Yes—but only certain Dexcom systems can do it. Dexcom G7 and Dexcom G7 15 Day can send glucose readings directly from the sensor to a compatible Apple Watch over Bluetooth. With Dexcom’s Direct to Watch feature enabled, you can view the current glucose value, trend arrow, graph and alerts without keeping your iPhone nearby.
This is a current feature rather than a brand-new 2026 launch: Dexcom announced U.S. availability in 2024. An iPhone is still required for initial setup and pairing new sensors, and compatibility varies by country, app version and device combination.

Standard Apple Watch display
In the traditional arrangement, the Dexcom sensor sends data to the iPhone, and the watch receives information through the phone. The iPhone generally needs to remain within Bluetooth range. Leave it behind, and the watch may stop receiving current readings and alerts.
Dexcom describes this arrangement in its Apple Watch FAQ.
Direct to Watch
Direct to Watch creates a Bluetooth connection from the G7 sensor to the Apple Watch itself. That is why you can go for a run, move around the house or leave your phone in a bag while continuing to see glucose information on your wrist.
The iPhone does not disappear from the system. It remains necessary for initial configuration, pairing a new sensor and managing parts of the setup. The important distinction is that it no longer has to sit in the middle of every sensor-to-watch reading.
What you need
For the current U.S. configuration documented by Dexcom, you need:
- A Dexcom G7 or Dexcom G7 15 Day sensor system.
- An Apple Watch Series 6 or later.
- watchOS 10 or later.
- An iPhone running iOS 17 or later.
- Dexcom G7 app version 2.2.1 or later, according to Dexcom’s Direct to Watch FAQ.
- A supported regional app and device configuration.
Dexcom G6 does not support Direct to Watch. Nor should Android-phone users assume that an Apple Watch feature applies to their setup: an Apple Watch requires an iPhone pairing arrangement, and Dexcom’s current documentation presents Direct to Watch as an Apple Watch-specific capability.
Software support can change as Apple and Dexcom update their operating systems and apps. Confirm the exact combination at Dexcom’s compatibility page before purchasing a watch.
Does the Apple Watch need cellular?
No. The sensor-to-watch connection uses Bluetooth, and Dexcom says a cellular plan is not required for Direct to Watch.
These connections serve different purposes:
- Bluetooth: carries readings from the sensor to the watch.
- Wi-Fi or internet: supports cloud-dependent services and remote sharing.
- Cellular: can give the watch internet access away from Wi-Fi and the iPhone, but is optional for Direct to Watch itself.
A non-cellular Apple Watch can therefore display local readings directly from a compatible G7 sensor. It does not automatically provide uninterrupted remote monitoring for caregivers when the iPhone is absent.
How to set up Dexcom Direct to Watch
- Install or update the Dexcom G7 app on the iPhone.
- Pair and start the G7 sensor using the iPhone.
- Pair the Apple Watch with the iPhone and make sure it meets Dexcom’s compatibility requirements.
- Open the Dexcom G7 app on the iPhone.
- Go to Connections → Direct to Watch.
- Follow the in-app instructions to connect the sensor and watch.
- If the watch app does not install automatically, open Apple’s Watch app on the iPhone.
- Tap My Watch → Available Apps.
- Find Dexcom G7 and tap Install.
- Open the watch app and confirm that readings and alerts appear.
- Optionally add the Dexcom complication to a supported watch face.
Dexcom’s setup instructions are available in its Direct to Watch connection FAQ.
What appears on the watch?
Depending on the supported configuration, the watch can show:

The watch is still a display and alert device, not a complete replacement for the Dexcom iPhone app. Settings are configured through the phone app, and the phone is part of starting new sensors and maintaining the broader system.
What still requires an iPhone or internet?
Setup and new sensors
You still need a compatible iPhone to complete setup and pair a new sensor. Direct to Watch means “no iPhone nearby for ongoing display,” not “no iPhone ever.”
Dexcom Share and Follow
Seeing your glucose locally on the watch is different from allowing a caregiver to follow it remotely. Dexcom Share and Follow depend on an internet connection. Dexcom’s documentation also says standard Share/Follow use generally requires the phone to remain within about 20 feet (6 meters), while newer watch-based sharing requires the compatible Apple Watch to connect to the internet over Wi-Fi.
The watch alone cannot provide every Share or Follow function when the iPhone is absent. If continuous remote monitoring is essential, test the exact phone, watch, Wi-Fi and sharing arrangement rather than assuming that local Direct to Watch readings guarantee caregiver access.
Third-party health apps
Direct to Watch cannot send real-time readings directly to certain third-party Health app partners, including examples Dexcom gives such as Sugarmate and Happy Bob. Data may be backfilled to partner apps after the iPhone reconnects, but that is not the same as a live direct feed.
What does the orange phone icon mean?
An orange phone icon beside the reading indicates that Direct to Watch is temporarily not working. The watch may be relying on the phone rather than receiving data directly from the sensor.
Try these steps:
- Confirm Bluetooth is enabled on the watch and iPhone.
- Keep the watch on your wrist and within normal range of the sensor.
- Bring the iPhone nearby.
- Open the Dexcom G7 app on the iPhone and the Dexcom app on the watch.
- Check that the watch is still paired with the iPhone.
- Install any pending Dexcom, iOS or watchOS updates that are compatible with your system.
- Restart the iPhone and Apple Watch if the connection remains stuck.
If the problem continues, follow Dexcom’s support process. A restart cannot repair every sensor or hardware failure, and a defective sensor may require replacement through Dexcom.
Real-time does not mean instantaneous
Dexcom describes G7 readings as automatically sent to a compatible display device every five minutes and presents Direct to Watch as a real-time glucose experience. In practical use, however, the displayed value can be delayed, briefly stale or unavailable.
Quick wins for a faster PC:
Clear out junk files and repair common Windows errorsFree Scan →Fix the driver behind crashes, sound loss and screen glitchesFind Drivers →There are several reasons: glucose in interstitial fluid does not change at exactly the same moment as blood glucose; Bluetooth can be interrupted; watch wake behavior and software refresh can affect what appears on screen; and a dead battery or lost connection can stop updates.
Dexcom measures glucose in interstitial fluid and displays a calculated glucose value—it is not measuring blood directly. If a reading does not match your symptoms or expectations, follow Dexcom’s instructions to use a blood-glucose meter when making treatment decisions, and follow the advice of your clinician.

Swimming and bathing
Dexcom warns that readings may not be available on the watch while swimming or bathing when Direct to Watch is active. Treat this as a connection limitation, not as evidence that the sensor is unsafe around water. Follow the product’s water-use instructions separately.

Battery and connection management
Direct to Watch removes the need to carry the iPhone nearby, but it adds another device whose battery and connection status matter. A dead watch, disabled Bluetooth connection or interrupted sensor link can remove the benefit. Check the watch’s connection indicator rather than assuming that an installed app means the sensor is currently connected.

Regional availability
Dexcom app features, compatible devices and partner integrations can differ by country. Requirements listed above reflect Dexcom’s current U.S. documentation and should not be treated as a worldwide guarantee.
Is it worth buying an Apple Watch for Dexcom?
Direct to Watch makes the most sense if you already use Dexcom G7 or G7 15 Day, already own a compatible iPhone and want phone-free access during exercise, errands or everyday movement. It can also be useful for discreet alerts and quick checks.
It is a weaker buying reason if you use G6, Android, an incompatible watch or a treatment setup whose pump and automated insulin-delivery compatibility has not been confirmed. It is also a poor fit if your primary requirement is dependable caregiver monitoring without the wearer’s phone or internet path.
If you only want Dexcom readings on your wrist while carrying your iPhone, the standard iPhone-linked watch display may be enough; buying a new Apple Watch solely for Direct to Watch may not provide enough additional value.

Remember that the watch is an additional hardware purchase, while Dexcom costs vary substantially by country, insurance, pharmacy and cash-pay status. In the U.S., Dexcom’s cost and coverage page advertises benefits checks and savings programs, including claims that many covered patients pay $20 or less per month. Those are not guaranteed prices for every patient.
Alternatives
Keep using Dexcom through the iPhone
This is the simplest choice if you already have a compatible Apple Watch and do not need to leave the phone behind. It avoids buying new hardware, but uninterrupted watch readings generally depend on the iPhone staying nearby.
Use a Dexcom receiver
Dexcom’s G7 information describes a receiver as an optional display device. It may suit someone who cannot or does not want to depend on an iPhone, but it does not provide the same wrist-based convenience and should be checked against the user’s other devices and treatment configuration.
Consider FreeStyle Libre 3 Plus
Abbott describes FreeStyle Libre 3 Plus as a sensor that automatically sends readings to a smartphone and can be worn for up to 15 days. Abbott’s materials also describe Apple Watch visibility. However, that should not automatically be treated as the same sensor-to-watch Bluetooth architecture Dexcom documents for Direct to Watch. Confirm the exact workflow for your country, phone and watch before switching systems.
Bottom line
Dexcom G7 and G7 15 Day can display glucose readings directly on a compatible Apple Watch without the iPhone remaining nearby. The feature does not support Dexcom G6, does not eliminate the need for an iPhone during setup, and does not guarantee uninterrupted remote sharing or third-party app access. Check compatibility first, watch for the orange phone icon, and treat the Apple Watch as a convenient display—not a substitute for the full Dexcom system or clinical guidance.
