In October 2024, iOS 18.1 developer beta 6 enabled Apple Watch sleep-apnea notifications when paired with watchOS 11.1 beta 4. The iPhone did not detect sleep apnea by itself: compatible Apple Watches collected overnight movement data, while the iPhone’s Health app displayed the results and handled setup.
This is now a historical beta milestone, not a current software recommendation. The feature is a screening aid that can identify patterns suggestive of moderate-to-severe sleep apnea; it is not a medical diagnosis.
What changed in iOS 18.1 beta 6?
Apple’s iOS 18.1 developer beta 6, released on October 7, 2024, exposed sleep-apnea notification support for compatible Apple Watches. The companion requirement was watchOS 11.1 beta 4; updating only the iPhone was not sufficient. Earlier iOS 18.1 beta users might not have seen the feature because Apple had not yet fully enabled or exposed it.
The change depended on both devices. The Apple Watch gathered the overnight sensor data, and the iPhone stored, presented, and configured the feature through Health. Later stable releases superseded those beta builds. See the contemporaneous reports from MacRumors and 9to5Mac.

What are Breathing Disturbances?
Breathing Disturbances is Apple’s name for a nightly metric derived from small wrist movements associated with interruptions to normal breathing patterns during sleep. The Apple Watch uses its accelerometer; it does not directly measure airflow, and the feature does not require the blood-oxygen sensor.
Health presents readings as either:
- Elevated
- Not Elevated
Those labels are not a count of apnea events and should not be converted into a clinical apnea-hypopnea index. Apple provides trend views covering one month, six months, and one year. The company describes the mechanism and output in its health-feature announcement and watchOS 11 announcement.
How the sleep-apnea alert works
The system does not issue a meaningful alert after one unusual night. It evaluates data in consecutive 30-day windows and requires sufficient overnight readings. Apple’s user instructions specify at least 10 nights of watch wear within a 30-day period.

- The watch records Breathing Disturbances while you sleep.
- Health accumulates enough qualifying nights for an evaluation window.
- The software looks for a consistent pattern rather than relying on one elevated reading.
- If the pattern suggests signs of moderate-to-severe sleep apnea, you may receive a Possible Sleep Apnea notification.
An alert includes the relevant period and educational information. Apple also provides a report that can be exported as a PDF, containing three months of Breathing Disturbances data and notification details. The FDA clearance describes the feature’s intended use and 30-day analysis in document K240929.

For the October 2024 beta rollout, Apple listed:
- Apple Watch Series 9
- Apple Watch Series 10
- Apple Watch Ultra 2
You also needed a compatible iPhone, the corresponding iOS and watchOS software, sleep tracking configured, and enough overnight wear. Apple Watch SE was not included in the original launch documentation. Later Apple Support documentation has expanded compatibility wording to include newer hardware, including Apple Watch SE 3, but that does not mean SE 3 supported the October 2024 beta. Check Apple’s current support page for the applicable software and regional requirements.
How to view Breathing Disturbances
In the later public interface, open:
- Health on your iPhone.
- Search or Browse, depending on the interface and region.
- Respiratory.
- Breathing Disturbances.
Beta labels and menu placement can change between builds, so a screenshot from iOS 18.1 beta 6 should not be treated as a permanent interface reference.

How to enable Sleep Apnea Notifications
- Open Health on the iPhone.
- Tap Search or Browse.
- Open Respiratory.
- Select Sleep Apnea Notifications.
- Tap Set Up.
- Review the information and confirm your age and whether you have already been diagnosed with sleep apnea.
- Complete setup, then wear the watch to sleep.
The intended population is adults aged 18 or older who have not already received a sleep-apnea diagnosis. Regional regulatory authorization also affected availability; the feature was not guaranteed to appear everywhere at the same time.
What an alert means—and what it does not
A notification means the watch detected a pattern that may be consistent with sleep apnea. It does not confirm that you have the condition, establish its severity, or prescribe treatment. Discuss the notification and its PDF report with a healthcare professional. Do not start or stop medication, begin treatment, or assume a diagnosis based only on the watch.

Apple explicitly cautions that not everyone with sleep apnea will receive one. Loud snoring, witnessed pauses in breathing, gasping during sleep, morning headaches, or excessive daytime sleepiness remain reasons to seek medical advice regardless of what the watch reports.

Breathing Disturbances can also be influenced by alcohol, medication, sleeping position, and other causes of disrupted sleep. Persistent symptoms should not be dismissed as an incidental reading.

No alert after several nights
This is expected. The feature needs enough data for its evaluation period, including at least 10 nights within 30 days. A few nights are not enough to draw the intended pattern-based conclusion.
No Breathing Disturbances data
- Confirm that sleep tracking is configured.
- Wear the watch consistently and snugly overnight.
- Check that the battery lasted through the night.
- Verify wrist detection and watch fit.
- Confirm that the iPhone and watch use compatible software.
- Look under Health > Respiratory, since beta menu labels may vary.
The watch is supported, but the feature is missing
Check the watch model, iOS and watchOS versions, region availability, completed onboarding, and the amount of overnight data. Regulatory authorization and rollout timing varied by country or region.

An elevated reading appears without an alert
That is not necessarily a malfunction. An elevated nightly classification is different from a Possible Sleep Apnea notification, which depends on a consistent pattern across the evaluation window.
Should you install an iOS or watchOS beta for this feature?

Apple describes beta software as preview software intended for testing and recommends reviewing release notes and maintaining a recovery path. Its guidance is available at Apple Developer Support. Because this feature required a second beta on the watch, installing it added another possible source of pairing or data problems.
Buying an Apple Watch for sleep-apnea notifications
If you are considering hardware specifically for this feature, the launch-supported Series 9, Series 10, and Ultra 2 are the relevant 2024 models. Series 10 offers newer hardware than Series 9, while Ultra 2 is better suited to buyers who also value its rugged design and larger battery. Buying an older or refurbished watch solely for screening may not make sense if you need newer hardware, long-term availability, or a smaller watch.
When considering refurbished stock, check battery health, Activation Lock status, warranty, return terms, iPhone compatibility, and regional feature availability. Current 2026 pricing and inventory vary and are not included here. No Apple Watch guarantees that sleep apnea will be detected, and none replaces a clinician-directed home sleep test or formal overnight sleep study.
What happened after the beta?
Apple announced the health feature in September 2024, enabled it for testers in iOS 18.1 beta 6 and watchOS 11.1 beta 4 on October 7, 2024, and later incorporated it into public software after regulatory clearances. The accurate current framing is therefore that iOS 18.1 beta 6 was the historical enablement point—not that it remains the latest iOS beta or a current installation target.
