Yes, the technology is real—but it is not a smartwatch, a consumer health tracker, or a proven cancer test. Researchers at ETH Zurich and Empa developed MenstruAI, a passive diagnostic prototype integrated into a standard menstrual pad. It uses microfluidics and paper-based lateral-flow assays to detect signals associated with C-reactive protein (CRP), carcinoembryonic antigen (CEA), and cancer antigen 125 (CA-125) in menstrual blood.
The important qualification is that MenstruAI demonstrated biomarker detection, not disease diagnosis. It remains a research platform rather than a verified retail product that people can buy and use for screening.

What MenstruAI actually is
MenstruAI is better described as a pad-integrated, passive wearable assay than as an electronic wearable. There is no indication that it continuously measures data like a smartwatch or requires onboard Bluetooth electronics.
- A small microfluidic device is placed in or attached to a sanitary pad.
- Menstrual blood enters through a controlled inlet.
- Pressure gradients and capillary flow meter the sample.
- The sample reaches embedded lateral-flow test strips.
- Antibody-linked particles produce visible color changes.
- The result can be read by eye or analyzed with a smartphone application.
The platform and its demonstrated markers are described in the researchers’ peer-reviewed publication. An earlier version of the work appeared as a medRxiv preprint.

Which health markers can it detect?

The research reported semi-quantitative detection. That means the signal can provide an approximate indication of the amount of a target biomarker, but it is not equivalent to a calibrated laboratory measurement.
How the test works
MenstruAI uses a lateral-flow assay, the same broad paper-based technology used in many rapid tests. Antibodies on a test strip bind to a target molecule. A particle label—such as gold nanoparticles—creates a visible colored signal when binding occurs. In general, a stronger signal can correspond to more target material, but interpretation depends on the assay design, sample volume, timing, and calibration.
Menstrual blood makes that engineering problem more difficult than a simple laboratory liquid sample. It can contain cells, uterine-lining tissue, clots, and other material. Its strong red color may interfere with optical readout, while too little or too much sample can affect the strip. The MenstruAI design uses fluidic control and assay configuration to manage these issues, but solving them in a prototype is not the same as proving reliable performance in routine use.
Why menstrual blood is an interesting sample
Menstrual blood is biologically more complex than blood from a small skin cut. It includes blood along with tissue, cells, proteins, and other biological material. Researchers are studying whether this combination can provide information about reproductive, inflammatory, metabolic, and oncological processes. A 2026 Nature Reviews Bioengineering article describes the broader potential while also highlighting the field’s early stage.
Its potential advantages are practical:
- Collection happens during a normal monthly process.
- No needle or clinic visit is required to obtain the sample.
- Repeated sampling could eventually show changes across menstrual cycles.
- A pad-based format may be more familiar than a dedicated medical device.
These are reasons to investigate the sample, not evidence that every analyte can be measured reliably or that a future result will be clinically useful.

How strong is the evidence?
The work is meaningful proof of technical feasibility. The researchers assessed the assays using menstrual blood and described a volunteer-worn prototype embedded in a pad. The underlying study also examined relevant biomarker behavior in relation to venous-blood testing and tested the assays in unprocessed blood.
That evidence does not answer the questions required for a medical diagnostic. The available research does not establish:
- Clinical sensitivity and specificity for cancer, endometriosis, infection, or another disease.
- False-positive and false-negative rates in ordinary users.
- Validated thresholds that tell a clinician how to act on a result.
- Performance across different ages, menstrual flows, cycle timings, and health conditions.
- Whether using the device improves diagnosis, treatment, or health outcomes.
The authors also disclosed an ETH Zurich/Empa patent application. A patent application signals intellectual-property activity; it does not establish regulatory approval, commercial readiness, or clinical effectiveness.
Why a positive result would not prove disease
CRP, CEA, and CA-125 can change for multiple reasons. Inflammation, infection, benign conditions, medication, menstruation-related physiology, and other factors may influence biomarker levels. A marker associated with a disease is therefore not the same thing as a disease-specific signal.
For a future result to be medically useful, it would need a validated threshold, an appropriate reference range, and a clear follow-up pathway. An abnormal reading might eventually help with monitoring or triage, but it would require confirmation through established clinical evaluation, which could include examination, venous blood tests, imaging, pathology, or other procedures. A negative result could also create false reassurance if the test lacked adequate sensitivity.

Menstrual blood is not automatically interchangeable with venous blood
Researchers have reported correlations between menstrual and venous blood for some analytes. But correlation does not mean that the two samples can use the same reference ranges or substitute for one another.

A clinically reliable test would need to establish sample-specific analytical accuracy and reproducibility—not merely show that a molecule is present.
Real-world failure points
A pad-based test would have to work across conditions that are difficult to standardize:
- Very light flow may not provide enough sample.
- Heavy flow or clots could overwhelm or obstruct the fluidic pathway.
- Tissue may interfere with flow or color interpretation.
- Different pad materials could absorb blood before it reaches the sensor.
- Reading too early or too late could produce an invalid result.
- Heat, humidity, and storage could degrade the test components.
- Biomarker concentrations may vary across the days of a period and between cycles.
A device designed for a pad also should not be assumed to work with tampons, menstrual cups, discs, or period underwear. Those products collect or absorb menstrual fluid differently.

There are adjacent commercial technologies, but they do different things. For example, Emm describes a smart menstrual cup that tracks flow, duration, and frequency. In a June 2026 announcement, the company said its product had received UKCA marking and MHRA registration in the United Kingdom; that announcement is not evidence that the cup performs MenstruAI’s CRP, CEA, or CA-125 assay.
Tulipon presents a menstrual-fluid collection and diagnostics-oriented platform, but the available information does not establish a clearly purchasable, clinically validated home biomarker test. Research sample-collection programs are likewise not equivalent to an instant consumer wearable.
What must happen before clinical use?
Before a technology like this could support medical decisions, developers would need to demonstrate:
- Analytical validity: accurate, repeatable measurement of each biomarker.
- Clinical validity: evidence that results distinguish relevant clinical states in appropriately designed studies.
- Clinical utility: proof that using the test improves decisions or outcomes.
- Standardization: performance across flow levels, cycle timing, clots, tissue, pads, storage, and temperature.
- Usability: clear procedures for positioning, reading, invalid results, and disposal.
- Regulatory review: authorization appropriate to the countries and diagnostic claims involved.
- Privacy safeguards: especially if smartphone analysis stores or shares sensitive reproductive-health data.
- Clinical follow-up: instructions that prevent an app result from being mistaken for a diagnosis.
Bottom line: promising wearable science, not a home diagnosis
MenstruAI shows that a menstrual pad can potentially collect menstrual blood and use paper-based assays to detect selected health-related biomarkers. That is a credible and important research result.

It does not show that the device detects cancer, diagnoses endometriosis, replaces venous blood testing, or is ready for consumers. For now, the technology belongs in the category of promising wearable health research. The decisive next step is large, diverse, prospective clinical validation with defined thresholds, real-world failure analysis, regulatory review, and a safe route from an abnormal signal to professional care.
