How to Enable ECG and Blood Pressure on the Samsung Galaxy Watch 4

Galaxy Watch4 ECG and blood-pressure monitoring require Samsung Health Monitor, a compatible Samsung Galaxy phone, current software, and regional eligibility. Learn the exact setup, cuff calibration, recording technique, and fixes for missing or failed readings.

The steps below cover compatibility, installation, ECG recording, blood-pressure calibration, recurring measurements, and the most common reasons a feature is missing or a reading fails. Samsung Health Monitor is a wellness and monitoring tool; it does not replace a clinical ECG, a diagnostic blood-pressure monitor, a doctor, or emergency care.

Check compatibility before trying to enable the features

Start with this checklist. If even one of these conditions is not met, Samsung Health Monitor may be unavailable, or ECG and blood pressure may not appear after installation.

Samsung has older support pages that mention legacy requirements such as Android 7 or Wear OS 3. Those figures describe earlier software or feature versions. For a current Watch4 setup, use the requirements in Samsung’s blood-pressure support documentation and the current Health Monitor compatibility guidance.

Check country and device-region availability

Samsung treats ECG and blood pressure as regulated health or medical-device functions in some markets. Availability is therefore controlled by country, software, and sometimes the sales region of both the watch and phone. A technically compatible Watch4 can still show no ECG, no Blood pressure option, or a message that there is no compatible watch.

Check Samsung’s current country and device availability list rather than relying on an old Watch4 article. The document was updated July 13, 2026 and lists the United States for ECG and blood-pressure support on Watch4 and later models. Samsung began the phased US blood-pressure rollout on March 31, 2026, so older pages saying that blood pressure is unavailable in the United States are now date-stamped information, not current universal guidance.

Imported watches and phones deserve special attention. Samsung says Health Monitor may work only when the watch and smartphone were purchased in a country where the service is available. A phone bought for one market and a watch bought for another can therefore create a region mismatch. Samsung also warns that the service can become restricted after a factory reset or after deleting and reinstalling the app while traveling in a country where the service is not available. Do not treat a region mismatch as a sensor failure.

Pair the Watch4 with your Samsung Galaxy phone

Health Monitor depends on the supported Samsung pairing path. Pairing the watch only from the phone’s ordinary Bluetooth settings is not enough.

  1. Turn on Bluetooth on the Samsung Galaxy phone.
  2. Install or open Galaxy Wearable.
  3. Tap Start, or choose the option to add a new device.
  4. Select the Galaxy Watch4 or Galaxy Watch4 Classic.
  5. Confirm the pairing code if one appears, then complete the on-screen setup.
  6. Keep the watch near the phone and connected while installing apps and completing Health Monitor setup.

The Watch4 uses Wear OS and cannot be set up as an iPhone-compatible watch for these features. Samsung’s pairing instructions cover the initial connection.

Update the watch, phone, and apps

Missing Health Monitor options are often caused by an incomplete update rather than a disabled sensor. Update before troubleshooting further.

  1. Open Galaxy Wearable on the phone.
  2. Tap Watch settings.
  3. Tap Watch software update.
  4. Tap Download and install.
  5. Install available updates for the phone and Galaxy Wearable.
  6. On the watch, open the Play Store, go to My apps, and update Samsung Health Monitor and other pending apps if Samsung Health Monitor is not current.

Menu names can vary slightly by software version. Samsung documents the update process at Galaxy Watch software and app updates. For blood pressure, verify that the watch has reached Wear OS 4.0 or later; a Watch4 that has not received or installed the required update may not qualify even though the model contains the necessary sensors.

Install and initialize Samsung Health Monitor

The ordinary Samsung Health app is not the setup app for these two features. Samsung Health may display or use synced health information, but ECG and blood-pressure onboarding takes place in Samsung Health Monitor.

  1. Open Samsung Health Monitor on the watch.
  2. Follow the prompt to download or open the phone app.
  3. Install Samsung Health Monitor on the connected Samsung Galaxy phone, usually through the Galaxy Store, if prompted.
  4. Open the phone app.
  5. Accept the terms and privacy information.
  6. Grant the requested permissions.
  7. Create or complete the Health Monitor profile.
  8. Allow the app to discover and connect to the paired watch.
  9. Complete the on-screen setup.
  10. Confirm which wrist normally wears the watch.

If Samsung Health Monitor does not appear at all, do not assume it can be installed on any Android device. Samsung restricts the app to supported Galaxy phones and eligible watch, software, and regional combinations. Enable and record an ECG

Configure ECG on the phone

  1. Open Samsung Health Monitor on the phone.
  2. Tap ECG.
  3. Tap Get started if that button is shown.
  4. Read and complete the instructions, including the wrist selection and permissions.
  5. Open Samsung Health Monitor on the watch and confirm that ECG is now available.

Record the ECG on the watch

  1. Rest quietly for approximately 5 to 10 minutes before recording.
  2. Clean and dry the sensor on the back of the watch, your wrist, and the fingertip you will use.
  3. Wear the watch snugly, slightly above the wrist bone, with the sensor lying flat against the skin.
  4. Sit down and rest both forearms on a table or another flat surface.
  5. Open Samsung Health Monitor on the watch.
  6. Tap ECG.
  7. Lightly place a finger from the opposite hand over the watch’s top or Home button.
  8. Cover the button fully, but do not press it. Pressing the button can stop the recording.
  9. Remain still and silent for approximately 30 seconds. Breathe normally.
  10. Review the result, add symptoms if appropriate, then tap Save and Done.

What ECG results mean

Samsung Health Monitor can present classifications such as:

  • Sinus rhythm
  • Atrial fibrillation
  • High heart rate
  • Atrial fibrillation with high heart rate
  • Inconclusive
  • Poor recording

On Samsung’s current US page, sinus-rhythm and AFib classifications are described around 50–99 beats per minute, while high-heart-rate classifications are described from 100–150 beats per minute. These are the app’s supported classification ranges, not clinical diagnostic thresholds and not proof that a heart is healthy or unhealthy.

A sinus-rhythm result does not rule out an arrhythmia or another heart condition. The ECG app cannot diagnose a heart attack, and it cannot replace a clinical ECG, medical evaluation, or treatment. Samsung’s instructions also exclude users under 22, people with known arrhythmias other than atrial fibrillation, and people with an implanted pacemaker, implanted cardiac defibrillator, or another implanted electronic device from using the ECG app.

View, sync, and share an ECG report

ECG results normally synchronize with the paired phone. To force a sync, open the phone app and use More options > Settings > Accessories > Sync data. If the result is present on the watch but not the phone, first check that Bluetooth is connected and that the watch is still paired through Galaxy Wearable.

To create a report, open the ECG history on the phone, select the recording, and tap Share this report. Samsung provides a PDF that can be sent or shown to a healthcare professional. Samsung says reports are stored locally until you delete them or reset the device, so save anything you need before removing app data or resetting the watch.

Enable and calibrate blood-pressure monitoring

Blood pressure is not fully enabled just because the app is installed. The Watch4 first has to be calibrated against an upper-arm cuff. Samsung requires three cuff measurements within 30 minutes, followed by recalibration every 28 days.

Prepare for calibration

For at least 30 minutes before calibration:

  • Do not smoke, exercise, bathe, or consume caffeine or alcohol.
  • Keep the arm and wrist dry and free of lotion or perspiration.
  • Use a quiet indoor location.
  • Sit with your back supported, legs uncrossed, and feet flat on the floor.
  • Rest the arm and forearm on a table.
  • Once seated, rest quietly for at least 5 minutes before beginning.
  • Wear the watch snugly, but not so tightly that it is uncomfortable.

Use a separate upper-arm cuff monitor, not a wrist cuff. Put the cuff on one arm and wear the Galaxy Watch on the wrist of the opposite arm. Keep the phone within reach and use the same watch wrist for later measurements.

Calibrate the watch

  1. Put the upper-arm cuff on one arm according to the cuff manufacturer’s instructions.
  2. Wear the Watch4 on the opposite wrist.
  3. Open Samsung Health Monitor on the phone.
  4. Tap Blood pressure.
  5. Tap Calibrate the watch.
  6. Start the cuff measurement when the app instructs you to do so.
  7. Remain still and silent during the reading. Breathe normally.
  8. Enter the cuff reading into Samsung Health Monitor when prompted.
  9. Repeat until all three cuff readings are completed within 30 minutes.
  10. Finish the calibration in the app.

Samsung lists a calibration operating range of 80–170 mmHg systolic and 50–110 mmHg diastolic. Its listed measurement range is 70–180 mmHg systolic and 40–120 mmHg diastolic. These are app operating ranges, not healthy, unhealthy, safe, or emergency blood-pressure ranges.

Take a blood-pressure reading after calibration

  1. Wear the watch on the same wrist used during calibration.
  2. Repeat the quiet-rest and pre-measurement precautions.
  3. Open Samsung Health Monitor on the watch.
  4. Tap Blood pressure.
  5. Tap Measure.
  6. Read the prompt and tap OK.
  7. Keep the arm still and do not talk until the measurement finishes.
  8. Review systolic pressure, diastolic pressure, and pulse on the watch.
  9. Open Samsung Health Monitor on the phone to review the synchronized result.

The cuff is not required for every ordinary watch reading after calibration. It is required again when the 28-day recalibration interval arrives. The watch uses optical sensor signals to estimate blood pressure after calibration; it is not independently measuring pressure the way an upper-arm cuff does.

Troubleshooting common problems

Samsung Health Monitor is missing

Work through these checks in order:

  1. Confirm that the phone is a Samsung Galaxy phone.
  2. Confirm Android 12 or later.
  3. Confirm that the watch is a Watch4, Watch4 Classic, or later supported model.
  4. Confirm Wear OS 4.0 or later for blood-pressure support.
  5. Pair the watch through Galaxy Wearable, not only through Bluetooth settings.
  6. Update the phone, Galaxy Wearable, watch software, and watch apps.
  7. Open Samsung Health Monitor on the watch and use its prompt to open the phone app.
  8. Check Samsung’s current country list.
  9. Check whether the phone and watch were purchased for different regions.
  10. Restart both devices, then try the setup again.

If the app is still unavailable, the likely causes are regional eligibility, a cross-region device combination, unsupported phone hardware, or software that has not reached the required version. Samsung’s missing Health Monitor troubleshooting page is the official next reference.

The ECG option is missing after installation

Open Samsung Health Monitor on the phone and complete the ECG onboarding under ECG > Get started. Then open the app on the watch. If ECG still does not appear, verify the Galaxy phone requirement, Android version, watch update, country availability, and device-region match. ECG repeatedly says Poor recording

Poor recording usually means the electrical contact was not stable enough or the user moved during the 30-second capture. Try the following:

  • Clean and dry the rear watch sensor, wrist, and fingertip.
  • Warm a cold wrist before trying again.
  • Wear the watch snugly and slightly above the wrist bone.
  • Rest both forearms on a table.
  • Cover the top button completely with the opposite fingertip, without pressing it.
  • Do not talk or move during the recording.
  • Confirm the selected wrist in Samsung Health Monitor.
  • Rest for another 5 to 10 minutes and retry.

ECG says Inconclusive

An Inconclusive result can occur when the heart rate is outside the app’s supported classification range or when the rhythm cannot be classified as sinus rhythm or AFib. Rest for 5 to 10 minutes and try again with clean, dry contact and no movement. If inconclusive results continue, discuss them with a qualified healthcare professional rather than repeatedly relying on the watch for an interpretation.

The ECG waveform is upside down

The wrist setting is probably wrong. On the phone, open More options > Accessories > Wrist selection, choose the wrist actually wearing the watch, and record the ECG again. Samsung’s wrist-selection troubleshooting page covers this symptom.

Blood pressure calibration will not finish

Check each of the following:

  • Use an upper-arm cuff, not a wrist cuff.
  • Place the cuff on one arm and the watch on the opposite wrist.
  • Rest quietly for at least 5 minutes before starting.
  • Do not talk or move during any cuff measurement.
  • Complete all three readings within 30 minutes.
  • Keep the sensor and skin clean and dry.
  • Wear the watch snugly, but not too tightly.
  • Confirm that the readings fall within Samsung’s stated calibration range.
  • Keep the phone and watch connected throughout calibration.
  • Confirm Wear OS 4.0 or later.
  • Confirm that blood pressure is available for the device’s sales region.

Calibration is not a one-time unlock. If more than 28 days have passed, Samsung Health Monitor will require recalibration before continued use. Follow Samsung’s blood-pressure calibration instructions if the app’s prompts differ slightly from these labels.

Results are not appearing on the phone

Keep the watch near the phone, confirm Bluetooth is connected, and make sure the watch remains paired in Galaxy Wearable. For ECG, open the phone app and choose More options > Settings > Accessories > Sync data. Also check that Samsung Health Monitor has the permissions it requested and that there is enough free storage. Samsung provides a separate guide for ECG results that do not appear or sync.

An imported watch and phone do not work together

Check the sales region of both devices and compare it with Samsung’s current country list. A US phone with an imported watch, or the reverse, can leave the hardware paired while Health Monitor remains unavailable. Because Samsung applies regional controls to these features, changing the phone’s language or location is not the same as making the device combination eligible.

A US reader still cannot see blood pressure

The United States is listed for Watch4-and-later blood-pressure support in Samsung’s July 13, 2026 availability document, following the phased rollout that began March 31, 2026. A missing option in the US can therefore reflect an outdated watch or app, an incomplete rollout, a non-US watch or phone, a region mismatch, or an uninitialized Health Monitor installation. Update both devices, complete the official setup, and check the device-region combination before concluding that the model is unsupported.

Do not rely on unofficial region-unlock packages

Older search results may recommend modified Samsung Health Monitor packages, ADB commands, or region-changing procedures. These are not Samsung’s supported setup path. For a health-related app, unofficial modifications can create compatibility, privacy, update, data-integrity, or warranty concerns, and their behavior can change after an app or watch update. The reliable workflow is to use Samsung Health Monitor from Samsung’s supported app distribution and resolve eligibility, software, pairing, and regional issues through the official path.

What the Watch4 can and cannot tell you

  • ECG: The watch records a single-lead electrical signal and can classify some recordings. It is not a clinical 12-lead ECG and cannot diagnose a heart attack.
  • Blood pressure: The watch estimates systolic pressure, diastolic pressure, and pulse using optical signals after calibration against an upper-arm cuff. It is not a replacement for a diagnostic cuff monitor.
  • Wellness versus diagnosis: Samsung describes blood-pressure monitoring as a wellness and trend-monitoring function. Do not diagnose hypertension, change medication, or adjust a dosage from a watch reading.
  • AFib notifications versus an ECG: Any passive irregular-rhythm feature is not the same as recording an ECG and does not detect every episode. A watch notification or a normal result should be discussed with a healthcare professional when symptoms or concerns exist.

Do not use these functions as a substitute for medical advice. If a reading is concerning, repeated, or accompanied by symptoms, contact a qualified healthcare professional. Does the Galaxy Watch4 work with an iPhone for ECG or blood pressure?

No. The Wear OS-based Galaxy Watch4 is not supported for Samsung Health Monitor ECG and blood pressure setup with an iPhone.

Is blood pressure available on the Galaxy Watch4 in the United States?

Why is my ECG result Inconclusive?

No. Samsung describes blood pressure as a wellness monitoring function, and the ECG app cannot diagnose a heart attack. Do not change medication from a watch reading. Seek professional or emergency care when symptoms or readings are concerning.

What should I do if my watch and phone were bought in different countries?

Check Samsung’s current country and device availability list. Health Monitor may be restricted when the watch and phone come from different regional markets, even if the watch hardware is otherwise compatible.

For a supported Watch4 setup, pair the watch through Galaxy Wearable, update it to the current software, install Samsung Health Monitor, and complete its profile and wrist setup. Record ECGs with light opposite-finger contact on the top button for about 30 seconds. For blood pressure, use an upper-arm cuff for three calibration readings, then recalibrate every 28 days.
