The Apple Watch can be useful for spotting broad blood-oxygen trends when you are still and the watch fits well, but an individual reading can be several percentage points off. It is a wellness feature, not a substitute for a fingertip pulse oximeter, a clinician’s monitoring plan, or medical evaluation. A normal result does not rule out a problem, and a low result should not be interpreted on its own.
What the Apple Watch measures
The Blood Oxygen feature estimates peripheral oxygen saturation, or SpO₂: the percentage of hemoglobin in the blood carrying oxygen. Optical sensors on the back of the watch shine light into the wrist and analyze the light that returns. It does not take a blood sample or directly measure arterial oxygen saturation (SaO₂), which can be assessed through an arterial blood-gas test.
Any pulse oximeter provides an estimate. The practical question is how closely and consistently that estimate matches a reference under the conditions in which it is used. Apple says the Blood Oxygen feature is intended for general fitness and wellness, not medical use, diagnosis, or consultation with a doctor (Apple Support).
What accuracy studies show
The largest summary in the available evidence is a 2026 living systematic review and meta-analysis of 82 Apple Watch validation studies across health metrics. For blood oxygen saturation, the pooled mean bias was −0.04 percentage points, with a 95% confidence interval from −0.42 to +0.35. That near-zero average can sound reassuring, but it does not describe how close every individual reading is.

The same analysis reported limits of agreement from −4.00 to +3.94 percentage points. In plain language, individual results in the studies could differ from the reference by several points in either direction. If an illustrative reference value were 95%, a watch result could be several points higher or lower; that is an explanation of the reported agreement range, not a prediction for a particular wearer or reading. Other studies help explain why no single accuracy label fits every situation:
- A 2023 systematic review found no strong systematic bias for Apple Watch Series 6 compared with conventional medical-grade pulse oximeters, while noting limited patient-level evidence and the feature’s wellness positioning (review).
- In adults experiencing acute COPD exacerbations, a Series 6 study reported strong correlation with medical-grade pulse oximetry (r = 0.89) and an intraclass correlation coefficient of 0.940. Correlation means readings tended to move together; it does not establish that every displayed value was interchangeable with the reference (COPD study).
- A Series 6 study found the watch could detect short periods below 90% compared with a medical-grade device. Detecting a broad desaturation event is not the same as accurately measuring its exact percentage (controlled hypoxemia study).
- In a controlled hypoxia study, Series 7 had an accuracy root-mean-square difference of 5.82 percentage points below 88%, compared with 3.52 points for the clinical-grade comparator in that study. This result is a particular model and test condition, but it underscores the weakness of using the watch to interpret low oxygen values (study).
Results vary with the watch generation, oxygen range, participants, fit, movement, and reference method. The evidence supports conditional usefulness, not a guarantee that any reading is accurate. Apple’s validation materials do not change its stated wellness-only positioning (Apple validation white paper).
When the watch is useful—and when it is not
Better suited to broad patterns
Repeated readings taken under similar, settled conditions may help you notice that readings appear different from your usual pattern. The watch is convenient for background measurements, including during sleep when Track Sleep with Apple Watch is enabled. These readings are intermittent, however, and can be affected by wrist position, movement, fit, cold skin, and circulation. A pattern can be a reason to seek more appropriate measurement or advice; it is not a diagnosis.

Not a stand-alone medical monitor
Do not use the watch by itself to manage COPD, asthma, pneumonia, COVID-19, heart disease, or prescribed oxygen, or to decide whether urgent care is needed. Studies in particular patient groups do not validate the feature for every illness or home-monitoring plan. Follow the plan provided by your clinician.
Shortness of breath, chest pain, confusion, fainting, blue lips, or worsening symptoms deserve attention regardless of a watch reading. The FDA advises against relying only on pulse-oximeter numbers and recommends considering symptoms and clinical advice (FDA pulse oximeter guidance). There is no universal watch-reading threshold that determines whether a person is safe: baseline, altitude, health conditions, symptoms, and clinician instructions all matter.
Sleep and suspected sleep apnea
Overnight oxygen fluctuations on a watch are not equivalent to continuous clinical oximetry or a sleep study. The feature does not diagnose sleep apnea. If you suspect sleep apnea, seek a medical evaluation or an appropriate sleep-testing pathway rather than trying to diagnose it from watch data.
How to get a better reading
Apple’s instructions are to wear the watch snugly and comfortably, with its back flush against the top of the wrist. If the wrist bone prevents a flush fit, move the watch about 1–2 inches (2.5–5 cm) above it. For an on-demand measurement:

- Open the Blood Oxygen app on the watch.
- Rest your arm on a table or in your lap, with your wrist and palm flat and the display facing up.
- Tap Start and stay completely still through the 15-second countdown.
- View the result on the watch, or—in applicable U.S. configurations—in the Health app on the paired iPhone.
For a practical repeat check, warm a cold hand and wrist, sit quietly for several minutes, and take two or three readings under similar conditions. Do not average a result taken during movement or with poor contact into the others. If a result is unexpected, compare it with a properly used, credible fingertip pulse oximeter rather than treating the watch value as decisive.
Common reasons for a failed or questionable result

A failed measurement and a plausible-looking but inaccurate number are not the same. A failed result may mean the watch could not get a suitable signal; a number that looks ordinary does not prove the measurement was accurate. Apple lists these measurement limitations in its Blood Oxygen app guidance.

A fingertip device is not automatically accurate just because it clips onto a finger. The FDA’s guidance discusses limitations affecting pulse oximeters generally; it should not be read as proof of a particular Apple Watch error rate for a given skin tone. The available evidence does not establish a precise Apple Watch-specific error estimate across skin pigmentation, especially in real-world conditions and at low oxygen levels.
Which Apple Watches support it, and what about the United States?
Apple lists the Blood Oxygen app for Apple Watch Series 6 or later and all Apple Watch Ultra models. It is not supported on Apple Watch SE or Apple Watch For Your Kids, is unavailable to users under 18, and depends on country or region. Check Apple’s current availability and requirements for your location and model.

U.S. availability has changed, so “the feature is banned in the U.S.” is too broad. Apple announced redesigned Blood Oxygen functionality for some U.S. Series 9, Series 10, and Ultra 2 users in August 2025, with iOS 18.6.1 and watchOS 11.6.1. For affected configurations, the paired iPhone measures and calculates the data and results appear in Health rather than directly on the watch (Apple’s U.S. update).
Apple’s current documentation says U.S. models purchased on or after January 18, 2024, with part numbers ending in LW/A process the data on the paired iPhone. Exact availability can depend on the watch’s model, purchase region, part number, paired iPhone, and software. This matters especially with refurbished or imported watches; check the exact configuration rather than relying on the model name alone (Apple Watch User Guide).
Should you buy an Apple Watch for blood oxygen?
Choose an Apple Watch for its broader smartwatch and health features if occasional wellness readings are an added benefit. Do not buy it primarily as a dependable medical oxygen monitor. If you need spot checks, look for a fingertip oximeter with credible authorization or validation and transparent performance information, and use it as your clinician recommends. If you have a condition that requires monitoring, ask your clinician which device and response plan are appropriate.
