1. Open the Health app on the iPhone.
  2. Tap your picture or initials in the upper-right corner.
  3. Tap Health Checklist.
  4. Under Hypertension Notifications, tap Set Up.
  5. Follow the instructions and tap Done.

If Apple sends a notification, it prompts you to use a third-party cuff and record blood pressure twice a day for seven days. To begin the log, open Health → Search → Heart → Blood Pressure → Blood Pressure → Get Started.

To enter an individual result manually, go to Health → Search → Heart → Blood Pressure → Blood Pressure → Add a Measurement, enter the systolic and diastolic values, and tap Done.

Apple Watch is a good choice for Apple users who want an additional warning signal, not for anyone shopping for a watch that directly measures blood pressure.

What about Garmin, Fitbit, Pixel Watch and cheap marketplace watches?

Do not treat every number labelled “BP” as a medical measurement. A generic smartwatch or ring may show a plausible-looking result without a named regulatory authorization, a validated measurement method or useful calibration instructions.

The FDA safety communication warns consumers not to use unauthorized smartwatch and smart-ring features that claim to measure blood pressure. An inaccurate result can lead to false reassurance, unnecessary alarm or inappropriate medication changes.

That does not mean every cuffless technology is automatically inaccurate. It means the exact device, feature, intended use and evidence matter. Garmin, Fitbit, Pixel Watch and ordinary Apple Watch features should not be treated as interchangeable with a validated blood-pressure monitor simply because an app displays a number.

The upper-arm cuff you should still own

The safest default for home monitoring remains a validated automatic, upper-arm cuff-style monitor. The AHA recommends an upper-arm cuff; it considers wrist and finger monitors less reliable and recommends selecting a cuff that fits your upper-arm circumference.

Use it correctly:

  1. Avoid smoking, caffeine and exercise for 30 minutes beforehand.
  2. Sit quietly for five minutes.
  3. Keep your back supported and both feet flat on the floor.
  4. Rest your arm at heart level.
  5. Take two readings one minute apart and record them.

Take the monitor to a medical appointment so its result and your technique can be compared with office equipment. The AHA suggests doing this at least annually or as the manufacturer directs.

Which blood-pressure watch should you buy?

FAQ

Is there a smartwatch that really measures blood pressure?

Yes, but the methods differ. Omron HeartGuide uses an inflatable cuff built into the band. Hilo/Aktiia G0 uses cuffless routine readings after calibration with a separate cuff. Samsung estimates BP after upper-arm-cuff calibration. Apple Watch does not measure or display blood pressure.

Is the Omron HeartGuide FDA approved?

The more accurate regulatory language is to discuss a device’s specific clearance or authorization rather than calling it broadly “FDA approved.” HeartGuide is a wrist-worn monitor with an inflatable cuff; check the current manufacturer and FDA records for the exact model and intended use.

Can a Samsung Galaxy Watch replace a blood-pressure cuff?

No. Samsung’s feature is a calibrated wellness estimate, not a replacement for a validated upper-arm cuff or medical care. It requires a compatible Galaxy phone and recalibration every 28 days.

Does Apple Watch show blood-pressure numbers?

No. Hypertension Notifications analyze heart-sensor data for patterns associated with possible hypertension. If notified, Apple directs the user to take readings with a third-party cuff and log them in the Health app.

The Bottom Line

Bottom line: Buy the Omron HeartGuide if you want the most literal blood-pressure watch, accepting its bulk and spot-reading design. Choose the Hilo Band/Aktiia G0 for a current U.S.-cleared cuffless wearable within its specific age, wrist-size and daily-calibration limits. Galaxy Watch owners can use Samsung’s calibrated estimates for trends, while Apple Watch owners get hypertension screening notifications—not BP readings. For decisions about diagnosis or medication, keep a validated upper-arm cuff in the picture.
