How Apple Watch’s Hypertension Notifications Actually Work—and What They Can’t Tell You

Apple Watch Hypertension Notifications look for patterns in optical heart-sensor data but do not measure blood pressure or diagnose hypertension. Learn what an alert means, what to do next, and why no alert does not rule out high blood pressure.

Apple Watch’s Hypertension Notifications look for patterns in optical heart-sensor data that may suggest hypertension; they do not measure or display blood pressure. An alert is a prompt to take cuff readings for seven days and discuss them with a healthcare professional, while no alert does not rule out hypertension.

The feature runs passively after setup, analyzing qualified data over recurring 30-day periods. It is a screening prompt—not a diagnosis, treatment-monitoring tool, or substitute for routine blood-pressure checks.

How Apple Watch Hypertension Notifications work

The FDA submission describes software that analyzes photoplethysmography (PPG) data collected opportunistically by the watch. A machine-learning model scores qualified signals for hypertension risk, and iPhone software aggregates those scores to look for patterns associated with possible hypertension before generating a notification.

PPG uses the optical heart sensor to detect changes associated with blood flow beneath the skin. The feature recognizes patterns in those signals; it does not use a cuff or calculate a systolic or diastolic reading in mmHg. Apple’s U.S. Instructions for Use state that the feature does not provide blood-pressure measurements.

It runs in the background after setup. Apple says the feature evaluates recurring 30-day windows and requires at least 14 days of data collected while the user is awake within a window. If it has insufficient usable data, it will not issue a notification. Wear the watch consistently during the day to give it a chance to collect data.

What the validation study found

Apple’s prospective validation study enrolled 2,229 adults without a previous hypertension diagnosis. Participants wore an Apple Watch for 30 days and used an FDA-cleared home blood-pressure monitor as the reference. Of those enrolled, 1,863 provided at least 15 days of usable data for the primary endpoint analysis, according to Apple’s U.S. Instructions for Use.

Source: Apple’s 2025 U.S. Instructions for Use. Sensitivity and specificity describe study performance, not an individual’s personal probability of having hypertension. The results do not make an alert a diagnosis, and a no-alert result cannot rule out hypertension.

What an alert means—and what to do

An alert means the feature identified patterns associated with possible hypertension in recent heart-sensor data. It does not mean the watch has measured your blood pressure or diagnosed you. Apple recommends using a third-party cuff for a seven-day log and discussing the notification and readings with a healthcare professional. The U.S. instructions specify an FDA-cleared blood-pressure monitor.

  1. Use an automatic upper-arm blood-pressure monitor. Follow the monitor’s instructions and make sure its cuff fits properly.
  2. Take and log readings for seven days. Apple’s user guide describes logging readings in the morning and evening.
  3. Share the log and notification with a healthcare professional. They can advise you on what the readings mean and whether further evaluation is appropriate.

Do not start, stop, or change medication based solely on a watch notification or home readings. Follow your healthcare professional’s advice.

Why no alert does not mean normal blood pressure

The FDA submission says, “The absence of a notification does not indicate the absence of hypertension.” The feature cannot identify every case, and its validation sensitivity was 41.2%. Continue routine blood-pressure screening with a validated cuff or a healthcare professional, whether or not the watch sends an alert.

Who can use the feature?

Apple lists Apple Watch Series 9 or later, or Apple Watch Ultra 2 or later; Apple Watch SE is excluded. The paired iPhone must be an iPhone 11 or later, both devices need current compatible software, and Wrist Detection must be on. The feature is intended for adults aged 22 or older who have not been diagnosed with hypertension, and is not intended for use during pregnancy. Availability varies by region, and Apple notes that some newer models may still be awaiting regulatory clearance in a region. Check Apple’s watchOS feature-availability list for your country and model.

Apple says performance has not been tested in users with paced rhythms or irregular rhythms such as atrial fibrillation, which may affect performance. The feature is not intended to diagnose, treat, manage, or monitor treatment for hypertension. Apple also says the watch cannot detect a heart attack; do not use this feature for emergency monitoring.

How to turn on Hypertension Notifications

  1. Open the Health app on the paired iPhone.
  2. Tap your profile picture or initials.
  3. Open Health Checklist.
  4. Choose Hypertension Notifications and follow the on-screen prompts.
  5. Keep Wrist Detection enabled and wear the watch consistently during the day.

Apple’s Apple Watch User Guide documents the setup path. The option’s availability depends on eligibility, compatible devices and software, and regional availability.
