The feature looks for patterns in optical heart-sensor data over 30-day evaluation periods; it does not measure blood pressure or diagnose hypertension.
To use it, you also need an iPhone 11 or later with the latest iOS, Wrist Detection enabled, and must be at least 22, not pregnant, and not previously diagnosed with hypertension. If you receive a notification, follow up with a healthcare professional and track readings with a third-party blood-pressure cuff for seven days.

What the feature does—and does not do
Hypertension Notifications analyzes data from the watch’s optical heart sensor over 30-day evaluation periods and looks for patterns associated with chronic high blood pressure. It does not take a cuff-style blood-pressure reading or display a systolic or diastolic result.

A notification is a prompt to follow up, not a blood-pressure result or diagnosis. Apple says not all people with hypertension receive a notification. The feature is not intended to diagnose, treat, or help manage hypertension or other conditions.
Requirements to use it
- Apple Watch Series 9 or later, or Apple Watch Ultra 2 or later, with the latest version of watchOS.
- iPhone 11 or later with the latest version of iOS.
- Wrist Detection enabled.
- Availability in your country or region.
- You must be at least 22, not pregnant, and not previously diagnosed with hypertension. An active pregnancy recorded in Health prevents activation.
How to turn on Hypertension Notifications
- On your iPhone, open the Health app and tap your profile.
- Tap Health Checklist, then Hypertension Notifications.
- Confirm the eligibility information and follow the prompts to finish setup.
If the option is missing, check the exact watch model, software versions, Wrist Detection setting, regional availability, and eligibility. Apple’s instructions may change, so consult its current feature guidance.

What to do if you receive a notification
Follow up with a healthcare professional and discuss the notification and blood-pressure measurements. Apple prompts users to set up a Blood Pressure Log and use a third-party cuff to measure and track blood pressure for seven days. The watch notification itself is not a measurement.
Use a cuff that fits properly and follow its instructions. Apple’s guidance directs users to use a third-party blood-pressure cuff; the dossier does not endorse a particular model. When choosing one, check independent validation and cuff sizing. A relevant search is for a home blood pressure monitor cuff.

Is it worth upgrading from an unsupported Apple Watch?
Hypertension Notifications may be one consideration if you are choosing a watch, but an upgrade is not necessary for everyone and does not replace blood-pressure monitoring or medical care. Check that the specific model, iPhone, software, region, and your eligibility meet Apple’s requirements before buying. The Apple Watch Series 9 and Apple Watch Ultra 2 are the earliest generations in their respective families included in Apple’s stated support range.

Can no notification rule out hypertension?
No. Apple says not all people with hypertension receive a notification. A lack of an alert does not establish that your blood pressure is normal.
Can Apple Watch detect a heart attack?
No. Apple says Apple Watch cannot detect heart attacks. For chest pain, pressure, tightness, or suspected heart attack, call emergency services immediately.
