How to use Apple Watch’s Hypertension Notifications in watchOS 26

Apple Watch’s watchOS 26 hypertension feature detects patterns associated with possible hypertension—it does not measure blood pressure. Here is how to enable it, complete the follow-up cuff readings, and troubleshoot missing alerts.

Apple Watch can screen for patterns associated with possible hypertension, but it cannot measure your blood pressure. In watchOS 26, the Hypertension Notifications feature analyzes data from the watch’s optical heart sensor during recurring 30-day evaluation windows. If it detects a concerning pattern, you must confirm it with a separate blood-pressure cuff and follow up with a healthcare professional.

This guide covers eligibility, setup, the 30-day analysis period, the seven-day cuff log Apple recommends after a notification, troubleshooting, and the feature’s medical limitations.

The short version: Turn it on from Health → profile picture or initials → Health Checklist → Hypertension Notifications on the paired iPhone. Then wear the watch consistently while awake. The watch does not produce a systolic or diastolic reading, and a missing notification does not prove that your blood pressure is normal.

What Apple Watch’s hypertension feature actually does

Hypertension Notifications is a passive screening feature. It uses photoplethysmography (PPG) data gathered by the Apple Watch’s optical heart sensor and analyzes that data for patterns suggestive of possible chronic high blood pressure. It is not the same as an irregular-heart-rhythm or AFib notification feature.

The watch does not inflate a cuff, calculate a blood-pressure value, or display systolic and diastolic measurements. It also does not diagnose hypertension, monitor whether treatment is working, or replace a clinician’s blood-pressure assessment. Apple’s regulatory documentation describes it as software intended to provide a notification that prompts follow-up, not as a standalone blood-pressure monitor. There are three separate things that are easy to confuse:

  • Hypertension Notifications: A background analysis of optical heart-sensor data that may send a possible-hypertension notification.
  • Blood Pressure Log: An Apple Health feature for manually storing systolic and diastolic numbers obtained from another device.
  • Actual blood-pressure measurement: A reading taken with an appropriate blood-pressure monitor, usually an upper-arm cuff. The cuff supplies the numbers that you enter into Health.

Check compatibility before looking for the setting

As of Apple’s August 10, 2026 supported-model listing, Hypertension Notifications works on the following Apple Watch models:

Apple’s watchOS 26 supported-model documentation lists Series 9 through Series 11 and Ultra 2 through Ultra 3. The feature is not available on Apple Watch SE models.

Eligibility checklist

You generally need all of the following:

  • Apple Watch Series 9, Series 10, Series 11, Ultra 2, or Ultra 3.
  • The latest compatible watchOS 26 software.
  • An iPhone 11 or later running the latest compatible iOS 26 software.
  • The watch paired with that iPhone.
  • Wrist Detection enabled.
  • A country or region where the feature is available.
  • You are at least 22 years old.
  • You have not previously been diagnosed with hypertension.
  • You are not pregnant, and an active pregnancy is not recorded in the Health app.

Onboarding must be completed on the paired iPhone; it cannot be completed directly on the Apple Watch. Apple also limits the feature by country and region. The United States is listed as supported, but availability can change and is not universal. Check Apple’s watchOS feature-availability page for your location.

Menu wording can vary slightly with language, region, or later watchOS 26 updates. The relevant area is still the Health app’s Health Checklist, and the feature is called Hypertension Notifications. Apple’s regulatory documents refer to the same feature as the Hypertension Notification Feature, or HTNF.

How to turn on Hypertension Notifications

Use the Health app on the iPhone paired with your Apple Watch:

  1. Open the Health app.
  2. Tap your profile picture or initials in the upper-right corner.
  3. Under Features, tap Health Checklist.
  4. Tap Hypertension Notifications.
  5. Confirm your age and answer whether you have ever been diagnosed with hypertension.
  6. Tap Continue.
  7. Tap Next and read Apple’s explanation of how the feature works.
  8. Tap Done.

Once onboarding is complete, the feature runs in the background. There is no separate cuff-pairing step, and you do not need to start a workout or manually initiate a heart-rate reading for the analysis.

If you cannot find the menu, use the troubleshooting section below rather than assuming that your watch is analyzing blood pressure anyway.

What happens during the 30-day analysis

Hypertension Notifications evaluates watch data across a 30-day window. After one window ends, another begins automatically. The watch is not expected to issue a daily blood-pressure result or a progress score.

Apple’s regulatory instructions add an important detail that is easy to miss in the basic setup instructions: the feature needs at least 14 days of usable data collected while you are awake during each 30-day window. Wearing the watch consistently during the day gives the system more opportunity to collect the required data.

During this period:

  • The feature operates passively in the background.
  • You may receive a possible-hypertension notification before or after a full 30-day period only if the required analysis and data conditions are met; do not expect a daily result.
  • If Apple does not identify a qualifying pattern, it does not send a normal, clear, or all-good notification.
  • Removing the watch frequently, leaving it powered off, disabling Wrist Detection, or having too little usable awake data can prevent the analysis from completing.
  • If the watch’s storage is full, it may be unable to collect new data. Apple recommends freeing space by deleting unwanted apps, music, or podcasts.

A 30-day period with no notification therefore has several possible explanations: no qualifying pattern was identified, the watch did not collect enough usable data, or the feature missed the condition. It is not a blood-pressure check with a reassuring negative result.

What a Possible Hypertension notification means

Possible Hypertension means that Apple Watch detected a pattern associated with possible hypertension in recent heart-sensor data. It is not a blood-pressure reading and not a diagnosis.

When the notification arrives, do not try to confirm it by looking for a number on the watch. Instead:

  1. Read the notification and its follow-up instructions.
  2. Arrange an appointment or discuss the result with a healthcare professional.
  3. Use a separate, appropriate blood-pressure monitor.
  4. Take readings twice a day for seven days.
  5. Record the systolic and diastolic values in Apple Health.
  6. Bring or share the seven-day results with the healthcare professional.

Notification history is saved in the Health app on the iPhone or iPad. Apple says the health data can be shared by exporting health data, which may be useful when sending the record to a clinician.

Set up the seven-day Blood Pressure Log

The Blood Pressure Log does not turn the Apple Watch into a blood-pressure monitor. It is simply a place in Apple Health to store measurements from a cuff or other appropriate blood-pressure monitor.

To start the log after a notification:

  1. Open Health on the iPhone.
  2. Tap Search.
  3. Tap Heart.
  4. Tap Blood Pressure.
  5. Tap Blood Pressure again.
  6. Under Blood Pressure Log, tap Get Started.
  7. Follow the prompts and schedule reminders.

Apple’s reminders are intended for after waking and immediately before bedtime. Follow the schedule shown in your Health app and try to take the readings at roughly consistent times each day.

Enter each cuff result manually

  1. Go to Health → Search → Heart → Blood Pressure.
  2. Tap Blood Pressure.
  3. Under Blood Pressure Log, tap Add a Measurement.
  4. Adjust the date and time if necessary.
  5. Enter the Systolic value, the upper number.
  6. Enter the Diastolic value, the lower number.
  7. Tap Done.

Check that both numbers and the measurement time are correct before saving. The Apple Watch is not supplying either value; you are transcribing the result from the external monitor.

How to take the seven-day readings properly

For U.S. users, Apple’s regulatory instructions call for an FDA-cleared blood-pressure monitor. The American Heart Association recommends an automatic, validated upper-arm cuff with the correct cuff size for home monitoring. You can search the ValidateBP device listing for monitors that have undergone its validation process. Outside the United States, choose a device appropriately authorized and validated for your country, and ask a healthcare professional if you are unsure.

Before each session:

  • Do not smoke, exercise, or consume caffeine for 30 minutes beforehand.
  • Empty your bladder.
  • Sit quietly for at least five minutes.
  • Support your back and keep both feet flat on the floor.
  • Keep your legs uncrossed.
  • Place the bare upper arm on a flat surface at heart level.
  • Use the correct cuff size and follow the monitor’s placement instructions.
  • Do not talk, text, or use the phone while measuring.

Take at least two readings one minute apart and record both. Repeat the process in the morning and evening for seven days, using approximately consistent times. Apple’s workflow uses the seven-day log for clinician follow-up; it is not asking you to estimate your pressure from the watch.

One reading is only a snapshot. Home monitoring can provide useful information, but it does not replace clinical care. The American Heart Association’s home blood-pressure guidance provides additional measurement instructions.

When a high cuff reading needs urgent action

If a cuff reading is higher than 180/120 mm Hg, wait at least one minute and measure again.

  • If it remains above 180/120 without concerning symptoms, contact a healthcare professional promptly.
  • If it remains above 180/120 and you have symptoms such as chest pain, shortness of breath, back pain, numbness, weakness, vision changes, or difficulty speaking, call 911.

This safety guidance is separate from the Apple Watch notification process. Do not wait for a watch notification, a seven-day log, or a future 30-day window when a cuff reading or symptoms require urgent care. If no notification arrives

No alert after 30 days does not mean your blood pressure is normal. Possible reasons include:

  • The watch did not identify a qualifying pattern.
  • The current evaluation window has not completed.
  • You had fewer than 14 days of usable awake data.
  • You did not wear the watch consistently.
  • Wrist Detection was disabled.
  • The watch was frequently off your wrist or powered off.
  • The watch ran out of available storage and could not collect new data.
  • The feature missed the condition. Apple explicitly states that it cannot identify every instance of hypertension.

If you already have high cuff readings, symptoms, or a clinician’s concern, arrange medical follow-up rather than waiting for Apple Watch to produce an alert.

Troubleshoot a missing Hypertension Notifications setting

Work through these checks in order:

  1. Verify the model. Series 8 and earlier watches, all Apple Watch SE models, and older Ultra models are not supported.
  2. Check regional availability. Look up your country on Apple’s watchOS feature-availability page.
  3. Update both devices. Install the latest compatible iOS 26 release on the iPhone and the latest compatible watchOS 26 release on the watch.
  4. Confirm pairing. The supported watch must be paired with the iPhone where you are opening Health.
  5. Enable Wrist Detection. Check the Wrist Detection setting in the Apple Watch settings on the paired iPhone.
  6. Check eligibility answers. The feature is unavailable for people under 22, people who report a previous hypertension diagnosis, or people with an active pregnancy recorded in Health.
  7. Open the correct location. Go to Health → profile picture or initials → Health Checklist. Searching only in the Watch app may not find the feature.
  8. Check watch storage. If storage is full, remove unwanted apps, music, or podcasts so new data can be collected.
  9. Contact Apple Support. If the model, software, pairing, region, Wrist Detection, eligibility, and storage all check out but the setting is still missing, Apple Support is the appropriate next step.

A missing menu is a software, eligibility, region, or hardware-support issue. It does not provide information about your blood pressure.

Who should not use this feature?

People already diagnosed with hypertension

Hypertension Notifications is intended for people who have not previously been diagnosed with hypertension. Apple’s regulatory documentation says the feature is not intended to monitor treatment effectiveness or provide ongoing blood-pressure surveillance for people with an existing diagnosis.

That limitation does not prevent you from using Apple Health’s manual Blood Pressure Log as part of a clinician-directed monitoring plan. It simply means that the watch’s passive notification feature should not be treated as a tool for deciding whether prescribed treatment is working.

People who are pregnant

Apple says the feature should not be used during pregnancy, and an active pregnancy recorded in the Health app prevents onboarding. Pregnancy-related blood-pressure concerns require pregnancy-specific medical advice; do not wait for a watch notification.

How reliable is the feature?

Apple’s Hypertension Notification Feature received FDA clearance through the 510(k) pathway under number K250507, with a decision date of September 11, 2025. The FDA decision was a substantial-equivalence clearance for the software’s intended notification use. It was not clearance for the Apple Watch to function as a standalone blood-pressure monitor. The FDA decision record and 510(k) summary describe the intended use and regulatory basis.

Apple’s U.S. Instructions for Use report a prospective validation study with 2,229 enrolled participants who did not have a previous hypertension diagnosis. Of those, 1,863 contributed at least 15 days of usable data to the primary endpoint analysis. Apple reported:

These are results from Apple’s validation study, not a guarantee for every person or situation. The most important practical implication is that the feature can miss a substantial proportion of hypertension cases. A lack of notification should never override concerning cuff readings, symptoms, or a clinician’s advice. Alternatives and complementary tools

If your goal is to know your actual blood pressure, the primary tool is a validated automatic upper-arm cuff. A clinician’s measurement is also important for interpretation, diagnosis, and treatment decisions. In some cases, a clinician may recommend ambulatory blood-pressure monitoring over an extended period to confirm a pattern.

Apple Health’s Blood Pressure Log is useful for organizing readings from an external monitor, but entering numbers into Health does not make the Apple Watch measure them. Be cautious with other wearable apps and cuffless devices that claim to measure or estimate blood pressure. The FDA warns consumers about unauthorized devices and software features making such claims; use an appropriately authorized device and discuss results with a healthcare professional. How to turn Hypertension Notifications off

  1. Open Health on the iPhone.
  2. Tap your profile picture or initials.
  3. Tap Health Checklist.
  4. Tap Hypertension Notifications.
  5. Turn Hypertension Notifications off.

Turning off the notification feature is separate from deleting blood-pressure entries already stored in Apple Health. Disabling notifications does not automatically remove your manually entered cuff readings.
