Huawei TruSense and Cough-Based Health Checks: Can a Smartwatch Really Spot Respiratory Problems?

Huawei’s cough-based Respiratory Check is an intriguing smartwatch wellness feature, but it is not continuous cough monitoring or a medical diagnosis. Here is what it measures, which watches support it, and how to interpret the result.

Huawei smartwatches can perform a guided cough-based respiratory assessment on supported models and in supported markets. You sit still, take a deep breath, cough near the watch several times, and receive a broad result such as “good,” “average,” or “poor.”

That is not the same as TruSense diagnosing pneumonia, asthma, COPD, COVID-19, or another respiratory disease. TruSense is Huawei’s wider health-sensing platform; the cough assessment is a separate Health Glance or Respiratory Health feature, and Huawei does not publish clinical accuracy figures for it in the documentation reviewed here.

What Huawei TruSense actually is

TruSense is Huawei’s branded health-monitoring architecture, not a single cough sensor. It combines watch hardware, signal processing, and software algorithms to interpret health-related signals.

Depending on the model, that platform can support optical measurements such as heart rate and blood-oxygen-related readings, electrical measurements such as ECG, sleep and breathing analysis, temperature-related sensing, and other Health Glance functions. Huawei describes TruSense as part of a broader system that connects sensors and detection technologies with applications such as TruSleep and health reports. See Huawei’s European Health Report and the WATCH 5 product page.

Google Pixel Watch 5 (45mm) - Android Smartwatch - Obsidian - Wi-Fi
  • The new Google Pixel Watch 5 offers a smarter way to a healthier you.[1,2]; Designed for Gemini Intelligence, get proactive help, and personalized insights from Google Health[3,4]; watch is compatible with Android devices only; not compatible with iOS devices
  • Precision-crafted to do it all: The Google Pixel Watch 5 features the domed Actua 360 display with scratch-resistant Gorilla Glass and up to 40 hours of battery life[1,5]
  • Metrics that matter: Advanced sensors help you stay ahead of your health and wellness; and Google Pixel’s most accurate heart rate tracking is fine-tuned for your activity[6,7]
  • Understand your sleep like never before: Your watch captures insights like the time spent in sleep stages to breathing regularity, then summarizes it all in a personalized Sleep Score[8,9]
  • Detects loss of pulse, then calls for help: Pixel Watch 5 is the only smartwatch to feature Loss of Pulse Detection, which can prompt a call to emergency services[10]

It is therefore more accurate to say that a supported Huawei watch uses TruSense-era sensing and algorithms alongside a microphone-based cough workflow than to say “TruSense listens for respiratory disease.”

How the cough-based respiratory assessment works

Huawei’s documented process is a deliberate, user-initiated spot check—not continuous automatic listening for every cough during the day.

  1. Open the respiratory-health, lung-function, or Health Glance feature in the watch or Huawei Health app.
  2. Sit down and remain still.
  3. Move to a quiet environment and wear the watch snugly but comfortably.
  4. Take a deep breath.
  5. Cough forcefully approximately 30 centimetres from the watch.
  6. Repeat the cough two or three times, following the on-screen instructions.
  7. Wait while the watch analyzes the recorded sound and produces its assessment.

Menu names vary. Huawei support documentation refers to both “Respiratory Study” and lung-function assessment, while product pages may use “Respiratory Check” within Health Glance. Look in the Health section of the watch and Huawei Health app rather than relying on one universal menu path. The official procedure is described in Huawei’s support article.

What the watch measures—and what it does not

Huawei says the watch assesses lung function by analyzing the sound of the cough. In practical terms, the microphone captures an intentionally produced cough and software evaluates acoustic characteristics in the recording. Other health signals or contextual information may be part of the wider implementation, but Huawei’s public documentation does not provide enough technical detail to describe the exact algorithm.

It should not be presented as a direct measurement of lung capacity, airway obstruction, or blood-gas exchange. The result is a broad category—typically “good,” “average,” or “poor”—rather than a spirometry value or named medical finding.

Research supports the general idea that cough audio can contain useful respiratory information. Studies have explored cough detection and classification using smartwatch microphones and machine learning, including smartwatch cough-detection research and the CoughSense respiratory-classification study. Those studies make the technology plausible, but they do not validate Huawei’s commercial algorithm or establish its clinical performance.

Which Huawei watches support it?

Huawei support and product material indicate availability on at least some versions of the following watches:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • HUAWEI WATCH 4
  • HUAWEI WATCH 4 Pro
  • HUAWEI WATCH 4 Pro Space Edition
  • HUAWEI WATCH 5
  • HUAWEI WATCH Ultimate 2
  • Selected WATCH Ultimate Design variants

This is not a guarantee that every listed model has the feature everywhere. Availability can depend on the exact model, country, firmware, Huawei Health region, language, regulatory status, and whether the watch is paired with Android or iOS. Huawei’s WATCH 5 compatibility information illustrates how features can differ by platform and market.

The WATCH 5 is the clearest current example: Huawei advertises Health Glance reports including Respiratory Check and Sleep Breathing Awareness on its UK product page. However, Health Glance is unavailable while Battery Saver mode is active on that model, according to Huawei.

What to check before buying

  • Open the local Huawei product page—not just a global page—and confirm that Respiratory Check is listed.
  • Check the model-specific support documentation for your country.
  • Confirm Android or iPhone compatibility and feature differences.
  • Update Huawei Health and the watch firmware.
  • Do not assume that a watch carrying the TruSense branding includes the cough assessment.

How accurate is Huawei’s cough assessment?

The most important limitation is the lack of a published performance number in the reviewed Huawei sources. There is no verified sensitivity, specificity, accuracy percentage, confidence interval, or comparison with clinical spirometry to cite for this feature.

That means the result should be treated as a wellness-oriented indication, not a validated diagnosis or a reliable rule-out test. A “good” result cannot exclude respiratory disease, and a “poor” result does not prove that disease is present.

Conditions during the test matter. Huawei warns that results may be affected by:

  • Colds, pneumonia, and other acute respiratory illness
  • Smoking
  • Alcohol consumption
  • Recent intense exercise
  • A loose or incorrectly positioned watch
  • Background noise
  • A cough that is too quiet
  • Failure to maintain the recommended distance

This creates an important paradox: the feature may be harder to interpret when someone is actively ill, even though that is when they are most likely to try it.

What “revolutionizing” means—and what it does not

Cough analysis is an interesting development because it turns an ordinary smartwatch microphone into a potential health sensor. It may make a respiratory check more accessible, less intimidating, and easier to repeat than visiting a dedicated instrument for every informal wellness check. Combined with a broader health dashboard, it could help users notice changes or decide when to ask a clinician for advice.

It does not replace:

  • Clinical spirometry
  • A clinician’s examination
  • Appropriately used pulse oximetry
  • Chest imaging or laboratory testing
  • Formal sleep-apnea testing

The FDA explains that intended use and medical claims help determine whether a product is regulated as a medical device. A general wellness feature and a device intended to diagnose or manage disease should not be treated as interchangeable.

What to do with an abnormal result

  1. If the result seems inconsistent with how you feel, repeat the assessment later under the recommended conditions: seated, still, quiet, correctly fitted, and with the cough at the stated distance.
  2. If a poor result persists, or you have ongoing symptoms, contact a healthcare professional.
  3. Do not postpone medical care because the watch reports “good.”
  4. Do not interpret “poor” as proof of a specific condition.

Seek urgent medical attention for severe difficulty breathing, chest pain, bluish lips, confusion, or rapidly worsening symptoms. A smartwatch result should never override serious symptoms.

Huawei WATCH 5: who should buy it?

The WATCH 5 is the most natural Huawei choice for buyers who want Health Glance, Respiratory Check, ECG-related functions, fingertip SpO2, and other advanced wellness features in one watch. Huawei lists up to 4.5 days of typical use for the 46 mm model under stated conditions, with lower figures in some always-on-display and iPhone-pairing scenarios. Battery and features vary by configuration, so check the official local product page.

Buy it for the overall smartwatch, fitness, battery, and health-dashboard experience—not solely for cough analysis. It is a poor fit if your primary goal is a medical diagnosis, full iPhone feature parity, long battery life above all else, or a dedicated respiratory test.

Premium WATCH Ultimate and Ultimate Design models may also offer Health Glance features, but their size, price, and regional availability make them difficult to justify if the cough check is the only attraction. Huawei’s Ultimate Design page should be checked for the exact market and model.

Do not assume the more fitness-oriented WATCH FIT range supports this particular cough assessment simply because it uses TruSense branding. The reviewed official material does not establish that the cough feature is available on every such model.

Privacy questions worth checking

A cough recording can be sensitive health-related data. The available product and support sources establish how the feature works and that compatibility varies, but they do not establish a complete, model-specific answer to every privacy question.

Before enabling the feature, check Huawei’s current privacy documentation for your region and verify:

  • Whether audio is processed locally or uploaded
  • How long recordings or derived data are retained
  • Whether results synchronize with Huawei Health cloud services
  • Which Huawei ID region controls storage and feature access
  • How respiratory data can be deleted
  • Whether Android and iPhone versions handle the data differently

How it compares with alternatives

Tool Best suited to What it is not
Huawei cough assessment Consumer wellness spot checks Diagnostic spirometry or disease confirmation
Samsung Galaxy Watch sleep-apnea feature Sleep-apnea risk assessment on compatible watch-and-phone combinations A general cough or lung-function test
WatchPAT ONE Dedicated home testing for suspected sleep-related breathing disorders A general-purpose smartwatch or cough-analysis product
Clinical spirometry Measuring airflow and lung function under clinical protocols A casual daily smartwatch feature

Samsung’s feature has an FDA authorization for its specific sleep-apnea use case, while WatchPAT ONE is a dedicated home sleep-testing device. Their regulatory status and intended uses do not make them direct substitutes for Huawei’s cough check. The FDA’s sensor-based digital-health list is a useful reminder to compare the precise measurement and intended use, not merely the presence of a microphone or oxygen sensor.

Verdict

Huawei’s cough-based assessment is a promising example of consumer wearables using microphones and algorithms for respiratory-health screening. It is convenient, accessible, and potentially useful as a prompt to pay attention to changing health signals.

But the accurate headline is narrower than the marketing claim: Huawei TruSense does not, by itself, diagnose respiratory problems from spontaneous coughs. On supported watches, a guided Respiratory Check analyzes a deliberately recorded cough and returns a broad wellness assessment. Because Huawei does not publish clinical accuracy figures for the feature, treat it as an optional health insight—not as a replacement for medical testing or professional advice.
