On a compatible Apple Watch, turn on irregular rhythm notifications from your iPhone: Health → Search → Heart → Irregular Rhythm Notifications → Set Up. Complete the age, AFib-diagnosis, and notification prompts.
Before starting, confirm that the feature is available in your country or region, both devices are updated, you are at least 22, you have not been diagnosed with AFib, and AFib History is not enabled. Apple Watch checks periodically for patterns suggestive of AFib; it does not continuously monitor your heart or diagnose AFib.

Before you begin
- Apple Watch: Apple’s regulatory instructions list Apple Watch Series 3 or later, subject to regional and software availability. Check Apple’s current compatibility information if you are unsure.
- Software: Update the iPhone and Apple Watch to the latest available versions.
- Age: Apple says the feature is not intended for people under 22.
- Diagnosis: Irregular Rhythm Notifications are intended for people who have not been diagnosed with AFib. If you already have AFib, discuss Apple’s AFib History feature and monitoring options with your clinician.
- AFib History: It must not be enabled. Setting up AFib History can automatically turn off irregular rhythm notifications.
- Region: Apple’s heart-health features are not available in every country or region.
Apple’s official regulatory instructions identify Series 3 or later as compatible with the Irregular Rhythm Notification Feature, but model, region, and software requirements can differ. See the official instructions if the option does not appear.

- On your iPhone, open the Health app.
- Tap Search.
- Tap Heart.
- Tap Irregular Rhythm Notifications.
- Tap Set Up.
- Follow the prompts. You will be asked to confirm your age, state whether a physician has diagnosed you with AFib, review information about the feature, and enable notifications.
Menu labels can vary slightly by iOS version or region. The Health app route is the most dependable current path. The Apple Watch user guide also describes starting setup from Apple Watch app → My Watch → Heart → Set Up Irregular Rhythm Notifications in Health.

How to check or turn off the setting later
After setup, manage the feature from:
iPhone → Apple Watch app → My Watch → Heart → Irregular Rhythm
From there, you can check whether irregular rhythm notifications are enabled and switch them off if necessary. Turning the feature off does not affect the separate ECG app or other heart measurements.
What irregular rhythm notifications actually do
The feature examines pulse data periodically for a pattern that may suggest atrial fibrillation, or AFib. It often attempts readings when you are relatively still because movement can interfere with pulse measurements. The watch may need multiple readings before sending an alert.
An alert means Apple Watch identified and confirmed a pattern suggestive of AFib. It is not a definitive diagnosis, and some irregular rhythms may have causes other than AFib. Apple also says the feature is not continuously checking and cannot detect every instance of AFib.

Likewise, receiving no alert does not prove that your rhythm is normal. The watch may not have been checking during an episode, may not have obtained a reliable reading, or may not identify the particular rhythm or condition involved.
What to do if you receive an alert
- Read the notification and note when it appeared.
- If you have not been diagnosed with AFib, arrange a medical evaluation and show the notification to your clinician.
- If your watch supports the ECG app and you are having symptoms, record an ECG while the symptoms are occurring and save or share the result with your clinician.
- Do not start, stop, or change medication based only on a watch alert. Discuss medication with a healthcare professional.
The ECG app is a separate, user-initiated recording that takes about 30 seconds. Apple Watch cannot detect a heart attack or stroke. Seek emergency help for chest pain or pressure, severe shortness of breath, fainting, or possible heart-attack or stroke symptoms. Do not wait for another watch notification.

The ECG app requires Apple Watch Series 4 or later or any Apple Watch Ultra, subject to regional availability. It is not supported on Apple Watch SE or Apple Watch For Your Kids. You do not need the ECG app for irregular rhythm notifications, and ECG availability does not by itself prove that irregular rhythm notifications are available on a particular watch.

Why the option is missing
Check these possibilities in order:
- Region: The feature may not be approved or available where you live. Apple’s health features vary by country and region.
- Software: Update both the iPhone and Apple Watch, then reopen the Health app.
- Age: The setup flow is not intended for people under 22.
- Existing AFib diagnosis: Apple does not design this feature for people already diagnosed with AFib.
- AFib History: If it is enabled, irregular rhythm notifications may have been turned off automatically. Do not disable a clinician-recommended setting simply to restore alerts.
- Watch model: Verify the model against Apple’s current compatibility information. Series 3 or later is listed in Apple’s regulatory instructions, but regional and software restrictions still apply.
- Pairing and permissions: Confirm that the watch is paired normally with the intended iPhone and that Health setup and permissions are complete.
Why notifications may not arrive
No alert can have several explanations. The watch checks periodically rather than continuously; you may not have worn it consistently; the band may have been loose; movement may have prevented a reliable reading; or the rhythm may not have met Apple’s detection and confirmation conditions.
Also check whether Low Power Mode is enabled. Apple says Low Power Mode turns off high-heart-rate, low-heart-rate, and irregular-rhythm notifications. A missing alert does not rule out AFib or another heart condition.
If you need monitoring that is more continuous or clinically directed because of recurring symptoms, ask a clinician whether an ambulatory ECG or medical patch monitor is appropriate. Apple Watch’s background feature is not a replacement for diagnostic monitoring.

Irregular Rhythm Notifications are not intended for people with an existing AFib diagnosis. Apple’s AFib History feature is designed for diagnosed users and provides retrospective estimates of AFib burden rather than an alert when an episode begins.

Use the feature your healthcare professional recommends, and do not change treatment or monitoring based solely on Apple Watch data.

Do not wait for the watch. Palpitations, a rapid or fluttering heartbeat, dizziness, fainting, chest pain, or shortness of breath deserve medical judgment even if no notification appears. For chest pain, pressure, tightness, severe breathing difficulty, fainting, or possible heart-attack or stroke symptoms, contact emergency services immediately.

Do I need the ECG app?
No. Irregular rhythm notifications and ECG are separate features. ECG can provide a user-initiated recording on supported models and in supported regions, but it is not required for background notifications.

Use Health → Search → Heart → Irregular Rhythm Notifications → Set Up to enable the feature, then manage it in Apple Watch → My Watch → Heart → Irregular Rhythm. Treat an alert as a prompt for medical follow-up—not a diagnosis—and never interpret the absence of an alert as proof that your heart rhythm is normal.
