A Blood Pressure Monitor for Smartwatches: What They Really Measure in 2026

Some smartwatches estimate blood pressure after cuff calibration, while Apple Watch detects possible hypertension without showing numbers. Here is what each feature really does—and when you still need an upper-arm cuff.

Yes—but not in one universal way. Some smartwatches estimate systolic and diastolic blood pressure after calibration with an upper-arm cuff. Others, notably supported Apple Watch models, look for patterns associated with possible hypertension but do not display a blood-pressure reading. Neither approach generally replaces a validated automatic upper-arm monitor when you need a diagnosis, medication guidance, or dependable home measurements.

The important question is not simply whether a smartwatch “measures blood pressure.” It is whether the watch produces a number, detects a risk pattern, or uses a pressure-based cuff—and whether that specific feature is authorized and validated for your intended use.

Cuffless estimation

A watch can use photoplethysmography (PPG)—optical sensing of blood-volume changes in the wrist—and an algorithm to estimate blood pressure. Samsung’s compatible Galaxy Watch feature works this way. It must first be calibrated against an upper-arm cuff, and the calibration must be repeated regularly. See Samsung Health Monitor.

Hypertension-pattern detection

Apple’s Hypertension Notifications feature does not calculate and show systolic and diastolic values. Instead, it analyzes heart-sensor data over time and can alert a user to a pattern associated with possible chronic high blood pressure. Apple then directs the user to confirm the concern with a separate cuff and a healthcare professional. See Apple’s feature requirements and instructions.

Pressure-based wrist devices

A purpose-built wrist monitor may contain an inflatable bladder or use another pressure-based method. That is a different category from an optical smartwatch estimate. A wrist-worn form factor does not, by itself, establish accuracy or regulatory authorization; evaluate the exact device and intended use.

Why heart rate is not blood pressure

Smartwatches can measure pulse relatively easily because heart rate is the frequency of beats. Blood pressure is different: it has two values—systolic pressure, the pressure when the heart contracts, and diastolic pressure, the pressure between beats. Estimating those values also depends on arterial stiffness, posture, arm position, sensor contact, calibration, movement, and individual physiology.

Consequently, a watch’s heart-rate or ECG feature is not a blood-pressure monitor. Nor does an app displaying a number prove that the number is clinically reliable. The source, measurement method, authorization, validation conditions, and intended use all matter.

Samsung Galaxy Watch: the numeric-reading option

Samsung’s blood-pressure feature can display systolic pressure, diastolic pressure, and pulse on a compatible Galaxy Watch or paired phone. Samsung describes it as a wellness feature, not a tool for diagnosing or treating a medical condition.

What you need

  • A compatible Galaxy Watch, generally Galaxy Watch4 and later released Galaxy Watch models; Galaxy Fit is excluded.
  • A compatible Galaxy smartphone.
  • The Samsung Health Monitor app and current software.
  • A separate automatic upper-arm cuff.

Availability depends on the country or region, watch model, paired phone, software, and applicable regulatory approval. Check Samsung’s local documentation before buying.

How to calibrate it

  1. Rest quietly for at least five minutes.
  2. For at least 30 minutes beforehand, avoid caffeine, alcohol, smoking, exercise, and bathing.
  3. Put the upper-arm cuff on one arm and the watch on the opposite wrist.
  4. Follow Samsung Health Monitor’s instructions and complete three cuff measurements within 30 minutes.
  5. Use the resulting calibration to take watch readings.
  6. Repeat calibration every 28 days.

Samsung lists an approximate displayed operating range of 70–180 mmHg systolic and 40–120 mmHg diastolic. Its narrower calibration range is approximately 80–170 systolic and 50–110 diastolic. These are device operating ranges, not a promise that the feature is suitable for every clinical situation.

How to take a reading

  • Use the same wrist and broadly the same posture used during calibration.
  • Sit with your back supported, feet flat, legs uncrossed, and forearm supported.
  • Remain still and do not talk.
  • Measure indoors in a quiet setting.
  • Make sure the watch is positioned and fastened as instructed, with clean, dry skin and good sensor contact.

Sweat, lotion, movement, a loose or overly tight strap, poor arm support, and changes in physiology can affect the optical signal. Recalibration every 28 days is a reminder that the estimate is not permanently individualized.

A Galaxy Watch is therefore not a self-contained blood-pressure monitor. It still depends on an upper-arm cuff, both initially and periodically. It is most useful to someone who already owns a compatible Galaxy phone and wants convenient trend information—not to someone seeking a standalone clinical monitor.

Apple Watch: a hypertension alert, not a blood-pressure monitor

Supported Apple Watch models can provide Hypertension Notifications, but Apple Watch is not a blood-pressure monitor in the ordinary numeric sense. It does not show you a systolic or diastolic reading.

The feature analyzes heart data over 30-day periods and looks for patterns associated with possible hypertension. It does not require cuff calibration, but a separate cuff is still necessary if Apple sends a notification or if you need actual blood-pressure readings.

Requirements and exclusions

  • Apple Watch Series 9 or later, or Apple Watch Ultra 2 or later.
  • iPhone 11 or later with current software.
  • User age 22 or older.
  • Not intended for people already diagnosed with hypertension.
  • Not intended for use during pregnancy.
  • Unavailable on Apple Watch SE.
  • Availability varies by country or region.

How to turn it on

  1. Open the Health app on the iPhone.
  2. Tap the profile icon.
  3. Open Health Checklist.
  4. Tap Hypertension Notifications.
  5. Confirm the requested age and diagnostic-history information.
  6. Follow the prompts and tap Done.

What to do after a notification

Do not treat the alert as a diagnosis. Use a separate cuff and follow Apple’s instructions to record readings twice daily for seven days. Share the log with a healthcare professional. A lack of notification also does not rule out hypertension; Apple says not everyone with hypertension will receive one.

The FDA has authorized Apple’s Hypertension Notification Feature, listed under K250507. That authorization applies to the specific notification software feature—not to a claim that Apple Watch provides cuff-equivalent blood-pressure measurements.

What FDA authorization and validation actually mean

These terms are often treated as interchangeable, but they answer different questions:

  • FDA authorization or clearance: The product or software has passed the applicable FDA marketing pathway for its stated intended use.
  • Clinical validation: Evidence that readings or notifications perform acceptably under specified test conditions.
  • Diagnostic suitability: Whether clinicians can safely use the result to diagnose hypertension or adjust treatment.
  • Independent validation: Testing by researchers or validation bodies under accepted protocols, rather than relying only on manufacturer claims.

FDA authorization does not mean a device is clinically perfect in every person and every situation. The American Heart Association has specifically cautioned that FDA clearance alone does not guarantee formal accuracy testing under a standardized protocol for all cuffless technologies. Its December 2025 scientific statement says cuffless wearables remain promising but are not yet sufficiently proven for diagnosing hypertension or guiding treatment decisions in ordinary real-world use. The FDA warns consumers not to use unauthorized devices or software features that claim to measure or estimate blood pressure. It recommends checking the device’s authorization in the FDA’s databases; blood-pressure devices commonly use product code DXN. The FDA database separately lists Apple’s feature under K250507 and Aktiia’s G0 Blood Pressure Monitoring System under K250415. How accurate are smartwatch blood-pressure readings?

The defensible answer is: accuracy varies, and current evidence does not justify treating most cuffless smartwatch readings as interchangeable with a validated upper-arm cuff.

Cuffless systems are attractive because they can collect frequent data without repeatedly inflating a cuff. But real-world performance can change with:

  • Movement, exercise, talking, and sleep;
  • Posture and whether the arm is supported;
  • Watch position, strap tension, sweat, and sensor contact;
  • Skin and tissue characteristics that affect optical signals;
  • Calibration age and changes in the user’s physiology;
  • Medication changes and other health conditions.

A device may perform acceptably in a controlled validation study yet behave differently during daily activity, after exercise, during sleep, or after a change in medication. More frequent readings are not automatically better readings: unreliable data can create false reassurance, unnecessary alarm, or confusion.

If a watch displays an unexpected high or low result, confirm it with a properly used upper-arm cuff. Do not change medication based on a smartwatch estimate alone.

Which option fits your situation?

What to check before buying

  • Does the product produce numbers, show a trend, or only issue an alert?
  • Is the exact feature authorized for the intended claim in your country?
  • Does it require calibration, and how often?
  • Is a separate upper-arm cuff mandatory?
  • Is independent validation available for the exact model and use case?
  • Does it work with your phone, watch generation, software, and region?
  • Can you export or share readings with a clinician?
  • Does the feature exclude pregnancy, diagnosed hypertension, younger users, or other groups?
  • What does the device do when it cannot obtain a reliable signal?
  • Does the manufacturer clearly distinguish wellness tracking from medical use?

Practical measurement checklist

For any reading that may influence a health decision:

  1. Use a validated upper-arm cuff as the reference device.
  2. Rest quietly for at least five minutes.
  3. Avoid caffeine, alcohol, smoking, exercise, and bathing for at least 30 minutes beforehand when following Samsung’s procedure.
  4. Sit with your back supported, feet flat, legs uncrossed, and arm supported.
  5. Keep still and do not talk during the reading.
  6. Repeat a concerning result with the cuff rather than relying on the watch.
  7. Record the time, reading, circumstances, and any recent medication or activity changes.
  8. Discuss persistent or concerning readings with a healthcare professional.

Chest pain, pressure or tightness, severe shortness of breath, sudden weakness, confusion, trouble speaking, or other emergency symptoms require emergency medical services. Do not wait for another smartwatch reading; Apple also warns that Apple Watch cannot detect a heart attack.

What is coming next?

Researchers are investigating ways to improve cuffless sensing, including PPG combined with machine learning, pulse-transit-time estimation, ECG plus PPG, bioimpedance, ultrasound, radio-frequency or near-field sensing, and miniature inflatable cuffs.

One February 2026 report from IEEE Spectrum described University of Texas research using reflected radio signals from the wrist to infer blood-pressure-related changes. The prototype used a wrist-strapped antenna and custom electronics; it was still undergoing development and was not a consumer smartwatch product. Research-stage sensing should not be confused with a commercially available, clinically ready monitor.

Verdict

For clinical decisions, choose a validated upper-arm cuff. For Android users who want convenience, Samsung’s compatible Galaxy Watch system can provide cuff-calibrated estimates, provided you maintain the required 28-day calibration and cross-check results. For iPhone users who want passive screening, Apple’s Hypertension Notifications feature can flag a possible pattern, but it does not provide blood-pressure numbers.

Be wary of any unsupported watch or app that promises effortless, cuff-free blood-pressure measurement without clearly identifying its method, authorization, validation, limitations, and intended use.
