The update was scheduled to roll out progressively from February 4, 2021, with market-specific timing varying by country and carrier.
The list below describes that January 2021 expansion—not every country where ECG is supported today. Samsung’s later regional documentation shows broader availability and separates Watch3 support from markets limited to Watch4 and newer models.
The 31 countries added in Samsung’s January 2021 expansion
Samsung described the expansion as 28 European countries plus Chile, Indonesia and the United Arab Emirates. The complete list was:

When did ECG become available?
Samsung announced the expansion on January 26, 2021 and said compatible Samsung Health Monitor updates would begin rolling out progressively on February 4. That was a rollout start, not a promise that every watch would activate on that date. Samsung warned that availability could vary by market and carrier.

What Samsung actually changed
This was a software and regulatory expansion, not a new Watch3 hardware revision. Samsung enabled Samsung Health Monitor’s ECG function on the existing Galaxy Watch3 and Galaxy Watch Active2 through watch and phone updates.
Samsung said the app received CE marking in December 2020, allowing the European expansion. CE marking was the European compliance step Samsung cited; it did not automatically approve the service in every country worldwide. Chile, Indonesia and the UAE were separately named as additional markets in the announcement.

Requirements for the 2021 Watch3 ECG rollout
- Galaxy Watch3 or Galaxy Watch Active2.
- A compatible Samsung Galaxy smartphone.
- Android 7.0 or later on the paired phone—the requirement Samsung stated in 2021.
- Samsung Health Monitor installed on both the watch and phone.
- Current watch, Galaxy Wearable and phone software.
When the watch was updated, Samsung said the watch-side app could install automatically and direct the user to download Samsung Health Monitor from the Galaxy Store on the phone. Later app versions and regional documentation may specify newer requirements, so Android 7.0 should be read as the original announcement requirement, not a universal current minimum. Samsung’s compatibility guidance is available through its US support page.
How to set up an ECG on a Galaxy Watch3
- Update the watch through the Galaxy Wearable app.
- Install or update Samsung Health Monitor on the paired Samsung Galaxy phone.
- Open Samsung Health Monitor and complete the ECG setup, permissions and safety notices.
- Follow the on-screen instructions to start a recording.
- Wear the watch snugly, keep still and maintain the instructed finger and skin contact until the recording finishes.
Samsung’s instructions say to confirm that the ECG app is approved in your country and that the watch is compatible before setup: ECG instructions for use.
Regional restrictions matter more than the model name
Not every Galaxy Watch3 can use ECG simply because it contains the necessary sensors. Eligibility can depend on the country where the watch and phone were sold, regulatory approval, Samsung Health Monitor availability, firmware, phone compatibility and regional service restrictions.
Samsung says the service may work only on watches and smartphones purchased in a supported country or region. Travel, a factory reset, deleting and reinstalling the app, or combining devices from different sales regions can affect availability. Changing a phone’s language, SIM card or location is not an official unlock method, and sideloading an APK is not a dependable solution.

An imported Watch3 can therefore have ECG-capable hardware but still fail to offer the official app. Samsung’s regional information includes country-specific restrictions; examples are its India, Australia and Gulf pages.
What the ECG result can—and cannot—tell you
The watch records electrical heart activity and can produce classifications suggestive of atrial fibrillation. It is not a complete diagnostic ECG, does not detect a heart attack, and does not replace a clinician, clinical ECG equipment or emergency care. Samsung’s warnings also state that the app is not intended for people under 22 or for users with known arrhythmias other than atrial fibrillation, subject to the applicable regional instructions.
Chest pain, fainting, severe breathing difficulty or other serious symptoms require emergency medical care, regardless of what a watch recording says. A watch result should not be the sole basis for changing treatment or making a clinical decision.

Why a Watch3 may still lack ECG
- The watch or phone was sold for an unsupported region.
- Samsung Health Monitor is unavailable for the device’s regional configuration.
- The phone is not a compatible Samsung Galaxy model; Samsung’s Watch3 ECG workflow is not officially supported on generic Android phones.
- Watch, Galaxy Wearable or Samsung Health Monitor software is out of date.
- Carrier or market rollout timing delayed activation.
- The device was reset, re-provisioned or paired across incompatible regions.
If the app is missing
- Confirm that the watch is a Galaxy Watch3 or Watch Active2.
- Update Galaxy Wearable and the watch firmware.
- Install or update Samsung Health Monitor on the Galaxy phone.
- Verify that the phone is a supported Samsung Galaxy model.
- Check the watch and phone’s original sales region against Samsung’s regional support information.
- Avoid unofficial APKs and region-spoofing tools; contact Samsung support if the official app remains unavailable.
If a recording cannot be analyzed
Movement, loose fit, poor skin contact and recording noise can produce unusable results. Wear the watch snugly, rest the arm, remain still and follow the app’s finger-contact instructions. Samsung discusses these recording conditions in its medical-device material: Samsung Research.
Is the 31-country list still the current global list?
No. The 31 countries are the markets in Samsung’s January 2021 expansion. Later Samsung documentation shows that ECG availability grew beyond those markets and that some countries support ECG only on Galaxy Watch4 and newer hardware. The dated Samsung availability PDF, marked last updated September 30, 2025, is the latest dated official list identified here: Samsung regional availability PDF. Availability can change, so check that document and your local Samsung support page before relying on a Watch3 purchase or setup.
Newer Galaxy Watches may support additional health features, including irregular-heart-rhythm notifications, that are not automatically available on the Watch3. Feature eligibility is model- and region-specific.
